(A) PRE-CHECK
(2) Safety
(a) Setiap pekerjaan harus ada JSA & GWP
(b) Setiap hari ikuti 5 menit safety meeting ( TTOFS ) ,meeting mingguan dan bulanan. Isi
daftar hadir dan tanda tanggani.
(c) Sopir dan operator serta swamper harus memakai PPE, badge tanda pengenal selama
jam kerja.
(d) Sopir, operator dan swamper harus memastikan telah memakai safety belt sebelum
mengemudi / mengoperasikan kendaraan/ equipment.
(e) Sopir dan operator serta swamper harus dalam kondisi sehat fisik dan mental sebelum
dan ketika mengemudi , mengoperasikan equipment.
(f) Harus ada segi tiga pengaman, P3K, bendera merah, jack dan kunci ban bila dibutuhkan.
(g) Jauhkan badan , pakaian dari bagian-bagian mesin yang bergerak atau berputar.
(h) Jaga jarak aman terhadap jaringan listrik, ikuti CPI / PGT safety procedure.
(i) Jangan mencoba-coba mengeluarkan equipment yang terpuruk , laporkan kepada
supervisor untuk minta bantuan.
(j) Jangan berdiri atau duduk-duduk dibawah atau dibelakang alat-alat berat.
(k) Jaga kebersihan equipment setiap hari.
(l) Dilarang bekerja atau diatas boom crane , jika mesin hidup.
(m) Jaga jarak aman terhadap equipment lain, bangunan-bangunan dan facilitas produksi
ketika bekerja
(n) Pergunakan tangga yang ada apabila naik / turun kendaraan / equipment. Gunakan 3
point contact (2 tangan berpegang dan 1 kaki berpijak) ketika turun naik dump truck.

(3) Dokumentasi
Site Supervisor (PIC) mengecek adanya permit serta SIM yang masih
berlaku – Qualified Personnel.
(a) Sopir & operator harus mempunyai SIM dan CPI permit yang masih berlaku yang selalu
dibawa setiap hari.
(b) Harus ada STNK, buku keur Polisi dan bukti tanda lunas pajak disimpan pada equipment.
(c) Jika salah satu point a atau b tidak tersedia , sopir dilarang mengemudikan atau
mengoperasikan dump truck.
(d) SOP harus direview dan direvisi setiap saat bila perlu guna meningkatkan kesadaran
keselamatan terhadap standar operasi.
(B) OPERATION
( 5 ) Work Order
(a) Harus ada perintah kerja yang sah untuk pemakaian equipment CPI / rental
(b) Jangan menerima perintah kerja selain dari pengawas anda.
(c) Bila ada perubahan instruksi, hentikan pekerjaan – dan tinjau kembali pertanggung
jawaban perintah kerja yang baru.
( 8 ) Travelling
a) Untuk perjalanan resiko tinggi , supervisor akan memilih lansung sopir/ operator.
b) Sopir harus berpengalaman minimum 2 tahun untuk pekerjaan / perjalanan resiko tinggi.
c) Crane harus selalu dipandu ( convey ) dijalan umum.
d) Jangan mendahului kendaraan lain dipersimpangan jalan, tempat-tempat umum dan
padat penduduk.
e) Perhatikan pejalan kaki , sepeda dayung, kendaraan roda dua disekitar anda.
f) Hidupkan lampu besar jika jarak pandang + 100 meter.
g) Jangan membawa penumpang yang tidak berhak.
h) Isi minyak solar ditempat yang aman.
i) Untuk crane rantai , jarak maximum berjalan 200 meter.
( 9 ) Parkir
(a) Parkir ditempat aman dan yang sudah ditentukan
(b) Turunkan boom ke tempat yang sudah ditentukan.
(c) Pasang rem parkir , rem boom dan rem badan ( swing ).
(d) Letakkan tuas kontrol diposisi neutral.
(e) Jangan parkir ditempat yang ramai seperti, komplek gereja, mesjid, sekalah, perkantoran,
pasar dan restoran/ kedai kopi.
(f) Serahkan document , kunci kontak kepada Mandor / Pengawas kerja.
(g) Matikan switch starter ke posisi “ OFF “
(h) Jaga jarak parkir yang aman dengan kendaraan lain, jangan terlalu dekat.
(7) Reporting
(a) Isi laporan tiket harian equipment dan kirim ke DCS setiap hari.
(b) Isi laporan PTI dan kirim ke Equipment Operation setiap hari.
(c) Laporkan setiap kecelakaan, luka-luka dan hampir celaka ke bagian keselamatan
secepat mungkin tanpa memandang besar / kecil kecelakaan.

(2) Safety
(a) Must have JSA & GWP for every job.
(b) Attend TTOFS (daily), weekly & monthly safety meetings & sign off MOM.
(c) Wear appropriate PPE & company badge.
(d) Operator/Driver & passenger must wear safety belt.
(e) Operator/Driver must be in good health with correct vision & fit to drive.
(f) Have triangle, safety cone, first aid kid & jack.
(g) Keep body clothing away from moving parts.
(h) Keep safe distance to energize power line follow PG&T safety procedure.
(i) Do not attempt stuck equipment out it self. Report your immediately supervisor to assist
and help.
(j) Do not take rest or standby under heavy equipment/ crane or behind equipment ever.
(k) Clean equipment daily. Keep windshield clean. Fuel tank always clean.
(l) No personal allowed stay or work in the boom section while engine crane is running.
(m) Assure that no body stay at the engine room before start crane
(n) Use format step ladder when went go to and from crane, no jumped.
(3) Documentation
(a) Driver must have a valid government license & CPI permit and SIO Depnaker.
(b) Must have valid Police registration & tax books in the crane ( mobile – tire crane only )
(c) if either one above item not available, crane must be standby until such document
available report to Equipment operation office.
(4) DDC Rules Obey & practice DDC & Smith System rules
(a) Follow safety & traffic signs, speed limits & regulations (CPI, Local & GOI).
(b) Follow backing procedures, DONOT back without help from others.
(c) Keep minimum 50 meters distance from the vehicle ahead to avoid tailgating.
(d) Reduce speed & drive with cautious near public places, intersections, wet roads
(e) curves, dust, smoke and low visibility conditions
(f) Use horn to communicate in traffic and for public awarness.
(g) Turn on yellow light when travelling on public and field road.
(h) Under wet or bad weather, stop operation and park equipment in the safe place , till safe
back to work / operation
(i) Follow 5(five) essential key of smith system
(j) Follow posted speed limits but adjust speed based on conditions.
(k) Maximum speed 40 Km/ Hrs
(l) When operator leaving crane, engine must shut down , and take out ignition key.
(6) Park
(a) Park at the designated, safe and level areas.
(b) Engage hand brake, boom lock and swing brake
(c) Put all control lever on neutral position, dog the boom at the designated place point.
(d) Do not park at unauthorized places (mosque, church, schools, coffee shops, etc.)
(e) Turn all document , keys to authorized person
(f) Do not park too close to each other, keep safe distance.
(g) Put starter switch off position.
(7) Reporting
(a) Forward completely filled equipment ticket to D.C.S department daily.
(b) Forward completely filled P.T.I form to Equipment Operations office daily.
(c) Report near miss, injury, minor / major incidents to Safety department, immediately.
