Blast Controlling Procedure
1. PURPOSE
This work instruction aims at providing a controlled
blasting better explanation for Leighton MGM
Coal Mine Project so that these operations take
place safely.
2. SCOPE
This work instruction covers the tasks done by the
blast crew for blasting preparation, evacuation, blast
controlling and verify that the blasting operation is
safe.
3. DEFINITION
Personal Protective Equipments (PPE)
All personal protective equipments to be worn when
occupied in the task like: helmet, clear safety
glasses, vest, safety boots/shoes, rain coat
Qualified Person
A person who has a Class II Blasted Certificate and
competency for all blasting procedures.
Leading Shotfirer
a MGM - LCI qualified Shotfirer that is responsible
for several areas. He is also as the Shotfirer in
charge of that area. He is ultimately responsible for
pushing the firing gun.
Shotfirer in Charge
an LCI-MGM qualified Shotfirer that is responsible
for one blasting area that may comprise of several
locations he is also responsible to push the firing
gun.
Asistant Shotfirer
A LCI-MGM qualified Shotfirer that works under the
supervision of the Shotfirer in charge.
Blasting Controller
A LCI-MGM qualified Blast Supervisor or a higher
level that is responsible for coordinating the
preparation of one area or more prior to handling
over to the Leading Shotfirer. The Blast Coordinator
must have a Class II Blasted Certificate and Blasting
License (KIM).

1. TUJUAN
Instruksi ini bertujuan untuk memberikan penjelasan
yang lebih baik mengenai tata cara pengamanan
peledakan di Leighton Proyek Pertambangan
Batu Bara MGM sehingga proses peledakan dapat
berjalan dengan aman.
2. RUANG LINGKUP
Instruksi ini mencakup semua tugas-tugas yang
dikerjakan oleh petugas peledakan dari persiapan
peledakan, evakuasi, penjagaan area peledakan
sampai peledakan dinyatakan aman.
4. DEFINISI
Alat Pelindung Diri (APD)
Alat pelindung diri yang wajib digunakan saat
melakukan pekerjaan seperti : helmet, kaca mata
safety, rompi reflektor, sepatu safety, jas hujan.
Orang yang Berkualifikasi
Seorang yang memiliki sertifikat Juru Ledak Kelas II
dan sudah memiliki kompetensi terhadap semua
prosedur peledakan.
Pemimpin Juru Ledak (Pemimpin Blaster)
Blaster berkualifikasi LCI-MGM yang bertanggung
jawab terhadap beberapa area. Dia juga dapat
merangkap sebagai blaster yang bertugas terhadap
salah satu dari area tersebut. Dia juga bertanggung
jawab untuk menekan starter gun.
Blaster yang Bertugas
Blaster berkualifikasi LCI-MGM yang bertanggung
jawab terhadap satu area peledakan yang mungkin
terdiri dari beberapa lokasi dia juga bertanggung
jawab untuk menekan starter gun.
Asisten Blaster
Blaster berkualifikasi LCI-MGM yang bekerja di
bawah pengawasan dari Blaster yang bertugas.
Koordinator Peledakan
Karyawan setingkat blast supervisor atau lebih yang
berkualifikasi LCI-MGM yang bertanggung jawab
dalam mengkoordinasikan persiapan satu atau lebih
area peledakan sebelum dialihkan ke pemimpin
blaster . Blast controller harus memiliki sertifikat
juru ledak kelas II dan Kartu Ijin Meledakkan

4.2 Work Method
1. The blasting sign board in figure 1 must have be
installed at the mine access and placed one day
before blasting schedule by 03.00 pm.Red flag
shall be installed in the morning before blasting
by least at 08.00 am.

2. At least 3 hours prior to the blasting, the Drill &
Blast Engineer must have communicated the
blasting time to Mine Technical Manager, all
department heads, the mining Superintendent
and other related parties by email. This
information must be communicated in production
meeting (at 8:30 am).
3. Drill & Blast Engineer must have given the blast
radius map to the Blast Controller at least by 9.00
am prior the blast.
4. Blast Controller will erect the yellow and green
flag as the safe distance of equipment (300 m)
and people (500 m) as figure 2. If the fumes or fly
rock ia a risk, the safe distance must be added.
The Blaster must inform to the Blast Controller
about the risk without limiting to fumes occuring
caused by ANFO slumping, the over loading of
blastholes, any short spaces or free space, effect
by fly rock.

