Perbandingan Stimulasi Barorefleks dan Denervasi Renal dalam Tata Laksana Hipertensi Resisten

ABSTRAK Hipertensi resisten didefinisikan sebagai tekanan darah yang tidak mencapai target walaupun pasien telah mendapat 3 agen antihipertensi atau lebih dengan kombinasi ideal, dosis optimal, dan didukung dengan kepatuhan berobat yang baik serta upaya perubahan gaya hidup secara rutin. Optimalisasi terapi merupakan masalah utama dalam tata laksana hipertensi pada populasi ini. Sejumlah studi klinis terakhir menunjukkan bahwa stimulasi barorefleks dan denervasi renal memiliki efikasi dan profil keamanan yang relatif baik sebagai penunjang terapi farmakologis dalam tata laksana hipertensi resisten. Kedua jenis terapi semi-invasif ini bekerja dengan menekan aktivasi saraf simpatis dalam regulasi tekanan darah. ABSTRACT Resistant hypertension is defined as blood pressure that remains above target of therapy in spite of concurrent use of at least 3 antihypertensive agents of different classes with ideal combination and optimal doses, supported by good patients compliance and sustained healthy lifestyle changes. Optimalization of therapy is the primary problem in the management of hypertension in this population. Recent clinical studies showed that baroreflex stimulation and renal denervation had good efficacy and tolerable safety profiles as adjuvant to pharmacological therapy in management of resistant hypertension. Both semi-invasive procedures work by decreasing symphatethic nerve activation in blood pressure regulation. Leonardo Paskah Suciadi, Augustine Purnomowati. PENDAHULUAN Hipertensi saat ini merupakan penyebab utama morbiditas dan mortalitas di seluruh dunia. Sekitar 5 juta populasi dewasa di Amerika Serikat dan tidak kurang dari 1 milyar penduduk dunia dilaporkan menyandang hipertensi. Hipertensi menyebabkan 7,1 juta kematian per tahunnya di seluruh dunia.1 Analisis oleh National Health and Nutrition Examination Survey (NHANES) menunjukkan bahwa hanya 53% penyandang hipertensi yang berobat berhasil mencapai target tekanan darah di bawah 140/90 mmHg.2 Angka tersebut tentu lebih kecil lagi pada populasi hipertensi dengan target tekanan darah lebih rendah, seperti pada penyandang diabetes melitus dan penyakit ginjal kronik. Sebagian besar pasien kelompok ini belum mendapat regimen terapi yang tepat, mendapat dosis

kurang optimal, kepatuhannya buruk, atau belum mengubah gaya hidup secara rutin. Sebagian lainnya adalah kasus hipertensi resisten, yang perlu dibedakan dari kelompok hipertensi tidak terkontrol tersebut.3 Hipertensi resisten sebenarnya dijumpai pada penderita hipertensi esensial sehingga penegakan diagnosisnya memerlukan eksklusi terlebih dulu terhadap kemungkinan sebab-sebab sekunder hipertensi (penyakit parenkim ginjal primer, stenosis arteri renalis, feokromositoma, dll.) maupun pseudohipertensi (white-coat hypertension dan penggunaan ukuran manset tidak tepat).4 Hipertensi resisten didefinisikan sebagai tekanan darah yang tidak mencapai target walaupun pasien telah mendapat 3 agen antihipertensi atau lebih dengan kombinasi
ideal, dosis optimal, dan didukung dengan kepatuhan berobat yang baik serta upaya perubahan gaya hidup secara rutin. Kombinasi ideal adalah penggunaan dua atau lebih agen antihipertensi yang berasal dari kelas dengan mekanisme kerja berbeda, yang salah satunya diuretik.3 Hipertensi resisten hampir selalu memiliki penyebab multifaktorial yang berkontribusi terhadap terjadinya resistensi terapi.3 Prevalensi hipertensi resisten belum jelas diketahui. Sebuah studi kohort, ALLHAT (antihypertensive and lipid-lowering treatment to prevent heart attack trial), melaporkan sekurang-kurangnya 15% kasus hipertensi resisten pada populasi studi.3 Populasi hipertensi resisten ini merupakan kelompok yang berisiko tinggi terhadap kejadian

Rheos menyerupai alat pacu jantung, terdiri atas 3 komponen: generator sinyal elektrik, dua buah elektroda sinus karotis bilateral, dan sistem pemrograman Rheos. Generator sinyal elektrik menggunakan daya baterai dan memiliki sistem sirkuit yang menghantarkan energi aktivasi 1-7,5 V.14 Parameter-parameter stimulasi, yang meliputi amplitudo tegangan, frekuensi, dan durasi impuls, dapat diprogram secara eksternal. Energi aktivasi dibangkitkan dari generator dan dihantarkan ke sinus karotis bilateral melalui kabel dan elektroda, mencetuskan stimulasi serabut saraf baroreseptor di dalam sinus karotis yang selanjutnya menstimulasi barorefleks (Gambar 2).13

Perangkat generasi kedua sistem stimulasi baroreseptor, Barostim neo, hanya terdiri atas sebuah elektroda kecil (unilateral) serta stimulator yang lebih kecil dan lebih canggih. Pengembangan teknis ini membuat prosedur pemasangan menjadi lebih singkat, terapi lebih efisien, dan baterai lebih tahan lama. Saat ini, Barostim neo sudah mendapatkan lisensi Eropa dan digunakan pada praktik klinis pengelolaan pasien hipertensi di beberapa negara Eropa.14
