disebabkan oleh PRM (41 juta kematian)

Di tingkat nasional, hasil Riskesdas 2018 menunjukan

prevelensi PTM mengalami kenaikan jika dibandingkan
dengan Riskesdas 2013.

Pandu PTM dilaksanakan secara
komprehensif dan berkelanjutan dengan tetap
mengacu pada pedoman tata laksana
penyakit yang berlaku.

A. Sasaran Pandu PTM

Pengunjung berusia 15 tahun keatas yang
dating ke Puskesmas Melong Asih untuk
kunjungan sakit maupun kunjungan sehat.

B. Algoritma Pandu PTM

Berisi alur dan penjelasan mulai dari
identifikasi factor resiko melalui anamnesis,
pengukuran dan pemeriksaan serta
pemeriksaan prediksi resiko dan penanggakan
diagnosis PTM (bila ada), serta rujukan jika
diperlukan.
