3.1 crew sijil for 3 persons for signing on and 1 person for signing off was not provided
yet)
Root Cause Diketemukan 3 orang tidak tersijil on dan 1 orang tidak tersijil off saat inspeksi diatas
(Akar Masalah) kapal.
Corrective Action Telah dilaksanakan sijil on untuk 3 orang kru dan sijil off untuk 1 orang kru.

Preventive Action Nakhoda agar tetap memastikan kru yang sign on /off tercatat pada buku sijil kru
(Tindakan Pencegahan) diatas kapal
