 POI Daily Covid-19 Self-Monitoring Form
Instructions
1. This Self-Monitoring Form is to be completed by EVERYONE intending to work
/ visit Premier Oil Indonesia offshore facilities (platforms, FPSO, vessels).
2. This daily home self-monitoring is to be done throughout 14 days, not making
any travel to other offshore or onshore field locations or city or country for the
last 14 days, prior to departure.
3. ALL fields on the Form shall be filled. Specifically, for Covid-19 Symptoms
fields:
 Fill-out with actual body temperature
 The rests with: X if NO;  if YES
 N/A if nothing to add on “Others” field
4. Whenever the incumbent experience ANY Covid-19 Symptoms (e.g. Body Tem.
>37.30C, experiencing Dry Cough, Shortness of Breath, etc.):
 For Premier Oil personnel, report to destination Offshore Medic / Offshore
Doctor who will provide further advice
 For Contractor personnel, report to Contract / Job Owner (Sponsor) who
will then consult with the destination Offshore Medic / Offshore Doctor
5. The incumbent send this Self-Monitoring Form to destination offshore Medic /
Doctor on H-7 and H-2 prior to offshore departure
Note: The destination Offshore Medic / Offshore Doctor then reports the
medical decision for the incumbent’s Go / No Go to:
 The destination Planner (or, to Marine Coordinator for work / visit to
the vessels) in case of Premier Oil personnel, or
 Contract / Job Owner (Sponsor) in case of Contractor personnel, who
then inform the destination Planner about the decision

1. Formulir Self-Monitoring ini harus dilengkapi oleh SETIAP orang yang
bermaksud untuk bekerja/mengunjungi fasilitas lepas pantai (platform,
FPSO, kapal) Premier Oil Indonesia.
2. Self-monitoring harian ditempat tinggal ini harus dilakukan sepanjang 14
hari, tidak melakukan perjalanan ke lokasi lapangan offshore atau onshore
atau kota atau negara lain, sebelum keberangkatan.
3. SETIAP kolom pada formulir harus diisi. Khusus, untuk gejala Covid-19:
 Isi dengan suhu tubuh aktual
 Selebihnya isi dengan: X jika tidak;  Jika ya
 Isi dengan N/A bila tidak ada yang perlu ditambahkan pada kolom
“Others”
4. Setiap kali yang bersangkutan mengalami salah satu gejala Covid-19 (e.g.
suhu tubuh > 37,30 C, mengalami batuk kering, sesak nafas, dll.):
 Bagi personil Premier Oil, laporkan kepada Offshore Medic / Dokter
Offshore dari fasilitas yang akan dituju untuk saran lebih lanjut
 Bagi personil kontraktor, laporkan kepada pemegang kontrak /
pekerjaan (Sponsor) yang kemudian akan berkonsultasi dengan
Offshore Medic / Dokter Offshore
5. Yang bersangkutan mengirimkan Formulir Self-Monitoring ke Offshore
Medic / Dokter dari fasilitas yang dituju H-7 serta H-2 sebelum
keberangkatannya ke offshore.
Catatan: Offshore Medic / Dokter Offshore dari fasilitas yang akan dituju
kemudian melaporkan keputusan medis Go/No Go bersangkutan ke:
 Planner dari fasilitas yang akan dituju (atau, ke Marine Coordinator
untuk bekerja / kunjungan ke kapal) bila menyangkut personil
Premier Oil, atau
 Pemegang kontrak / pekerjaan (Sponsor), yang kemudian akan
menginformasikannya ke Planner lokasi yang akan dituju, bila
menyangkut personil kontraktor