4.2 Metode Pekerjaan
1. Papan pengumuman peledakan seperti
gambar 1 sudah harus dipasang di pintu
masuk tambang dan tempat yang ditentukan
sehari sebelum jadwal peledakan paling
lambat pukul 15.00. Bendera merah dipasang
pada pagi hari sebelum peledakan paling
lambat pukul 08.00.
2. Paling lambat 3 (Tiga) jam sebelum peledakan
Drill & Blast Engineer harus sudah
memberikan informasi jam/waktu peledakan
kepada Kepala Tenik Tambang, seluruh Dept.
Head, General Superintendent dan pihak-
pihak terkait lainnya melalui email. Informasi ini
juga harus disampaikan pada saat meeting
produksi (pukul 8.30).
3. Drill & Blast Engineer harus sudah
menyerahkan peta radius peledakan kepada
Blast Controller paling lambat pukul 9.00 pagi
sebelum peledakan.
4. Blast Controller akan memasang bendera
kuning sebagai jarak aman dari lokasi
peledakan minimal untuk alat ( 300 m) dan
bendera hijau sebagai jarak aman untuk
manusia (500 m) seperti yang terlihat pada
gambar 2. Jarak aman harus dtambah jika
diketahui akan timbul bahaya seperti fume atau
fly rock yang bisa melewati jarak aman
tersebut. Juru ledak atau blast supervisor
harus memberitahukan kepada blast controller
jika terdapat hal-hal yang bisa menimbulkan
bahaya, misalnya (tetapi tidak terbatas pada):
lokasi peledakan yang diisi dengan ANFO
tetapi terendam air sehingga ada kemungkinan
timbul fume, lubang yang kelebihan isian,
adanya rongga atau bidang bebas yang terlalu
pendek sehingga bisa menimbulkan flyrock.

5. Blast Controller must be have the radio car, hand
talky, and sirine. The blast controller shall check
blast radius map the exclusion zone to ensure all
access (roads) have a green flag . A minimum of
500 meters.
6. The blast controller shall document the blast
guard position on the blast clearance map.
7. Blast controller must setting firing location for
shotfire outside 500 meters (human safe radius)
radius from the blast location.
8. An hour prior to blasting the Blaster will make
request to radio room to notify the blasting plan to
all employees by radio. ( Attention,you are

9. Blast Controller, Road Block, Sweeper, Leading
Blaster and Blaster in charge must conduct a
meeting at least 45 minutes prior to blasting.
Blast controller will give the written instruction as
in figure 3 about the blasting design such as the
name of the road block and sweeper, their
location and instructions.
Note: Blast controller shall document all road
blocker position and sweeper. He shall document
distance as well, shall make copies of map for all
blocker and sweeper, leading blaster, blaster or
blaster assistant that will check the blast location
after firing.

5. Blast controller harus dilengkapi radio mobil,
radio tangan dan sirine. Blast Controller harus
melakukan pemeriksaan pada exclusion zone
untuk memastikan semua jalan masuk (jalan-
jalan) sudah dipasang bendera hijau sebagai
tanda area yang aman. Bendera ini dipasang
minimal tiap 500 meter.
6. Blast Controller harus mencatat posisi dan
jarak blast guard dan menuliskan pada baik
rencana maupun laporan pengamanan
peledakan.
7. Blast controller harus menentukan firing
location kepada shotfire diluar radius 500
meter (radius aman manusia) dari lokasi
peledakan.
8. Pemimpin blaster 1 (satu) jam sebelum
peledakan akan meminta kepada radio room
untuk memberitahukan rencana peledakan
kepada seluruh orang melalui semua saluran
radio. (Perhatian, diberitahukan kepada
semua pihak bahwa PT. Leighton Contractors
Indonesia MGM Project akan melakukan
peledakan dalam waktu satu jam kedepan atau
pada pukul ........wita) disampaikan 2 (dua) kali
di semua saluran.
9. Blast controller, road blocker, sweeper,
pemimpin blaster dan blaster yang bertugas
harus melakukan meeting paling lambat 45
menit sebelum jam/waktu peledakan. Blast
controller akan memberikan instruksi tertulis
seperti yang terlihat pada gambar 3 pada peta
peledakan. Nama, posisi dan instruksi kepada
penjaga jalan dan sweeper harus tertulis pada
peta peledakan.
Catatan : Blast controller harus mencatat
semua posisi road blocker dan sweeper. Dia
juga harus mencatat jaraknya kemudian
membuat salinan peta peledakan untuk road
blocker dan sweeper, pemimpin blaster,
blaster, atau asisten blaster yang akan
melakukan pemeriksaan lokasi peledakan

