SPUN PILE FELL DOWN & BROKEN

BRIEF DESCRIPTION
On September 20th 2021, PT. ETI for piling works
package #3 working at area 35 for piling work.
Around 10.00 AM, PT. ETI Pile #3 continue to pile the
middle part of spun pile after bottom part finish for joint
the pile. When lifting the middle spun pile to setting into
the diesel hammer cap, suddenly the snatch block
broken and the spun pile material fell down and broken
as picture attached.
No Injury person in this case.

DO
➢ Ensure all of crane part has been inspected properly

BRIEF DESCRIPTION
Pada 20 September 2021, PT. ETI spun pile untuk
pekerjaan paket #3 bekerja dia area 35 untuk aktifitas
pemancangan.
Sekitar pukul 10.00 pagi, PT. ETI Pile #3 melanjutkan
pemancangan dengan mengangkat spun pile bagian
tengah setelah selesai memancang spun pile bagian
bawah untuk disambung. Ketika mengangkat spun pile
bagian tengah untuk dimasukkan ke topi diesel hammer,
tiba tiba pulley block rusak sehingga material spun pile
jatuh dan rusak..

➢ Pastikan semua bagian derek telah diperiksa dengan
benar
➢ Daftar periksa pra-penggunaan harian yang dilakukan
oleh operator dan pembantu
➢ Pemeriksaan rutin oleh orang yang kompeten untuk
mengangkat
➢ Pemeliharaan yang tepat dan dicatat
➢ Memasang barricade dan sign board saat pekerjaan piling
sedang berlangsung
DON’T
➢ Jangan modifikasi peralatan termasuk semua bagian
peralatan
➢ Jangan menggunakan peralatan apa pun tanpa sertifikat
inspeksi yang tepat dari pihak ketiga
➢ Jangan gunakan alat pengangkat yang tidak bersertifikat
➢ Jangan melewati barricade saat pekerjaan piling sedang
berlangsung
