1. Identifikasi klien : usia , jenis kelamin, data demografi
2. Riwayat Penyakit Sekarang : Riwayat trauma,proses
degeneratif, penyakit metabolik, glaukoma.
3. Keluhan Utama : adanya penurunan ketajaman
penglihatan secara progesif /penglihatan berkabut,
penglihatan tertutup film
4. Riwayat Penyakit Dahulu : seperti DM, HT, Radiasi.
5. Pemeriksaan fisik : kaji visus.
6. Pemeriksaan Diagnostik : opthalmoscopy, snellen card.
Kasus :
Tn A 56th dengan trauma okuli non perforans dengan
komplikasi HT okuli, subluksasi lensa + katarak traumatika
Diagnosa :
Gangguan persepsi sensori penglihatan
berhubungan dengan penurunan tajam penglihatan
(Tamsuri, 2011)
Tujuan :
Pasien melaporkan kemampuan yang lebih baik
untuk rangsang penglihatannya dan mampu
mengkomunikasikan perubahan visual
Kriteria Hasil :
Pasien mengidentifikasikan dan menunjukkan pola
alternatif untuk meningkatkan penerimaan
rangsang penglihatan
Intervensi dan Rasional :
Kaji ketajaman penglihatan : untuk
mengidentifikasi kemampuan visual pasien
Orientasikan pasien akan lingkungan sekitar :
untuk meningkatkan kemampuan persepsi
sensori pasien
Anjurkan penggunaan alternatif rangsang
lingkungan : untuk meningkatkan kemampuan
respon stimulus lingkungan pasien
Cegah sinar yang menyilaukan : untuk
mencegah distress
Optimalisasi lingkungan : untuk mengurangi
resiko cidera
Diagnosa : Gangguan rasa nyeri berhubungan dengan
luka post operasi (Tamsuri, 2011)
Tujuan : Nyeri berkurang atau terkontrol

Case :
Mr A 56 years with non-perforating ocular trauma with complications of ocular HT, lens subluxation +
traumatic cataract
DS: how come the eye is hit ± 1 month ago, immediately blurred until now, glare (+) pain (+)
DO : Vision : 1/60
TIO schiotz = 27mmHg
Lens subluxation > 120°
Nursing Diagnosis and Intervention
Diagnosis:
Impaired sensory perception of vision associated with decreased visual acuity (Tamsuri, 2011)
Purpose:
Patients report a better ability to stimulate their eyesight and are able to communicate visual changes
Result Criteria:
Patient identifies and demonstrates alternative patterns to increase receptivity to visual stimuli
Intervention and Rationale:
Assess visual acuity: to identify the patient's visual ability
Orient the patient to the surrounding environment: to improve the patient's sensory
perception ability
Recommend the use of alternative environmental stimuli: to increase the patient's
ability to respond to environmental stimuli
Prevent dazzling light: to prevent distress
Optimization of the environment: to reduce the risk of injury
Diagnosis: Risk for infection related to increased susceptibility secondary to invasive surgery on
the eye
Goal : Patient free from infection
Outcome Criteria: Signs of infection during the treatment phase did not appear
Intervention and Rationale:
Instruct the patient to get adequate rest: to minimize the occurrence of infection
Provide adequate nutritional intake: to increase body immunity
Teach aseptic technique: to prevent infection
Monitor for signs of infection: to monitor the progress of the patient