10. Leading Blaster or Blaster (as also will push firing
gun) must have received the blasting design after
the meeting. If the firing location more than one,
each of them must receive the blasting design.
11. Leading Blaster and Blaster in charge must go
back to the blasting area that they are
responsible for to make sure that the assembly
and the final check is complete.
12. Sweeper soon as possible prepare evacuation 30
minutes prior to blasting and make sure that all
equipment and people are in safe distance from
the blasting area (safe distance of equipment
300m marked with yellow flag and for people
500m marked with green flag).
13. If blasting impacts any uncleared areas, the blast
controller shall notify security to complete the
inspection 2 (two) hours prior to blast time where
available.
Roads shall be put in place to provide access for
blast controller or sweeper to check and secure
area.

11. Pemimpin blaster dan blaster yang bertugas
harus kembali ke area peledakan yang
menjadi tanggung jawabnya untuk
memastikan rangkaian dan pemeriksaan akhir
telah lengkap.
12. Petugas penyisiran (sweeper) harus segera
melakukan persiapan evakuasi 30 menit
sebelum waktu peledakan dan harus
memastikan semua peralatan dan semua
orang berada pada jarak yang aman dari
lokasi peledakan (bendera kuning 300m untuk
alat dan bendera hijau 500m untuk manusia).
13. Jika peledakan mempengaruhi area yang
berpenghuni, blast controller harus
memberitahu pihak security untuk melakukan
pemeriksaan 2 jam sebelum waktu peledakan
jika memungkinkan.
Disediakan jalan untuk memberikan akses
kepada blast controller atau sweeper untuk
memeriksa dan mengamankan area.

14.Road Blocks to move directly into position once
the blast controller has given him the instruction.
To ensure personnel and equipment can not
enter exclusion zone a minimum of 15 minutes
before blasting takes place. Road blocks to stop
their vehicle and put a stop sign on it. They must
be out side of vehicle and have a car radio and
hand talki. If not possible to take their vehicle
there, they must stand on responsible area and
erect the stop sign as figure 4.The road blocker
shall be parked perpendicular with road with
hazard light turned on, and a red safety cone in
front of vehicle and behind (Figure 5 and 6).

14. Setelah meeting, road blocker harus langsung
menuju posisi masing-masing ketika di
perintahkan oleh blast controller untuk
memastikan semua orang dan peralatan
berada di jarak aman dari lokasi peledakan,
paling lambat 15 menit sebelum waktu
peledakan. Setelah sampai di posisinya road
blocker akan melintangkan mobilnya ditengah
jalan dan memasang tanda stop pada mobil
tersebut. Road blocker harus berada diluar
mobil dan dilengkapi dengan radio mobil dan
radio gengam. Jika tempat tidak
memungkinkan road blocker membawa mobil
pada posisinya, road blocker harus berdiri
pada akses yang dijaganya dengan
memasang tanda stop seperti yang terlihat
pada gambar 4. Road blocker harus
melintangkan mobilnya ditengah jalan dan
menyalakan lampu hazard dan memasang
safety cone didepan dan belakang mobil
(gambar 5 dan gambar 6).

