VENTILATOR adalah sebuah alat elektromedis yang berfungsi untuk memberikan bantuan nafas buatan kepada pasien-pasien yang gagal napas. Sebelum ada alat ventilator kita mengenal alat AMBU BAG sebagai alat bantu napas. VENTILATOR selain berfungsi sebagai alat bantu napas juga berfungsi melatih otot paru pasien untuk napas spontan, sehingga pasien pada akhirnya bisa lepas dari alat ventilator, yang disebut dengan proses WEANING. Tapi kalau pada AMBU BAG tidak dapat dilakukan proses WEANING.

Di sini ada 2 gambar Ambu Bag, dengan Oxygen 100%. Gambar Ambu Bag yang di atas kondisinya belum ditekan, jadi TEST LUNG nya belum mengembang. Ketika Ambu Bag ditekan maka terjadi aliran gas Oxygen dari BAG menuju TEST LUNG, sehingga TEST LUNG mengembang dan berisi gas Oxygen yang disebut Tidal Volume.

TRIGGER OR SENSITIVITY
TRIGGER/SENSITIVITY, adalah jalur komunikasi antara pasien dengan mesin ventilator. Indikatornya menggunakan Tekanan negatip dari pernapasan pasien. Nilainya dari -0,5 s/d -20 cmH2O. Semakin angkanya kecil, maka semakin sensitif triggernya. Dan sebaliknya, semakin besar angkanya, semakin tidak sensitif. TRIGGER/SENSITIVITY, digunakan pada modus ASSISTED, ASSIST CONTROL, SIMV, dan SPONTAN / CPAP.

CONTROL, artinya pemberian bantuan napas pada pasien sepenuhnya dikontrol oleh mesin ventilator. Baik Tidal Volume atau Tekanannya maupun Respiratory Ratenya sepenuhnya diatur oleh mesin ventilator. ASSISTED, artinya pemberian bantuan napas pada pasien, di mana Respiratory Ratenya ditentukan oleh pasien sendiri, tapi Tidal Volume atau Tekanannya masih sepenuhnya ditentukan oleh mesin ventilator. ASSIST CONTROL, artinya pemberian bantuan napas pada pasien, di mana Tidal Volume atau Tekanannya masih sepenuhnya ditentukan oleh mesin ventilator, tapi Respiratory Rate pada data pasien akan selalu lebih besar atau sama dengan [] Respiratory Rate dari mesin ventilator. Artinya bila ada permintaan/trigger dari pasien, maka RR pasien akan menjadi lebih besar dari RR mesin ventilator. Tapi kalau tidak ada permintaan/trigger dari pasien, maka RR pasien sama dengan RR mesin ventilator.
SIMV [Synchronies Intermittent Mandatory Ventilation], adalah pemberian bantuan napas dari mesin ventilator kepada pasien, di mana selain ada napas Mandatory dan Assist juga ada napas Spontan. Di modus SIMV ini Sensitivity selalu aktif, dan RR setting mulai diturunkan tahap demi tahap bila kondisi pasien membaik. Sehingga diharapkan napas Spontan pasiennya menjadi lebih banyak.
SPONTAN / CPAP [Continuous Positive Airway Pressure], adalah pemberian bantuan napas dari mesin ventilator kepada pasien, di mana semua jenis pernapasannya adalah Spontan, tapi harus ada PEEP nya. Kalau PEEP nya sama dengan 0 [nol], maka modusnya menjadi Spontan bukan CPAP. Di sini sensitivitynya juga selalu aktif.
PRESSURE SUPPORT
PRESSURE SUPPORT [PS], adalah tekanan tambahan yang diberikan mesin ventilator pada saat pasien bernapas Spontan. Penggunaan Pressure Support yang pertama adalah untuk menghilangkan Airway Resistance pada ETT pasien. Dan dalam penggunaan yang ke dua, Pressure Support dipakai untuk membantu pengembangan paru-paru pasien karena otot parunya masih lemah. PRESSURE SUPPORT [PS], digunakan pada modus SIMV dan SPONTAN / CPAP.
PRESSURE CONTROL, artinya adalah yang dikontrol oleh mesin ventilator dalam proses pemberian bantuan napas buatan adalah Pressure Control, sedangkan Tidal Volumenya tidak ditentukan, tapi dapat dilihat di monitor dan nilainya akan selalu berubah-ubah tergantung dari nilai Compliance paru pasiennya dan nilai Resistance pada saluran pernapasannya. Di sini yang stabil adalah nilai Pressurenya. Pengamannya adalah High Vti Limit, supaya paru-paru pasiennya tidak trauma.
VOLUME CONTROL, artinya adalah yang dikontrol oleh mesin ventilator dalam proses pemberian bantuan napas buatan adalah Tidal Volume, sedangkan Pressurenya tidak ditentukan, tapi dapat dilihat di monitor dan nilainya akan selalu berubahubah tergantung dari nilai Compliance paru pasiennya dan nilai Resistance pada saluran pernapasannya. Di sini yang stabil adalah nilai Tidal Volumenya. Pengamannya adalah High Pressure Limit, supaya paru-paru pasiennya tidak trauma.

Disconnect alarms can be maintained by adjusting the set flow downward with prongs off the patient until the low pressure or disconnect alarms sound
In this way, the pressure drops below the auto set threshold if the prongs come off the nose
Many nasal prong systems replace the wye with the prongs
The
proximal pressure line is then connected to the prongs at the port designed into the prongs
F&P
The F&P nasal prongs replace the circuit wye but the pressure tap is centrally located
The clear proximal pressure line is then attached to the right side pressure connection port
Plug the power cord into AC Power Connect the air and O2 high pressure gas sources
Place the blue expiratory membrane into the expiratory valve cover
The diaphragm has the word DOWN written on one side to indicate that this side should face down toward the cover
plug the humidifier into A/C power
The exhalation cover is then placed in the From Patient area under the front of the Inspiration by twisting it into position
The patient circuit is basically a tube out, a wye, and a tube back including an optional proximal flow sensor
The Flow sensor is attached to the barbed fittings under the nose cone by lining up the blue tube with the blue barb The nebulizer is connected to the barbed fitting Delivers 6 8 lpm of flow
