 Definisi Ileus Obstruksi🤨🧐
Ileus atau obstruktif usus adalah suatu gangguan aliran normal isi usus sepanjang saluran
isi usus. Obstruktif usus dapat akut dengan kronik, partial atau total. Ileus obstruktif
terjadi ketika isi usus tidak dapat melewati saluran gastrointestinal (Nurarif & Kusuma,
2015). Ileus adalah gangguan atau hambatan isi usus yang merupakan tanda adanya
obstruktif usus akut yang segera membutuhkan pertolongan atau tindakan (Indrayani,
2013).
 Etiologi Ileus Obstruksi
Menurut Indrayani (2013) penyebab terjadinya ileus obstruksi pada usus halus antara lain :

■ Hernia inkarserata
● Hernia inkarserata adalah kondisi ketika usus terjebak (masuk) dan terjepit di dalam pintu
hernia sehingga dapat memicu gejala seperti nyeri hebat, mual, muntah dan sulit buang air
besar.
■ Non hernia inkarserata, dibagi beberapa macam antara lain:
● Adhesi atau perlekata usus
● Invaginasi
● Askariasis
● Volvulus
● Tumor
● Batu empedu
 Manifestas Uleus Obstruksi

Menurut (Price & Wilson, 2012) gejala pada obstruksi usus halus antara lain :
a. Peradangan pada abdomen Muntah, frekuensi muntah beragam sesuai dengan letak obstruksinya.
b. Nyeri, gejala ini muncul biasanya menyerupai kejang dan dipertengahan abdomen (terutama di
daerah paraumbilikaslis)
c. Konstipasi sering terjadi pada obstruksi di usus besar, tetapi flatus dan feses mungkin dapat
dikeluarkan pada permulaan obstruksi usus halus.
 Patofisiologi Ileus Obstruksi

Dalam patofisiologi dari ileus dapat menyebabkan akumulasi cairan dan gas
pada peningkatan tekanan intra luminal, disfungsi mikrosirkulasi dinding usus, dan
gangguan penghalang mukosa sehingga menyebabkan perpindahan cairan, periotonitis
transmigrasi dan hipovolemia (Vilz et al., 2017). Obstruksi usus terjadi karena adanya
lumen usus yang tersumbat yang dapat menjadi perkembangan bakteri dan mengakibatkan
akumulasi pada gas dan cairan.
 Komplikasi Ileus Obstruksi

Menurut (Smith & Kashyap, 2021) komplikasi pada obstruksi usus yaitu
abses intra abdominal (kumpulan nanah), sepsis, infeksi akibat dari peritonitis yang tidak
ditangani dengan cepat, dehiscence luka, pneumonia aspirasi dari proses muntah, sindrom
usus pendek karena ada absorb toksin dalam rongga peritoneum

 Pemeriksaan Penunjang Ileus Obstruksi

• Tes Laboratorium
• Ultrasonografi (USG) abdomen
• Foto polos abdomen
• (setengah duduk), dan Left lateral decubitus (LLD/ tidur miring)
Digunakan untuk menentukan diagnosa dari obstruksi strangulasi dan mengetahui
penyebab dari abdominal akut lain,

1.Diagnosis aktual
Diagnosis ini menggambarkan respon klien terhadap kondisi kesehatan atau proses
kehidupannya yang menyebabkan klien mengalami masalah kesehatan.
Tanda/gejala mayor dan minor dapat ditemukan dan divalidasi pada klien.
2. Diagnosis Risiko
Diagnosis risiko menggambarkan respon klien terhadap kondisi kesehatan atau
proses kehidupannya yang dapat menyebabkan klien berisiko mengalami
masalah kesehatan.
3. Diagnosis Promosi Kesehatan
Diagnosis ini menggambarkan adanya keiinginan dan motivasi klien untuk
meningkatkan kondisi kesehatannya ke tingkat yang lebih baik atau optimal.

 Implementasi
Berikut adalah beberapa langkah implementasi keperawatan yang dapat
dilakukan pada pasien dengan ileus obstruktif:
1. Stabilisasi Pasien:
oPastikan jalan napas bebas dan pernapasan baik.
o Berikan terapi cairan secara agresif untuk menjaga kecukupan cairan tubuh.
2. Pemasangan Selang Nasogastrik (NGT):
o Jika distensi abdomen berlebihan, pipa nasogastrik dapat dipasang untuk
dekompresi. NGT akan membantu mengurangi pembengkakan di perut dan mengalirkan isi
lambung ke luar.
3. Pengkajian dan Monitoring:
o Tentukan jumlah dan jenis cairan serta kebiasaan eliminasi pasien.o
Monitor membran kulit mukosa, turgor kulit, dan warna urine untuk memastikan cairan yang
tepat diberikan sesuai kebutuhan3
4. Hentikan Obat-Obatan yang Menyebabkan Ileus:
o Beberapa obat dapat memicu ileus obstruktif.
 Evaluasi

Penilaian terakhir proses keperawatan didasarkan pada tujuan keperawatan

yang ditetapkan. Penetapan keberhasilan suatu asuhan keperawatan didasarkan pada
perubahan perilaku dari kriteria hasil yang telah ditetapkan, yaitu terjadinya adaptasi pada
individu.
