Kelenjar Meibom (kelenjar tarsal)

• Kelenjar minyak yang terbuka ke perbatasan kelopak mata.
• Terdapat sekitar 30-40 buah di palpebra atas dan 20-30 buah di palpebra bawah.
• Fungsi: meminyaki air mata agar tidak cepat menguap.
Kelenjar Zeis
• Kelenjar minyak yang terbuka ke folikel bulu mata.

Kelenjar Moll
• Kelenjar keringat yang terbuka ke folikel bulu mata atau ke kelenjar Zeis, tidak terbuka langusng ke permukaan kulit.

Temuan Fisik / Klinis ;

• Terbentuk eritem tipis pada bagian ujung palpebral
• Sekret yang mengeras menempel pada bulu mata
• Pada perjalanan penyakit yang kronis sudah terbentuk krusta
• Dapat ditemukan injeksi konjungtiva bila mengenai palpebral
posterior
  Edukasi ; untuk tidak melakukan penggarukan atau mengucek mata
yang terasa gatal untuk menghindari penularan ke mata sisi lain
maupun penularan ke orang lain, mengingat etiologi blefaritis bisa
saja bakteri / virus / jamur.
 Terapi non-medikamentosa ; kompres hangat dengan handuk yang
lembab. Kompres asam borat.
 Terapi medikamentosa ; salep antibiotic topical, obat tetes mata
cendo xytrol (antiinflamasi & antibiotic).

Tindakan spesifik :
• krusta diangkat dengan kapas basah.
• salep antibiotik seperti sulfasetamid dan sulfisoksazol.
• Bila terjadi blefaritis menahun dilakukan penekanan manu
al kelenjar Meibom untuk mengeluarkan nanah dari
kelenjar Meibom (Meibormianitis)
BLEFARITIS SEBOROIK
 laki-laki usia lanjut (50 Tahun)
 Keluhan: mata kotor, panas dan rasa
kelilipan.
 Tanda: sekret yang keluar dari kelenjar
Meibom, air mata berbusa pada kantus
lateral, hiperemia dan hipertropi papil
pada konjungtiva.
 Pengobatan: membersihkan kelopak
dari kotoran. Dilakukan pembersihan
dengan kapas lidi hangat. Kompres
hangat selama 5-10 menit.
 Tetrasiklin oral 4x 250mg
BLEFARITIS SKUAMOSA
 blefaritis disertai terdapatnya
skuama atau krusta pada pangkal
bulu mata yang bila dikupas tidak
mengakibatkan terjadinya luka
kulit.
 sering terdapat pada orang yang
berambut minyak.
 Blefaritis ini berjalan bersama
dermatitik seboroik.
 Gejala: panas dan gatal.
 Pengobatan: membersihkan tepi
kelopak dengan shampoo bayi,
salep mata, dan steroid setempat
BLEFARITIS ULSERATIF
 blefaritis dengan tukak
 skuama yang terbentuk bersifat kering dan
keras, terdapat keropeng berwarna
kekuning-kuningan yang bila diangkat akan
terlihat ulkus yang yang kecil dan
mengeluarkan darah di sekitar bulu mata.
 Ulserasi berjalan lebih lanjut dan lebih
dalam dan merusak folikel rambut sehingga
mengakibatkan rontok (madarosis).
 Pengobatan:
 antibiotik (sulfasetamid, gentamisin
atau basitrasin) dan higiene yang baik.
 Apabila ulseratif luas pengobatan harus
ditambah antibiotik sistemik dan diberi
roboransia.

• Pada infeksi jamur sistemik (aktinomises atau nokarida)
dengan sulfonamid, penisilin, atau antibiotik spektrum luas.
• Amfoterisin B (histoplasmosis, sporotrikosis, aspergilosis)
• 0,05-0,1 mg/kg BB secara intravena lambat selama 6-8 jam
dalam dekstrosa 5%.
• Dosis dinaikan sampai 1mg/kg BB, tidak lebih dari 2 gram.
• Pengobatan selama 2-3 minggu atau sampai gejala
berkurang.
• Hati-hati karena toksik terhadap ginjal.

• Evaluasi margin kelopak mata, dasar bulu

• Evaluasi kuantitas dan kualitas air mata

 Pembedaan Berdasarkan Penampilan
Margo Kelopak Mata:
Blepharitis Staphyloccal sering menunjukan perlekatan ringan kelopak mata, margo
kelopak mata menebal, dan bulu mata hilang dan salah arah.

Blepharitis seboroik tampak sebagai serpih berminyak atau sisik sekitar pangkal bulu
mata dan kemerahan ringan pada kelopak mata.
Blepharitis ulseratif ditandai krusta keras di sekitar bulu mata yang ketika dihapus,
meninggalkan luka kecil yang mengeluarkan cairan dan berdarah. Mungkin juga
terdapat hilangnya bulu mata, distorsi tepi depan kelopak mata dan robekan kronis.
Meibomian blepharitis terbukti dengan penyumbatan kelenjar minyak di kelopak mata,
rendahnya kualitas air mata, dan kemerahan pada selaput kelopak mata.

Kompres hangat 4 kali sehari selama masing-masing 15 menit.

Bersihkan kelopak mata dengan air bersih atau pun dengan
sabun atau shampoo yang tidak menimbulkan iritasi.
Jangan menusuk kalazion sendiri karena dapat menimbulkan
infeksi yang lebih serius.
Hindari pemakaian make-up pada mata, karena kemungkinan
hal itu menjadi penyebab infeksi.
Hindari penggunaan lensa kontak karena dapat menyebarkan
infeksi sekunder.
 Medikamentosa

Steroid topikal.

Pemberian antibiotik tertrasiklin dosis rendah (doksisiklin tablet 100mg/minggu) selama 6 bulan
mungkin dapat menimbulkan perubahan biokimiawi  pembentukan asam lemak rantai pendek yang
dibandingkan dengan produksi asam lemak rantai panjang lebih jarang menimbulkan sumbatan pada
mulut kelenjar.

Bila kecil dapat dilakukan injeksi intralesi dengan steroid yang berdaya kerja lama
(triamsinolon 0.2-2 mL of 5 mg/mL).
INDIKASI OPERASI KALAZION
• Indikasi kosmetik
• Indikasi optik: bila mengganggu visus
• Indikasi sosial: bila mengganggu aktivitas sehari-hari
• Indikasi medik: bila berisiko komplikasi yang lebih parah
• Pada umumnya, bila tidak terjadi resorbsi dengan pengobatan konservatif
dalam waktu 2 minggu sudah dapat dilakukan pembedahan
