 Etiology

 Volvulus sigmoid  mesenterium yg panjang (A) dengan basis yang sempit (B)
 Diagnosis

 Nyeri kolik
 Di antara kolik terdapat nyeri perut menetap
 Perut sangat membesar karena distensi
 Kadang terdapat mual muntah
 Gambaran foto rontgen :
 dilatasi sigmoid coffee bean
 dengan barium, rektal terlihat tanda paruh burung
(bird’s beak sign)
 Perbedaan Volvolus Colon Sigmoid dan Caecal
Sigmoid Volvulus
 arises in the pelvis (left lower quadrant)
 extends towards the right upper quadrant
 ahaustral in appearance
 sigmoid volvulus causes obstruction of the
proximal large bowel, therefore the ascending,
transverse and descending colon may be dilated
 few air-fluid levels may be seen
Caecal Volvulus
 arises in the right lower quadrant
 extends towards the epigastrium or left upper
quadrant
 colonic haustral pattern is maintained
 distal colon is usually collapsed and the small
bowel is distended
 one air-fluid level may be seen

 Pada foto polos abdomen nampak gambaran biji kopi (coffee bean sign)

 Pada CT Scan nampak pola pusaran (whirled pattern) yang terbentuk dari
lengkungan aferen dan efferen usus yang berputar

soluble barium enema)

Menggambarkan penyempitan / pengurangan bertahap

dari kolon sigmoid hingga tingkat obstruksi selama
pemasangan kontras / barium ke rektum atau pada CT.

 Large gas-filled loop lacking haustra, forming a closed-loop obstruction

 Whirl sign: twisting of the mesentery and mesenteric vessels
 Bird's beak sign: if rectal contrast has been administered
 X-marks-the-spot sign: crossing loops of bowel at the site of the transition
 Split wall sign: mesenteric fat seen indenting or invaginating the wall of the bowel