15. The Blaster in charge would assign an assistant
Blaster that is responsible for the post blasting
checks (final post blast) in each location in
accordance to the blasting map.
16. If the tie-up has been completed before blasting
time, this location must be secured until
clearance of the blasting area. If there is a
breach to the secured location because
someone or some equipment enters the tie-up
location that has been checked, the location
must be re-checked by the Blaster in charge and
his Assistant.
17. If there are two or more blasting locations in one
pit, the secondary lead in line is extended
between those locations after all the equipment is
moved from the blasting area, and only if the lead
in line can be placed in a clear area of all
operation and the explosive signs must be
erected 10m away as shown in figure 7 from
access to lead in line between those locations.

15. Blaster yang bertugas akan menunjuk Asisten
Blaster yang bertanggung jawab untuk
pemeriksaan setelah peledakan (final check)
di masing-masing lokasi sesuai dengan peta
peledakan.
16. Jika rangkaian sudah selesai sebelum waktu
peledakan, lokasi harus dijaga sampai saat
pengosongan lokasi peledakan. Jika ada
pelanggaran terhadap lokasi yang terjaga
karena seseorang atau peralatan masuk ke
lokasi peledakan yang sudah terangkai dan
sudah diperiksa, lokasi tersebut harus
diperiksa ulang oleh Blaster yang bertugas
dan Asisten blaster.
17. Jika ada dua atau lebih lokasi peledakan
dalam satu pit, lead in line pembantu (antar
lokasi) dapat dibentangkan diantara lokasi
tersebut sesudah semua peralatan
dipindahkan dari area peledakan dan hanya
jika lead in line antar lokasi dapat ditempatkan
pada posisi yang aman dari semua operasi
dan harus dipasang tanda bahan peledak
minimal 10 m seperti yang terlihat pada
gambar 7 dari akses menuju lead in line yang
dibentang antar lokasi.

18. When extending the lead in line, do not try
connecting it to the blasting location to avoid the
chance of being pulled and blasted.
19. 30 minutes before the blasting, the blast
controller would activate the siren for one minute
and continued with this announcement: [first
warning, 30 minutes from now there would be a
blasting at location., so please prepare for the
evacuation from the blasting location], it must be
informed twice. On all channnels
20. The blast controller would arrange the clearance
and make sure with sweeper that the location is
safe, the equipment have moved out side of
yellow flag and people in outside of the green
flag. Blast controller and sweeper to use siren
when evacuating area.
21. 15 minutes before blasting (before counting
down) Blast Controller will give instructions to all
personnel involved on blasting operation to use
channel (151.100 ) (Please attention, all related
personnel must used channel 151.100), it is
communicated twice.
22. 15 minutes before blasting the Blast Controller
will call all Road Block to make sure they are at
the designed location. He will call :(Blast
controller to Road Blocker 1 its Edi in Midwest, is
your location safe and secured ?) and the
answer is (This is Edi as a Road Block 1,in
Midwest, I am in position and my location is
secured). If the condition not clear, they must
stay on it and inform to Blast Controller then he
will instruct Sweeper to re-check the location and
inform the Road Block, the road blocker shall
inform to blast controller if it is clear like (Road
Blocker to Blast Controller, Joko as Sweeper for
the mid west, the area has been checked and its
declared that the Midwest is clear).
23. Blast Controller will instruct the Blaster to
extend/draw the main lead in line (to starter gun).
24. After all area are safe, the main lead in line may
be extended, and between location may be
connected with the follow requirements :

18. Ketika membentangkan/menarik lead in line,
jangan sekali-kali mencoba menyambungkan
ke lokasi peledakan untuk mencegah
kemungkinan tertarik dan terledakkan.
19. Tiga puluh (30) menit sebelum peledakan, blast
controller akan mengaktifkan sirene selama 1
menit dilanjutkan dengan pengumuman :
(perhatian, 30 menit lagi akan dilakukan
peledakan di lokasi .. agar segera dilakukan
evakuasi dari area peledakan) dibaca dua kali.
Disemua channel.
20. Blast controller akan mengatur pengosongan
di radius peledakan dan memastikan dengan
petugas sweeper bahwa area peledakan
sudah aman, peralatan sudah berada di luar
bendera kuning dan semua orang sudah
berada di luar bendera hijau. Gunakan sirine
saat proses evakuasi.
21. Blast controller 15 (lima belas) menit sebelum
waktu peledakan (sebelum hitungan mundur)
akan memerintahkan kepada semua yang
terlibat dalam kegiatan peledakan untuk pindah
saluran menggunakan channel (151.100).
(perhatian, kepada semua pengguna radio
agar segera pindah ke channel 151.100.

