per 100.000 kelahiran hidup.

Abortus juga merupakan salah satu faktor penyumbang

angka kematian ibu, namun lebih sering dilaporkan dalam
bentuk perdarahan bukan dalam bentuk abortus.

Macam-macam abortus
✖ Abortus Imminens
• Gejala :Perdarahan pervaginam, ostium uteri masih
tertutup, hasil konsepsi masih baik di dalam.
• Pemeriksaan :Ostium uteri masih tertutup, besarnya uterus masih
sesuai dengan umur kehamilan dan tes kehamilan melalui urin masih
positif. Pemeriksaan USG untuk melihat pertumbuhan janin dan
plasenta, ukuran biometri/ kantong gestasi apakah sudah sesuai dengan
HPHT.
• Tatalaksana : Tirah baring, tokolitik, prostaglandin

✖ Abortus Insipiens
• Tanda :Servix yang telah mendatar dan ostium uteri yang
telah membuka akan tetapi hasil konsepsi masih di dalam kavum uteri
dan dalam proses pengeluaran.
• Pemeriksaan :Tes urin positif. Pada pemeriksaan USG
didapatkan besarnya uterus masih sesuai dengan umur kehamilan,
gerak janin dan denyut jantung janin masih jelas walau mungkin
sudah mulai tidak normal, biasanya terlihat penipisan servix uterus
atau pembukaannya, plasenta
• Tatalaksana :Perbaikan keadaan umum, evakuasi/pengeluaran
hasil konsepsi, kuretase

✖ Abortus Inkompletus
• Tanda : Sebagian hasil konsepsi sudah keluar
dari kavum uteri dan sebagian masih ada yang tertinggal di
dalam uterus
• Pemeriksaan : Pemeriksaan vagina, kanalis servikalis
masih terbuka dan teraba jaringan pada kavum uteri atau
menonjol pada ostium uteri eksternum. Pemeriksaan USG
besar uterus sudah lebih kecil dari usia kehamilan dan
kantong gestasi sudah sulit dikenali, di uterus akan tampak
massa hiperkoik yang bentukannya tidak beraturan
• Tatalaksana : Perbaikan keadaan umum, kuretase

✖ Abortus Kompletus
• Tanda :Abortus yang terjadi dimana seluruh
hasil konsepsi telah keluar dari vakum uteri
• Pemeriksaan :Semua hasil konsepsi telah keluar,
ostium uteri telah menutup uterus sudah kembali
mengecil sehingga perdarahan sedikit. Besar uterus
tidak sesuai dengan umur kehamilan. Pada
pemeriksaan tes urin masih positif
• Tatalaksana :Roboransi dan hematenik bila
diperlukan

✖ Missed Abortion
• Tanda : Fetus telah meninggal di dalam
kandungan sebelum usia kehamilan 20 minggu dan hasil
konsepsi seluruhnya masih tertahan di dalam kandungan.
Tanda sekunder kehamilan pada payudara menghilang.
• Pemeriksaan :Rahim semakin mengecil. Tes urin negatif.
Pemeriksaan USG didapatkan uterus yang mengecil, kantong
gestasi yang mengecil, dan bentuknya tidak beraturan
disertai gambaran fetus yang tidak ada tanda-tanda
kehidupan
• Tatalaksana : <12 minggu dilatasi dan kuretase, >12
minggu induksi, kuretase

✖ Abortus Infeksiosus/Abortus Septik

• Tanda :Disertai dengan infeksi pada alat
genitalia. Abortus septik disertai penyebaran infeksi
pada peredaran darah tubuh atau peritoneum
(septikemia atau peritonitis). Panas tinggi, tampak
sakit dan lelah,
• Pemeriksaan :Takikardi, perdarahan pervaginam
yang berbau, uterus yang membesar dan lembut,
serta nyeri tekan. Pada hasil laboratorium didapatkan
leukositosis.
• Tatalaksana :Keseimbangan cairan tubuh,
antibiotik, kuretase

✖ Abortus Habitualis
• Tanda :Terjadi 3 kali berturut-turut. Penyebab
abortus habitualis selain kelainan anatomik banyak yang
mengaitkan dengan reaksi imunologik yaitu kegagalan reaksi
dengan antigen lymphocyte trophoblast cross reactive (TLX),
penyebab lain yang sering terjadi adalah inkompetensia servix,
dimana servix uteri tidak mampu menerima beban untuk tetap
menutup setelah melewati trimester pertama
• Pemeriksaan :Ostium servix akan membuka tanpa disertai
rasa mules/kontraksi rahim dan akhirnya terjadi pengeluaran
janin
• Tatalaksana :Fiksasi serviks, UK 12-14 minggu cara
SHIRODKAR atau McDONALD
