Semua pekerjaan membutuhkan penilaian bahaya untuk menjamin kontrol yang proaktif. Bahaya
yang buruk dapat mempengaruhi keefektifitas pekerjaan. KONTRAKTOR mengembangkan.
Semua JSA menggunakan formulir JSA. Supervisor adalah orang yang bertanggung jawab atas
JSA di area kerjanya. Sebuah register JSA disimpan di proyek, hard copy JSA pada lokasi kerja.
All work requires a hazard assessment to ensure proactive control. Bad hazards can affect work
effectiveness. CONTRACTOR develops. All JSAs use JSA forms. The supervisor is the person
responsible for the JSA in his work area. A JSA register is stored in the project, a hard copy of the
JSA at the work site.

4.3 PENANGANAN ZAT KIMIA / CHEMICAL SUBSTANCE HANDLING

KONTRAKTOR akan mematuhi hal hal berikut / CONTRACTORS will comply with the following:
1) Minyak, bahan bakar, pelumas, bahan cat, pelarut, asam atau basa, bahan kimia, atau
perairan yang terkotaminasi/tercemar oleh limbah tidak diperkenankan untuk pindah ke
system saluran selokan, atau membiarkan bahan tersebut ke dalam tanah. / Oil, fuel,
lubricants, paints, solvents, acids or bases, chemicals, or contaminated waters /
contaminated by waste are not allowed to move to the sewer channel system, or leave the
material in the soil.
2) Jika terjadi tumpahan minyak dan bahan kimia atau bahan lain harus segera di laporkan
kepada departemen SHE KONTRAKTOR dan harus segera dilakukan upaya penanganan
tumpahan zat kimia dan tindakan pembersihan. / In the event of oil spills and chemicals or
other materials, it must be reported immediately to the SHE CONTRACTOR department
and immediate efforts must be made to deal with chemical spills and cleaning actions.
3) Semua tangki penyimpanan dan bongkar muat bahan kimia, bahan bakar harus berada di
tempat yang telah ditentukan. / All storage tanks and loading and unloading of chemicals, fuel
must be in the designated place.
4) Pencucian peralatan yang telah terkena bahan kimia, minyak, asam atau kontaminan
lainnya harus dilakukan di tempat yang telah ditentukan. / Washing of equipment that has

designated place.
5) Penggunaan bahan B3 dan bahan kimia harus disetujui oleh PERUSAHAAN dan

COMPANIES and CONTRACTORS.
6) Limbah seperti sampah kantor, bahan bangunan, reruntuhan beton dan besi tua yang
dihasilkan selama proyek tersebut akan dibuang setiap hari. / Waste such as office waste,
building materials, concrete debris and scrap metal produced during the project will be
disposed of every day.

4.4 ALAT PELINDUNG DIRI (APD) / PERSONAL PROTECTIVE EQUIPMENT (PPE)

Alat Pelindung Diri (APD) atau Personal Protective Equipment adalah alat-alat atau perlengkapan
yang wajib digunakan untuk melindungi dan menjaga keselamatan pekerja saat melakukan
pekerjaan yang memiliki potensi bahaya atau resiko kecelakaan kerja. Alat-alat Pelindung Diri
(APD) yang digunakan harus sesuai dengan potensi bahaya dan resiko pekerjaannya sehingga
efektif melindungi pekerja sebagai penggunanya. APD standar yang digunakan adalah / Personal
Protective Equipment (PPE) or Personal Protective Equipment are tools or equipment that must
be used to protect and maintain the safety of workers when doing work that has the potential
danger or risk of workplace accidents. Personal protective equipment (PPE) used must be in

accordance with the potential hazards and risks of the work so as to effectively protect workers as
users. The standard APD used is:

4.4.1. Perlindungan untuk Kepala / Head Protective Equipment

Semua personil di proyek, pengunjung, dan vendor wajib menggunakan Helm, kecuali di dalam
kompleks kantor, kendaraan atau daerah yang ditunjuk lainnya. Helm sesuai dengan standar
Amerika atau British Standard dan dilengkapi dengan tali dagu. Warna helm sesuai peraturan
KONTRAKTOR. / All personnel in the project, visitors, and vendors must use Helmets, except in
office, vehicle or other designated areas. Helmets conform to American or British Standard
standards and are equipped with chin straps. The color of the helmet is in accordance with the
rules of the CONTRACTOR.

