ABSTRACT
Integrated, complete and continuous patient care within the hospital service setting has become a
necessity. The complexity of patient problems and service management that involves multiple professions
has the potential to cause service fragmentation that can have implications for patient safety issues, if
team collaboration is ineffective, therefore interprofessional collaboration is needed as an effort to create
synergistic and mutual patient care so patients get a complete and sustainable service. . Hospital managers
and care professionals generally understand this need, but access to the collaboration model and how it is
implemented is still unclear. Interprofessional communication used is SBAR (Situation-Background
Assessment-Recommendation)
