1.0 Tujuan

Tujuan standar operasional prosedur ini adalah untuk menangani penggalian pada area misfire
secara aman.

2.0 Tanggung Jawab

2.1 Blasting Foreman bertanggung jawab terhadap prosedur ini. Jika ada kesulitan-
kesulitan atau masalah dalam pelaksanaan prosedur tersebut harus dilaporkan
kepada D&B Supervisor dan Production Superintendent sebagai Pejabat
Kepala Teknik, yang akan meninjau kelengkapan dan efisiensi prosedur ini
serta menyarankan revisi – revisi yang sesuai.
2.2 Blasting Foreman bertanggung jawab terhadap demarkasi area dimana misfire
dicurigai terjadi, supervisi terhadap penggalian, penanganan explosive misfire,
clearance area. Memastikan insiden report yang harus dibuat dan pencatatan
dalam Buku Misfire.
2.3 Mine Superintendent mengkomunikasikan kepada Regional Mine Manager.
2.4 Survey Supervisor bertanggung jawab terhadap pengambilan data koordinat
lokasi misfire untuk dijadikan sebagai salah satu dokumen.

4.0 Lingkup Kerja

Standar operasional prosedur ini berlaku untuk penggalian area dimana misfire dicurigai
terjadi.

5.0 Prosedur Kerja

5.1 Ketika misfire dicurigai terjadi dikarenakan cut off excel tube, primer atau
sesuatu hal lain, Blasting Foreman mendemarkasi area yang dicurigai secara
aman dan area tersebut kembali menjadi area peledakan. Melalui diskusi
Blasting Foreman menentukan langkah yang tepat, membuat insiden report dan
memastikan misfire dicatat dalam Buku Misfire. Survey Section mengambil
data koordinat lokasi dan dicetak kedalam plan harian. Blasting Foreman
mengkomunikasikan kepada D&B Supervisor, Production Supervisor dan
Mine Superintendent, terlebih jika menjadi peledakan tidur (sleeping blasting).
5.2 Should the misfire be in the digging face, the digging unit may not work within
coned and taped off areas without the supervision of the BLF or Shotfirer. The
Digging unit may only work within coned of area when supervised by BLF or
Shotfirer. When a digging machine approaches this area the BLF should be
notified and a close watch be kept to ensure this area is free of explosives.
5.3 Inside the coned off area digging should start on the side of the misfire from
top to the bottom allowing the broken rock to fall away exposing the unfired
 charge. When the BLF or Shotfirer want to inspect a suspected misfire
location, all digging will cease in this location. Digging shall not recommence
until the BLF or Shotfirer instruct the operator of the digging unit that it is safe
to commence digging again.
5.4 When it is safe to do so, an attempt should be made to recover the down hole
delay and primer by having the water truck standing by to wash out the product
i.e. ANFO, Energan, Powergel.
5.5 Never attempt to fire this type of misfire as there is very little burden and fly
rock is very dangerous.
5.6 When the Supervisor Blasting deems the area free of explosives the misfire
barriage shall be removed.
5.7 The Supervisor Blasting shall notify the Production Superintendent that the
area is safe to commence work.
