Kesimpulan / Conclusion
1. Berdasarkan hasil sesuai lingkup inspeksi, peralatan tersebut di atas memenuhi persyaratan regulasi
pemerintah dan referensi yang diacu.
Based on result with related scope of inspection, the equipment stated above was found comply to
government regulation and referred standards.
2. Sertifikat ini mendeksripsikan temuan pada saat dan di tempat pemeriksaan. PT. Pranata Dwi Mulia
menerbitkan sertifikat ini yang dibuat dengan sebenar-benarnya.
This certificate describes the findings on and at the examination site. PT. Pranata Dwi Mulia publishes
this certificate which is made in truth.
3. Pesawat Angkat ini hanya dapat dioperasikan sesuai dengan beban kerja aman diatas oleh operator
dan rigger yang telah mempunyai sertifikat Bidang Migas.
This Lifting Equipment can only be operated in accordance with the safe workload above by the
operators and riggers who already have a certificate of Oil and Gas Field.
4. PT. Berlian Amal Perkasa bertanggung jawab sepenuhnya atas keselamatan pengoperasian Peralatan
tersebut di atas.
PT. Berlian Amal Perkasa must take full responsibility for the safety the operation of the equipment
above.
5. Selambat-lambatnya tiga bulan sebelum berakhirnya masa berlaku Sertifikat, PT. Berlian Amal
Perkasa wajib melakukan pemeriksaan teknis terhadap peralatan tersebut di atas.
No later than three months before the end of the effect of certificate , PT. Berlian Amal Perkasa are
required to conduct examination technical of the equipment above.
6. Apabila terjadi hal-hal yang dapat menyebabkan Peralatan tersebut tidak layak dan tidak aman untuk
dioperasikan maka Sertifikat Inspeksi ini tidak berlaku.
If there is things that can cause equipment is not eligible to and uncomfortable to be operated so this
Certificate of Inspection does not apply.

Sertifikat ini berlaku sejak tanggal diterbitkan. (4 tahun)

This certificate take effect as of the date issued. ( 4 years)
