Definisi
Cedera Kepala Tidak Spesifik (Cedera Kepala Sedang dan Cedera Kepala Berat) adalah
cedera otak yang diklasifikasikan berdasarkan tingkat kesadaran yang diukur dengan
menggunakan skala GCS (Glasgow Coma Scake), GCS 9-12 untuk Cedera Kepala Sedang
dan GCS<8 Cedera Kepala Berat
Anamnesis
- Identifikasi Pasien: Nama, Umur, Jenis Kelamin, Suku, Agama, Pekerjaan, Alamat
- Keluhan Utama
- Mekanisme trauma
- Waktu dan perjalanan trauma
- Pernah pingsan atau sadar setelah trauma
- Amnesia retrograde atau anterograde
- Keluhan: nyeri kepala seberapa berat, penurunan kesadaran, kejang, vertigo
- Riwayat mabuk, alcohol, narkotika, pasca operasi kepala
- Penyakit penyerta: epilepsy, jantung, asma, riwayat operasi kepala, hipertensi dan
diabetes mellitus serta gangguan faal pembekuan darah
Pemeriksaan Fisik
Perhatikan, catat, dan
Pemeriksaan Evaluasi perbaiki
Patensi saluran napas? Suara
A. Airway tambahan? Obstruksi?
B. Breathing Apakah oksigenasi? Efektif? Rate dan depth gerakan
dada air entry Sianosis
Secondary Survey
Pemeriksaan Status Generalis
Pemeriksaan dengan inspeksi, palpasi, perkusi, dan auskultasi, serta pemeriksaan khusus
untuk menentukan kelainan patologis, dengan metode:
- Intoksikasi alcohol
- Stroke
- AVM
Pemeriksaan Penunjang
1. X-ray Servical
X-ray servical dikerjakan pada pasien CKS dan CKB untuk menyingkirkan adanya
kemungkinan cedera servical. Sensitivitas X-ray servical 70-80%.
2. X-Ray thorax
X-ray thorax dilakukan pada pasien trauma yang tidak membutuhkan CT-Scan. X-ray
dikerjakan berdasarkan mekanisme cedera dan temuan klinis. X-ray thorax dikerjakan
pada pasien trauma tembus dada, punggung atau perut yang tidak membutuhkan CT-
Scan
3. CT-scan adlah modalitas yang dipilih pada fase akut pada trauma kepala dan
sebaiknya dikerjakan secepatnya. CT Scan kepala direkomendasikan dikerjakan pada
semua pasien cedera otak dengan GCS 13 atau kurang. CT Scan evaluasi dapat
dikerjakan bila didapatkan deficit neurologis.
4. CT-Scan Whole Body
Whole Body CT digunakan pad akasus multitrauma untuk mengurangi waktu
diagnosis, dapat digunakan pasien dengan hemodinamik tidak stabil
Tatalaksana
1. Operasi
Indikasi untuk tindakan bedah segera pada Cedera Kepala Sedang atau Cedera Kepala
Berat berdasarkan status neurologis, biasanya GCS dan temuan CT Scan yang sesuai
kriteria seperti volume perdarahan yang besar atau ketebalan dan bukti adanya efek
massa termasuk midline shift.
- Tindakan bedah direkomendasikan untuk evakuasi EDH dengan volume >30 ml
berapapun GCS pasien juga pada EDH akut dengan GCS <8 dengan pupil anisokor.
- Tindakan bedah direkomendasikan untuk SDH akut dengan tebal >10 mm atau MLS
>5 mm berapapun GCS pasien.
Tindakan bedah juga direkomendasikan pada pasien dengan GCS <8 atau turun 2 poin
dibandingkan saat pasien datang dana tau pasien dengan pupil asimetris dan dilatasi
dana tau TIK >20 MMhG.
- Indikasi tindakan bedah pada ICH belum terlalu jelas. Beberapa sumber menyebutkan
evakuasi ICH pada volume >50 ml atau GCD 6-8 pada pasien dengan ICH di
temporal atau frontal volume >20 ml dengan MLS >5 mm dan atau kompresi sisterna
pada CT Scan.
- Pemasangan ICP monitor dilakukan pada pasien Cedera Kepala Berat (GCS 3-8
setelah proses resusitasi) dengan CT Scan kepala abnormal (hematoma, contusion,
edema serebri atau penyempitan sisterna basalis). ICP monitor juga dipasang pada
pasien Cedera Kepala Berat dengan CT Scan kepala normal jika didapatkan 2 atau
lebih dari hal berikut :
Usia >40 tahun
TDS < 90 mmHg
Postural bilateral atau unilateral .

Prognosis dipengaruhi :
- Usia
- Status neurologis awal
- Jarak antara trauma dan tindakan bedah
- Edema cerebri
- Kelainan intracranial dan seperti kontusional, hematom subarachnoid, dan hematom
epidural
- Factor ekstrakranial