22. Blast controller akan melakukan panggilan
yang pertama kepada seluruh road blocker 15
(lima belas) menit sebelum waktu peledakan

aman?) dan road blocker akan menjawab

sudah diposisi dan situasi dinyatakan aman).
Apabila situasi belum aman road blocker akan
memberitahukan kepada blast controller, blast
controller akan memerintahkan kepada
sweeper untuk melakukan pemeriksaan. Jika
situasi sudah aman kembali sweeper akan
melaporkan kepada road blocker, road blocker
kemudian akan melaporkan ke blast controller
(road blocker ke blast controller, sweeper
Joko sudah melakukan pemeriksaan dan
situasi di barat lama sudah dinyatakan aman).
23. Blast controller akan memerintahkan blaster
untuk membentangkan/menarik lead in line
utama (menuju starter gun)
24. Pada saat itu, lead in line utama boleh
dibentangkan dan lead in line antar lokasi
boleh disambung. Pada saat ini :

25.Blast controller to make radio announcement
Blast controller to all radio units, please maintain
radio silence for blasting operations. Blast

Midwest, is your location safe and secure ?) and
the answer is (This is Edi a Road Block 1, I am
in position and the condition is clear). If the
condition not clear, they must stay on it and
inform to Blast Controller then he will instruct
Sweeper to re-check the location and inform to
Road Block if it is clear like (Road Blocks and
Blast Controller, this is Joko as Sweeper, for the
Midwest has been checked and its declared that
the Midwest is clear).
26. If all Road Block have given the all clear, the
Blast Controller informs to the Blaster in charge
to connect the lead in line (Blaster, please
connect the main lead in line to the blasting
area). Blaster in charge confirms to Blast
Controller that he had connected it to blasting
area (Blast Controller, Blaster had connected the
main lead in line to blasting area).
27. Soon after connecting the main lead in line to
blasting area , the Blaster in charge must go to
the firing location and having arrived there he
must inform the Blast Controller (Blast
Controller, Blaster has already been on the firing
location, blasting is ready to take place).
28. If Blaster is already in the firing location and the
area is clear, Blast Controller will announce to all
person (Please attention, Informed that will be
conducted blasting operation in location ..eg
Midwest, commencing 5 seconds and finished
with Tembak. Please maintain radio silence
channel 1 and dont enter the blasting area until
safe blasting is clarified. If any unsafe condition

situasi di barat lama aman?) dan road blocker
akan menjawab (road blocker 1, Edi, di jalan
barat lama, sudah diposisi dan situasi
dinyatakan aman). Apabila situasi belum
aman road blocker akan memberitahukan
kepada blast controller, blast controller akan
memerintahkan kepada sweeper untuk
melakukan pemeriksaan. Jika situasi sudah
aman kembali sweeper akan melaporkan
kepada blast controller contoh : (dari sweeper
ke blast controller, sweeper Joko sudah
melakukan pemeriksaan dan situasi di barat

27. Segera setelah menyambung lead in line
utama ke lokasi peledakan, Blaster yang
bertugas harus langsung menuju firing location
penembakan. Setelah sampai di firing location,
blaster harus memberitahu blast controller
(dari blaster ke blast controller, blaster sudah
berada di firing location, proses peledakan siap
untuk dilanjutkan).
28. Apabila blaster sudah berada di firing location
dan situasi aman, blast controller akan
mengumumkan (Perhatian, diberitahukan
kepada semua pihak bahwa sesaat lagi akan
dilakukan peledakan di lokasi .. dalam waktu
lima detik dan diakhiri kata tembak dimohon
untuk tidak menggunakan radio pada channel
satu dan tidak memasuki area peledakan
sampai peledakan dinyatakan aman secara
keseluruhan. Apabila ada situasi dan kondisi

