PELAYANAN KEFARMASIAN RUMAH SAKIT

PENDAHULUAN

 Di dalam Undang-Undang Nomor 44 Tahun 2009 tentang Rumah Sakit dan

Pasal 21 ayat (4) Peraturan Pemerintah Nomor 51 Tahun 2009 tentang
Pekerjaan Kefarmasian, perlu menetapkan Peraturan Menteri Kesehatan tentang
Standar Pelayanan Kefarmasian di Rumah Sakit.
 SK Menkes RI Nomor. 1197/Menkes/SK/X/2004 tentang Standar Pelayanan Farmasi
di Rumah Sakit dicabut dan dinyatakan tidak berlaku.
 SK Menkes RI Nomor.1333/MENKES/XII/1999 tentang Standar Pelayanan Rumah
Sakit
 DEFINISI

 Rumah Sakit adalah institusi pelayanan kesehatan yang menyelenggarakan

pelayanan kesehatan perorangan secara paripurna yang menyediakan pelayanan
rawat inap, rawat jalan, dan gawat darurat.

 Standar Pelayanan Kefarmasian adalah tolak ukur yang dipergunakan sebagai

pedoman bagi tenaga kefarmasian dalam menyelenggarakan pelayanan kefarmasian.

 Pelayanan Kefarmasian adalah suatu pelayanan langsung dan bertanggung jawab

kepada pasien yang berkaitan dengan sediaan farmasi dengan maksud mencapai
hasil yang pasti untuk meningkatkan mutu kehidupan pasien.
 Tugas Instalasi Farmasi Rumah Sakit, meliputi:

1. menyelenggarakan, mengkoordinasikan, mengatur dan mengawasi seluruh kegiatan Pelayanan

Kefarmasian yang optimal dan profesional serta sesuai prosedur dan etik profesi;
2. melaksanakan pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai
yang efektif, aman, bermutu dan efisien;
3. melaksanakan pengkajian dan pemantauan penggunaan Sediaan Farmasi, Alat Kesehatan, dan
Bahan Medis Habis Pakai guna memaksimalkan efek terapi dan keamanan serta meminimalkan
risiko;
4. melaksanakan Komunikasi, Edukasi dan Informasi (KIE) serta memberikan rekomendasi
kepada dokter, perawat dan pasien;
5. berperan aktif dalam Tim Farmasi dan Terapi;
6. 6. melaksanakan pendidikan dan pelatihan serta pengembangan Pelayanan Kefarmasian;
7. memfasilitasi dan mendorong tersusunnya standar pengobatan dan formularium Rumah Sakit.
 Fungsi Instalasi Farmasi Rumah Sakit, meliputi:
1. memilih Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai sesuai kebutuhan
pelayanan Rumah Sakit;
2. merencanakan kebutuhan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai
secara efektif, efisien dan optimal;
3. mengadakan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai berpedoman
pada perencanaan yang telah dibuat sesuai ketentuan yang berlaku;
4. memproduksi Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai untuk
memenuhi kebutuhan pelayanan kesehatan di Rumah Sakit;
5. menerima Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai sesuai dengan
spesifikasi dan ketentuan yang berlaku;
6. menyimpan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai sesuai dengan
spesifikasi dan persyaratan kefarmasian;
7. mendistribusikan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai ke unit-unit
pelayanan di Rumah Sakit;

8. melaksanakan pelayanan farmasi satu pintu;

9. melaksanakan pelayanan Obat “unit dose”/dosis sehari;
10. melaksanakan komputerisasi pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis
Pakai (apabila sudah memungkinkan);
11. mengidentifikasi, mencegah dan mengatasi masalah yang terkait dengan Sediaan Farmasi, Alat
Kesehatan, dan Bahan Medis Habis Pakai;
12. melakukan pemusnahan dan penarikan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai
yang sudah tidak dapat digunakan;
13. mengendalikan persediaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai;
14. melakukan administrasi pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai.
Pengaturan Standar Pelayanan Kefarmasian di
Rumah Sakit bertujuan untuk:

1. meningkatkan mutu Pelayanan Kefarmasian;

2. menjamin kepastian hukum bagi tenaga kefarmasian; dan
3. melindungi pasien dan masyarakat dari penggunaan Obat yang tidak
rasional dalam rangka keselamatan pasien (patient safety).
