Abstrak
Pendahuluan : Salah satu manajemen nyeri pascaoperasi seksio sesarea yang dilakukan di
RSUP Dr. Hasan Sadikin Bandung adalah pemberian pethidine 100 mg dan ketorolac 30 mg
yang dilarutkan dalam kristaloid 500 cc diberikan dengan kecepatan 20-25 tetes per menit.
Tujuan : Menggambarkan penggunaan manajemen nyeri pascaoperasi seksio sesarea
menggunakan pethidine dan ketorolac.
Metode : Serial kasus ini melibatkan 10 pasien pascaoperasi seksio sesarea emergensi yang
mendapatkan anestesi spinal intraoperatif. Pascaoperasi seluruh pasien mendapatkan
pethidine 100 mg dan ketorolac 30 mg yang dilarutkan dalam 500 cc kristaloid diberikan
secara intravena dengan kecepatan 20-25 tetes per menit. Dilakukan penilaian nyeri
menggunakan skor VAS pada jam kedua dan keduabelas pascaoperasi.
Hasil : Pada jam kedua pascaoperasi, didapatkan 9 pasien memiliki skor VAS 1-3 dan 1
pasien memiliki skor VAS 6. Sedangkan pada jam keduabelas pascaoperasi, 5 pasien
memiliki skor VAS 1-3 dan 5 pasien memiliki skor VAS 4-6. Dari hasil tersebut didapatkan 2
pasien memiliki skor VAS yang tetap dan 8 pasien peningkatan skor VAS.
Kesimpulan : Pemberian analgetik dengan pethidine 100 mg dan ketorolac 30 mg yang
dilarutkan dalam kristaloid 500 cc dengan kecepatan 20-25 tetes per menit adalah
subanalgetik sehingga skor VAS pasien pascaoperasi seksio sesarea masih cenderung
meningkat.
Abstract
Background : One of the postoperative pain management for cesarean section at RSUP
Dr. Hasan Sadikin Hospital Bandung uses pethidine 100 mg and ketorolac 30 mg
dissolved in 500 cc crystalloid fluid and given at a rate of 20-25 drops per minute .
Objectives : Describing post-operative management of cesarean section using pethidine
and ketorolac.
Methods : This case series involved 10 postoperative patients who underwent emergency
cesarean section under spinal anesthesia. Postoperatively all patients received 100 mg
pethidine and 30 mg ketorolac dissolved in 500 cc of crystalloid administered
intravenously at rate 20-25 drops per minute. VAS score was used for pain asessment at
the second and twelfth hour postoperatively.
Results :In the second postoperative hour, 9 patients VAS score 1-3 and 1 patient had
VAS score 6. Whereas at the twelfth hour postoperatively, 5 patients had VAS score 1-3
and 5 patients had VAS score 4-6. From these results, 2 patients had a fixed VAS score
and 8 patients had an increased VAS score.
Conclusions : The administration of analgesic drugs with pethidine 100 mg and
ketorolac 30 mg dissolved in 500 cc crystalloid at a rate of 20-25 drops per minute is
subanalgetic dose so that VAS score in postoperative patients with cesarean section still
tend to increase.