process). At this time Blaster in charge will cut
the lead in line and put the signal tube on firing
gun.
29. Blaster will survey the blasting area and above
before counting down if any plane or helicopter
fly on it. He will inform to Leading Blaster if it is
safe (Leading Blaster, blasting area is ready).
30. If there is a danger or emergency situation (e.g.
there is a person within the blasting radius) after
the announcement / warning, the one who saw it
or the road blocks must immediately stop the
process by pushing the emergency button on the
radio or announcing abort blasting, abort
blasting in channel 151.100
31. If the Leading Blaster / the Blaster in charge
hears this statement abort blasting, he must
stop the blasting process by releasing the
connection to the firing gun. After the danger is
cleared the blasting process return back to
section 25.
32. If there is no statement abort blasting, the
Leading Blaster (for location) or the Blaster in
charge (for one area) will countdown the last 5
seconds (the countdown starts..five, four, three,
two, one, fire) in radio channel 1. Right after the
countdown, the Blaster in charge will spin the
starter gun to firing position and push it to blast
the location. Dont push the PTT of radio
continuously to get chance of interruption.
33. After blast has been initiated, Blaster in charge
to make radio call (Blaster to Blast Controller,
the blast has been fired, blaster in the firing
location is safe) and continue with (blasting
area on inspection, Please maintain radio silence
channel 1 and all road blocker are to remain in
position until the blast has been cleared).If no
answer of Blaster, Blast controller must go to
firing location to check his condition.

atau helicopter yang terbang diatas lokasi.
Jika aman akan memanggil Pemimpin blaster
untuk memastikan bahwa areanya sudah siap
(pemimpin blaster area peledakan sudah
siap).
30. Jika terjadi bahaya atau keadaan darurat
(misalnya ada orang di dalam radius
peledakan) setelah peringatan/pengumuman,
orang yang melihat atau road blocker harus
segera menghentikan proses peledakan
dengan menekan tombol radio pada channel
151.100 dan mengumumkan Batalkan
peledakan, batalkan peledakan.
31. Jika Pemimpin blaster/blaster yang bertugas
mendengar pernyataan batalkan peledakan ini,
dia harus menghentikan proses peledakan
dengan melepaskan sambungan ke starter
gun. Setelah bahaya dapat diatasi, proses
peledakan dimulai lagi dari no 25 .
32. Jika tidak ada pernyataan batalkan peledakan,
Pemimpin blaster (untuk beberapa area) atau
blaster yang bertugas (untuk satu area) akan
menghitung lima mundur dan diakhiri kata
tembak (hitungan lima mundur dimulailima,
empat, tiga, dua, satu, tembak) di radio
saluran/channel 1. Segera mengikuti hitungan
mundur ini, Blaster yang bertugas akan
menekan starter gun untuk meledakkan lokasi.
Pada saat penghitungan mundur tidak boleh
menekan PTT radio secara terus menerus
untuk memberi kesempatan jika ada interupsi.
33. Setelah terdengar bunyi ledakan, blaster yang
bertugas akan akan melaporkan (dari blaster
ke blast controller, peledakan sudah dilakukan,
blaster di firing location aman) dan blast
controller akan akan mengumumkan (area
peledakan masih dalam pemeriksaan, dimohon
untuk tidak menggunakan radio pada channel
satu dan road blocker tetap di posisi sampai
peledakan dinyatakan aman secara
keseluruhan) Apabila tidak ada pernyataan
dari blaster setelah peledakan, blast controller
harus segera menuju firing location untuk
memeriksa kondisi blaster.

34. After firing the Shotfirer in charge must stay at
least 10 seconds. He or his assistant must wait
until the dust and the fumes are clear (as figure
8), and head to the location set for them.

35. The Leading Blaster, the Blaster in charge and all
the Blaster assistants involved at the post
blasting check must stay in contact to monitor all
the communication on blasting.
36. If no misfire found, the Assistant Blaster will
inform that the area is clear to the Blaster. This is
an example of a clear statement (Blaster, Seam
area 2.3 is clear, no misfire).
37. The final check requires the inspection to the end
of each echelons/row in order to make sure that
all echelons/rows already fired. All level area
shall be watched closely to find the indication of
misfires.
38. The Leading Blaster on duty shall accept the
assurance directly from his assistant for all
location and mark it on the blast map/design
before confirming it to the Blast Controller
(Leading Blaster to Blast Controller, all blasting
area is clear, no misfire) Blast Controller will
announce that the whole area is clear and
instructs the Road-Blocker to unblock the
access road to blasting area, also that the radio
channel is ready to use. (Please attention,
informed to you that blasting is all clear, Road
Blocker please open the access road to blasting
area and channel 151.100 is ready to use, thank
you for your corporation) continue to sound the
siren for 5 seconds.

