Saya akan ceritakan pengalaman yang saya lihat tentang proses triage sepanjang

saya bertugas di unit kecemasan Hospital Sipitang.pengalaman ini tidak bermaksud


untuk menyalahkan sesiapa cuma saya akan ceritakan masalah yang dihadapi
disebabkan proses triage yang tidak sistematik. Sepanjang saya bertugas disana
banyak masalah yang saya lihat disebabkan sistem triage.Salah satu masalah yang
pernah dihadapi adalah mistriaging.Pesakit sepatutnya yellow zone tetapi hanya
didaftarkan Green zone sahaja.Masalah ini berlaku disebabkan kaunter triage tidak
berfungsi.kenapa? ini berlaku disebabkan masalah kekurangan staf yang bertugas
dalam masa satu shif.Pertama kali posting di unit kecemasan saya terkejut sedikit
dan risau apabila dalam satu shif hanya 4 orang yang bertugas.( 2 orang MA,1 orang
Nurse,1 orang PPK).Kadang-kadang nurse itu adalah jururawat Masyarakat. Apabila
ada red zone,yellow zone sibuk dan disertakan lagi pesakit asma yang datang,semua
staf akan sibuk untuk merawat pesakit.kaunter triage tidak akan berfungsi dan
kaunter pendaftaran hanya dijaga oleh PPK.
Oleh itu,apabila ada pesakit yang datang ke kecemasan dan ingin mendaftar, PPK
yang bertugas di kaunter pendaftaran hanya akan mendaftar mengikut complaint
pesakit sahaja.Apabila keadaan ini berlaku, mistriaging akan berlaku kerana pesakit
tidak disaring dengan betul.Mistraging ini akan menyebabkan proses rawatan
pesakit terlambat dan boleh menyebabkan complication lain berlaku dan keadaan
pesakit akan menjadi teruk.konflik antara Doktor dan juga staf boleh berlaku
disebabkan masalah.
Isu ini telah dikemukakan kepada ketua unit agar mengambil tindakan untuk
menangani masalah ini.Masalah ini juga telah disuarakan kepada Pakar kecemasan
yang menjaga Hospital sipitang.Setakat ini,ketua unit menetapkan setiap shif MA
mesti ada 3 orang bertugas.Tindakan ini mengurangkan masalah berlaku namun
kadang-kadang perkara lain berlaku seperti missed assesement on patient
disebabkan pesakit tidak memberitahu dengan lengkap tentang history of presenting
illness.
Kesimpulannya,triage ini sangat penting supaya pesakit dapat menerima rawatan
yang sepatutnya dan dan diagnosis pesakit dapat dilakukan dengan cepat dan tepat
mengikut zon-zon yang terdapat di unit kecemasan.Sistem triage ini juga
memudahkan staf untuk melakukan kerja.
I will share my story based on my experience facing the obstacles and problem
in triage process. During my working shift in the emergency unit of Sipitang’s
Hospital, I encountered a lot of problem in handling the triage cases. In this sharing,
It does not meant to blame anyone or to point the mistakes to anyone. One of the
problems I had encountered was mistriaging. According to the condition of the
patient,he should be registered in the yellow zone but due to some shortness, he was
only registered in the Green zone. This problem occurs because the triage counter
does not functioning as expected. Why? This happened due to the shortage of staff
working in one shift. The first time I posted in the emergency unit, I was surprised
and worried when I realized that there were only four staffs on duty in a shift. There
were 2 staffs of Medical Assistance (MA), 1 Nurse and 1 PPK in charge during that
time. Thing that made this problem occur was a situation like this, Sometimes the
nurse is a ‘Jururawat Masyarakat’. One day, we encounter a red zone but at the
meantime we were busy handling patient and cases in yellow zone. While we were
struggling handling a patient with asthma, we were facing shortage of staff to control
and handling the triage counter. As a result the process of registration in not work
properly and in addition, the registration counter is only maintained by the PPK. I’m
not saying she did not doing her job but their skills and knowledge in that particular
job limiting their further action in registering the patients.
Therefore, when there were patients who came to the emergency and wanted to
register, the PPK who works at the registration counter will only register according to
the patient's complaint only. When this situation occurs, mistriaging will occur
because the patient was not screened properly. Mistriaging lead to too late treatment
and can cause other complications to occur and the patient's condition will worsen.
Conflicts between Doctors and staff can occur due to this kind of problems.
This issue had been submitted to the head of the unit hoping that action will be
taken seriously to overcome this problem. This problem has also been voiced to the
emergency specialist who takes care of Sipitang’s Hospital. So far, the head of the
unit stipulates that every shift, there must be 3 MAs in charge. They can choose to
overtime if staff working in that particular shift is not enough. But sometimes, other
things happen such as missed assessment on patient because the patient does not
being honest to tell the complete history of their illness.
In conclusion, this triage is very important so that the patient can receive proper
treatment and and the registered and treated quickly and accurately based on their
registered zones in the emergency unit. This triage system also makes it easier for
staff to do the work.
