LATAR BELAKANG
Tahun 2007 angka kematian akibat PPOK
menduduki peringkat ke-6 dari 10 penyebab
kematian di Indonesia dan prevalensi PPOK
rata-rata sebesar 3,7% (Riskesdas, 2013).

●Tubular shadows atau farm lines
terlihat bayangan garis-garis yang
parallel, keluar dari hilus menuju apeks
paru. Bayangan tersebut adalah
bayangan bronkus yang menebal.
●Corak paru yang bertambah
●Gambaran defisiensi arteri, terjadi
overinflasi, pulmonary oligoemia dan bula.
Keadaan ini lebih sering terdapat pada
emfisema panlobular dan pink puffer.
●Corakan paru yang bertambah.

RIWAYAT
PENYAKIT

Pada Jumat, 23 Maret 2018 pasien datang ke UGD RSUD Klungkung pukul 09.00 WITA diantar oleh
keluarga dengan keluhan sesak napas sejak pukul 06.00 WITA. Di UGD, pasien diobservasi selama 3
jam, setelah diobservasi kemudian pasien diberikan pulang. Kemudian hari Minggu, 25 Maret 2018
pasien kembali ke UGD RSUD Klungkung dengan keluhan sesak napas dan nyeri ulu hati. Diobservasi
selama 4,5 jam. Dari hasil observasi didapatkan hasil TD = 130/90 mmHg, S = 36 0C, N = 98x/menit, RR =
25x/menit. Keadaan umum compos mentis (E=4, V=5, M=6). Di UGD diberikan terapi Salbutanol 3x2 mg,
Ranitidin 2x1 tab, dan Asetil Sistein 3x200 mg. Kemudian pasien dipindahkan ke Ruang Keodondong
pukul 16.00 WITA. Saat pengkajian di ruangan didapatkan hasil bahwa pasien mengeluh sesak dan
nyeri ulu hati. TD = 120/80 mmHg, S = 36,10C, N = 80x/menit, RR = 24x/menit.

 PENGKAJIANTERAPI DOSIS INDIKASI

Levofloxacin 1 x 750 mg digunakan untuk mengobati infeksi bakteri seperti infeksi kulit, jaringan
lunak, dan infeksi prostat, pneumonia, infeksi saluran kemih, sinusitis.

Combivent 1 amp bronkospasme yang berhubungan dengan PPOK

Fartison 2 x 200 mg untuk mengencerkan dahak

Paracetamol 3 x 500 mg untuk mengurangi rasa nyeri dan menurunkan demam

Omeprazole 2 x 20 mg menurunkan kadar asam yang diproduksi di dalam lambung

Asetilsistein 3 x200 mg berfungsi untuk mengencerkan dahak yang menghalangi saluran

pernapasan.

IVFD RL 7 Tetes / menit mengembalikan keseimbangan elektrolit pada keadaan dehidrasi dan
syok hipovolemik

O2 nasal kanul 4 Liter/menit Membantu pernapasan pasien