34. Sebelum melakukan pemeriksaan lokasi
peledakan, blaster yang bertugas atau Asisten
blaster harus menunggu 10 detik hingga debu
dan asap telah hilang (seperti yang terlihat
pada gambar 8) sebelum menuju ke lokasi
peledakan yang telah ditetapkan untuk
mereka.
35. Pemimpin blaster, blaster yang bertugas dan
semua Asisten blaster yang terlibat pada
pemeriksaan setelah peledakan harus saling
tetap berhubungan untuk memantau semua
komunikasi peledakan.
36. Setelah sampai dilokasi, jika tidak terjadi
misfire yang ditemukan dan jika terlihat sudah
terledakkan semuanya, semua Asisten blaster
akan menyampaikan pernyataan aman kepada
Pemimpin blaster. Pernyataan aman seperti
contoh (dari asisten blaster ke pemimpin
blaster, lokasi seam2,3 dinyatakan aman,
misfire nihil)
37. Pemeriksaan setelah peledakan memerlukan
pemeriksaan ujung akhir setiap echelon atau
baris untuk memastikan semua echelon atau
baris telah meledak. Semua area yang rata
harus diperiksa lebih dekat/ teliti untuk melihat
kemungkinan adanya misfire.
38. Pemimpin blaster harus menerima kepastian
langsung dari Asisten blaster untuk semua
lokasi dan menandainya di peta peledakan
kemudian melaporkan ke blast controller (dari
pemimpin blaster ke blast controller, semua
lokasi peledakan dinyatakan aman misfire
nihil) Blast controller akan mengumumkan
bahwa area peledakan aman secara
keseluruhan dan menyampaikan kepada road
blocker untuk membuka jalan menuju area
peledakan dan saluran radio dapat
dipergunakan kembali. (Perhatian,
diberitahukan kepada semua pihak bahwa
peledakan dinyatakan aman secara
keseluruhan, road blocker dipersilahkna
membuka jalan menuju area peledakan dan

39. If misfire does occur, the Assistant Blaster or
Blaster should report it to the Leading Blaster.
The leading Blaster will ask the Blast Controller
to broadcast on radio/channel 1 about the misfire
and ask the road-blocks to stay in the position
and that radio Channel 1 shall be kept clear
(Pay attention to all Road Blocks, misfire does
occur, please stay on your position and that radio
channel 1 shall be kept clear until safe blasting is
clarified).
40. The Blaster and Blast Supervisor will conduct
inspection and coordinate to the Drill and Blast
Superintendent to perform any necessary action
refer to Misfire Procedure.
41. Based on findings from the inspection, Blast
Controller will inform to Road Blocks, which point
can be unblocked.
42. If misfire occur, it must be investigated and
created the incident report. This must be
reported to Mine Technical Manager and the
continued handling must get approval from him
of his delegate.
43. If misfire have been found and need to
secondary blasting, blasting process resumes
back to section 22.
44. Misfire report shall be prepared by the Blast
Superintendent and Drill and Blast Engineer
based on investigation results to all personnel
involved in the blasting. Blast Superintendent will
attach misfire report to complete incident
investigation report.
45. Time arrangement/limit for blasting activities are
(blasting is conducted at 03:00 pm):
Completed loading blastholes at 02:00 pm
Blasting meeting at 02:15 pm
Conducted blasting at least :03:00 pm
Any advance of the time limit above shall be
under approval of Drill and Blast
Superintendent and Mine Technical
Manager. Drill and Blast Superintendent
must create the written notification about it.