4.4.2. Perlindungan mata / Eye Protective Equipment

Kacamata Safety bertujuan untuk melindungi mata dari paparan yang berbahaya.Beberapa jenis
mempunyai perisai samping. Goggles yakni kacamata pelindung yang menutupi semua ruangan
di sekitaran mata. Goggles dapat membuat perlindungan mata dari debu serta percikan bahan
kimia cair. Goggles dapat juga digunakan berbarengan dengan kacamata resep karna disainnya
yang semakin besar. Standard yang digunakan serta direferensikan oleh OSHA yakni standar
ANSI (American National Standard Institute) dengan nomor ANSI Z87. 1-1989.
Safety glasses aim to protect the eyes from harmful exposure. Some types have side shields.
Goggles are protective goggles that cover all rooms around the eyes. Goggles can make eye
protection from dust and splashes of liquid chemicals. Goggles can also be used in conjunction
with prescription glasses because the designs are getting bigger. The standard used and
referenced by OSHA is the ANSI (American National Standard Institute) standard with ANSI Z87
number. 1-1989.

4.4.3. Perlindungan pada Pendengaran / Ear Protective Equipment

Peralatan pelindung pendengaran (penutup telinga / earplug) digunakan di daerah atau pekerjaan
dengan tingkat kebisingan tinggi. / Hearing protection equipment (earplugs) are used in areas or
jobs with high noise levels.

4.4.4. Perlindungan Kaki / Safety Shoes

Alat pelindung kaki berfungsi untuk melindungi kaki dari tertimpa atau berbenturan dengan benda-
benda berat, tertusuk benda tajam, terkena cairan panas atau dingin, uap panas, terpajan suhu
yang ekstrim, terkena bahan kimia berbahaya dan jasad renik, tergelincir. Perlindungan kaki
berupa sepatu safety dengan persyaratan / Foot protection device serves to protect the foot from
being hit or collided with heavy objects, punctured by sharp objects, exposed to hot or cold liquids,
hot steam, exposed to extreme temperatures, exposed to harmful chemicals and microorganisms,
slipping .. Foot protection in the form of shoes safety with requirements:
- Karet bagi pekerja sipil atau pekerja yang berhubungan dengan air. Kulit bagi pekerja
mekanik, elektrikal, operator, dll / Rubber for civil workers or workers associated with
water. Leather for mechanical, electrical, operator, etc
- Memiliki sol karet / Has a rubber sole
- Ujung besi (toe cap). / toe cap.

4.4.5. Pakaian pelindung / Protective clothing

Pakaian yang akan digunakan selama di dalam site dan melakukan pekerjaan adalah wearpack
dengan reflective. Untuk pekerja direct dan subcontractor wajib menggunkan rompi warna orange
dan ber-reflective. Untuk pekerja indirect dan tamu menggunakan rompi warna hijau dan ber-
reflective. / The clothes that will be used while on site and do work are wearpack with reflective.
For direct and subcontractor workers must use an orange and reflective vest. For indirect workers
and guests use green and reflective vests.

4.4.6. Perlindungan pada Tangan / Hand guards

Pelindung tangan (sarung tangan) adalah alat pelindung yang berfungsi untuk melindungi tangan
dan jari-jari tangan dari pajanan api, suhu panas, suhu dingin, radiasi elektromagnetik, radiasi
mengion, arus listrik, bahan kimia, benturan, pukulan dan tergores, terinfeksi zat patogen (virus,
bakteri) dan jasad renik. Pelindung tangan terdiri dari sarung tangan yang terbuat dari logam, kulit,
kain kanvas, kain atau kain berpelapis, karet, dan sarung tangan yang tahan bahan kimia.
Hand guards (gloves) are protective equipment that serves to protect the hands and fingers from
exposure to fire, heat, cold temperatures, electromagnetic radiation, ionizing radiation, electric
current, chemicals, collisions, punches and scratches, infected with pathogenic substances
(viruses, bacteria) and microorganisms. Hand protectors consist of gloves made of metal, leather,
canvas fabric, coated fabric or cloth, rubber, and chemical resistant gloves.

4.4.7. Perlindungan untuk Pernapasan / Respiratory

Alat pelindung pernapasan beserta perlengkapannya adalah alat pelindung yang berfungsi untuk
melindungi organ pernapasan dengan cara menyalurkan udara bersih dan sehat dan/atau
menyaring cemaran bahan kimia, mikro-organisme, partikel yang berupa debu, kabut (aerosol),
uap, asap, gas/ fume, dan sebagainya. Alat pelindung pernapasan dan perlengkapannya terdiri
dari masker, respirator, katrit, kanister.
