Surat N1 s.d N6 dari Kelurahan di tempat domisili yang masih berlaku (3 bulan sejak
surat diterbitkan).

N1 - N6 Letter from District Administrative Office/ Kelurahan in Indonesia where the

applicant lived. The letter must be valid 3 month since it was issued.

Surat Rekomendasi Nikah dari Kantor Urusan Agama (KUA) dan atau Kantor Catatan
Sipil setempat.

Pemohon (Kedua calon mempelai) diharap datang dan mengisi serta menandatangani
surat pernyataan status perkawinan/affidavit diatas materai 1 (satu) buah Rp 10.000,-
(materai disediakan oleh pemohon).

Applicant (Bridge and Groom) must come to the Embassy and complete the form and
affidavit letter on 1(one) of the duty stamp/materai Rp.10.000,- (the duty stamp is
prepare by the applicant).