misfire dan meminta road blocker untuk tetap
diposisinya dan saluran radio belum bisa
digunakan (Dari blast controller, perhatian
kepada seluruh road blocker, telah terjadi
misfire, dimohon untuk tetap pada posisi dan
radio channel .... tidak dipergunakan sampai
peledakan dinyatakan aman secara
keseluruhan).
40. Blaster dan Blast Supervisor akan melakukan
inspeksi dan berkoordinasi dengan blast
superintendent dan drill & blast superintendent
untuk mengambil tindakan yang diperlukan
sesuai prosedur Penanganan Gagal Ledak.
41. Berdasarkan hasil pemeriksaan sementara,
Blast Controller akan menyampaikan kepada
Road Blocker, titik mana saja yang boleh
dibuka.
42. Apabila terjadi gagal ledak, maka harus
dilakukan investigasi dan hasil investigasi
dituliskan dalam laporan kecelakaan. Laporan
ini harus dilaporkan kepada Kepala Teknik
Tambang dan penanganan lebih lanjut harus
mendapat persetujuan dari Kepala Teknik
Tambang atau yang mewakilinya.
43. Apabila misfire berhasil ditangani dan perlu
dilakukan peledakan ulang, ulangi prosedur ini
dari nomor 22.
44. Laporan misfire harus dibuat oleh Blast
Superintendent dan Drill & Blast Engineer
berdasarkan hasil investigasi terhadap personil
yang terlibat dalam peledakan. Blast
Superintendent akan melampirkan Laporan
Misfire ini untuk pembuatan Incident Report.
45. Batas waktu untuk kegiatan peledakan adalah
sebagai berikut (untuk peledakan pukul 15.00):
Selesai pengisian lubang: 14.00
Meeting peledakan: 14.15
Peledakan paling lambat : 15.00
Perpanjangan terhadap batas waktu
pengisian dan peledakan harus seijin

46. Blasting must have been completed before 5.00
pm. If for particular reasons it needs longer time
than 5.00PM, it must be notified to the Drill and
Blast Superintendent for any further action
approved by the Mine Technical Manager. He
must prepare the notification report of blasting
schedule change (refer to attachment 1).
47. Any signs in the form of hand-signals, horn-
signal or any other type of communication other
than verbal communication are prohibited in
blasting process.
48. All the Leading Blasters, Blaster on duty, and
the Assistant Blaster who are responsible to this
procedure must have Class II Blasted Certificate
and pass the written test to test their knowledge
about blasting procedure.
49. Before anyone under training process is allowed
to use firing gun, he must own Class II Blasted
Certificate and pass the written test to indicate
their comprehension about blasting procedures.
50. The trainer shall always be right beside the
trainee who is holding the firing gun.
51. The Blaster on duty shall accomplish the
following tasks:
Obtain blast design/map and conduct
blast-plan meeting
Hook up the main initiator to the initiating
point
Countdown
Connect and pull the firing gun
Make sure the first and the all clear
announcement and inform to Blast
Controller
The above tasks shall not be divided or
delegated to other people.

harus memiliki sertifikat Juru Ledak Kelas II
dan lulus tes tertulis untuk memastikan
mereka mempunyai pengetahuan yang cukup
terhadap prosedur peledakan.
49. Orang yang di training di bawah pengawasan
trainer harus mempunyai sertifikat Juru Ledak
kelas II dan lulus tes tertulis untuk
menunjukkan pengetahuan mereka mengenai
prosedur sebelum mereka diizinkan
menggunakan starter gun.
50. Trainer harus selalu dapat menjangkau
(berada disamping) orang yang ditraining
yang sedang memegang starter gun.
51. Blaster Yang Bertugas harus melengkapi
tugas-tugas berikut :
Mendapatkan peta peledakan dan
melakukan meeting rencana peledakan
Menyambung inisiasi utama ke titik inisiasi
(lead in line utama ke lokasi peledakan)
Menghitung mundur
Menyambung dan menekan starter gun
Memastikan pemberitahuan aman pertama
dan keseluruhan dan menyampaikan
kepada blast controller
Tugas-tugas tersebut tidak boleh dibagi-bagi

dipersilahkna membuka jalan menuju area peledakan dan radio channel 1 dapat dipergunakan kembali. Terima kasih atas
kerjasamanya
dilanjutkan dengan bunyi sirene selama 5 detik.
