Asuhan Keperawatan Pada Pasien Chronic Kidney Disase (CKD) Dengan Penerapan
Virgin Coconut Oil (VCO) Dalam Menurunkan Pruritus Pasien Chronic kidney Disease
(CKD) Di Ruangan Agus Salim Rumah Sakit Tk III dr. Reksodiwiryo Padang 2024.

ABSTRAK
Di Indonesia prevelensi kejadian CKD berjumlah naik dari 2% menajdi 3,8% hal ini
meningkat sekitar 1,8%. Provinsi Sumatera Barat dengan prevelensi penyakit CKD berjumlah
2690 orang pada tahun 2017 meningkat menjadi 4076 jiwa ditahun 2018.. Salah satu keluhan
yang sering terjadi pada pasien gagal ginjal kronik adalah pruritus atau gatal-gatal. Intervensi
yang dapat diterapkan adalah penerapan pemberian ( Virgin Coconut Oil) dengan metode
studi kasus asuhan keperawatan. Hasil pengkajian yang didapatkan adalah klien mengatakan
sesak nya sudah berkurang. Klien mengatakan merasa pusing, lemah, lemas. Klien juga
mengatakan kulit terasa gatal dan kering pada bagian punggung. Klien mengatakan gatal
pada bagian punggung membuatnya kurang nyaman akibat nya klien sering terbangun
malam, klien merasa kulitnya sangat kering dan gatal, sehingga klien sering mengaruknya
dan menyebabkan luka lecet kecil dibagian punggung. Diagnosa keperawatan yang muncul
yaitu perfusi perifer tidak efektif, keletihan dan gangguan integritas kulit/jaringan. Intervensi
keperawatan pada diagnosa perfusi perifer tidak efektif yaitu perawatan sirkulasi dan
manajemen sensasi perifer, pada diagnosa keletihan yaitu manajemen energi dan edukasi
aktivitas/istirahat, pada diagnosa gangguan integritas kulit/jaringan yaitu perawatan integritas
kulit dan perawatan luka. Implementasi yang dilakukan untuk masing-masing diagnosa
selama 6 hari berturut-turut sama halnya dengan penerapan pemberian ( Virgin Coconut Oil)
untuk menurunkan skala pruritus pada pasien CKD dilakukan sesuai dengan SOP yang
digunakan. Evaluasi pada diagnosa perfusi perifer tidak efektif dan diagnosa keletihan
masalah belum teratasi , dan untuk diagnosa gangguan integritas kulit/jaringan masalah
teratasi sebagian. Diharapkan menjadi alternative dalam pemberian asuhan keperawatan pada
pasien CKD dengan menerapakan pemberian ( Virgin Coconut Oil) sebagai tindakan untuk
menurunkan prutitus.

Nursing Care for Chronic Kidney Disease (CKD) Patients Using Virgin Coconut Oil
(VCO) in Reducing Pruritus in Chronic Kidney Disease (CKD) Patients in the Agus
Salim Room, Kindergarten Hospital, Dr. Reksodiwiryo Padang 2024.

ABSTRACT
In Indonesia, the prevalence of CKD has increased from 2% to 3.8%, this has increased by
around 1.8%. West Sumatra Province with a prevalence of CKD disease numbered 2690
people in 2017, increasing to 4076 people in 2018. . One of the complaints that often occurs
in patients with chronic kidney failure is pruritus or itching. The intervention implemented
was the application of (Virgin Coconut Oil) to reduce the scale of pruritus in CKD patients.
The results of the assessment obtained were that the client said his shortness of breath had
reduced. The client says he feels dizzy, weak, weak. The client also said the skin felt itchy
and dry on the back. The client said the itching on the back made him uncomfortable because
the client often woke up at night. The client said that after being treated for 2 days, the client
felt that her skin was very dry and itchy, so the client often scratched it and caused small
abrasions on her back. The nursing diagnoses that emerged were ineffective peripheral
perfusion, fatigue and impaired skin integrity. Nursing intervention in the diagnosis of
ineffective peripheral perfusion is circulation care and peripheral sensation management, in
the diagnosis of fatigue, namely energy management and activity/rest education, in the
diagnosis of skin integrity disorders, namely skin integrity care and wound care. The
implementation carried out for each diagnosis for 6 consecutive days is the same as the
application of giving (Virgin Coconut Oil) to reduce the scale of pruritus in CKD patients
carried out for 6 consecutive days with the same time and treatment. Evaluation for the
diagnosis of ineffective peripheral perfusion and diagnosis of fatigue, the problem has not
been resolved, and for the diagnosis of skin/tissue integrity disorders, the problem has been
partially resolved. It is hoped as an alternative in providing nursing care to CKD patients who
experience pruritus.
