PENDAHULUAN
A.Latar Belakang
RTS adalah alat yang mudah digunakan untuk triase trauma dan
perkiraan keparahan awal yang tidak memerlukan tes atau perangkat medis
canggih dan sangat berguna dalam pengaturan pra-rumah sakit dan gawat
darurat / Emergency department (ED) . Sistem penilaian fisiologis ini terdiri
dari Glasgow Coma Scale (GCS), tekanan darah sistolik / systolic blood
pressure (SBP) dan laju pernapasan /respiratory rate (RR). Parameter
dikonversi ke nilai kode (0, 1, 2, 3 atau 4) yang ditetapkan oleh rentang yang
ditentukan. Setiap nilai dikalikan dengan koefisien tertimbang sebelum
ditambahkan.

merupakan prediktor kelangsungan hidup yang paling banyak digunakan dan
terkemuka untuk penelitian dalam pengendalian kualitas manajemen trauma
dan pencegahan.
Skor Mekanisme, GCS, dan Usia dan Tekanan Arteri (MGAP) adalah
sistem penilaian trauma fisiologis yang baru-baru ini dikembangkan. Mirip
dengan TRISS, MGAP memanfaatkan mekanisme dan usia, yang merupakan
variabel penting yang mempengaruhi prognosis pasien trauma. Skor akhir
dapat dengan mudah diperoleh setelah penambahan sederhana dari beberapa
angka. Sementara GCS ditransformasikan ke nilai kode mulai dari 0 hingga 4
dalam RTS, MGAP terdiri dari skor GCS aktual tanpa variasi karena nilai yang
sangat informatif dan perhitungan yang relatif tidak bias. Selain itu, SBP 120
mmHg dipilih untuk ambang, sedangkan 90 mmHg adalah cutoff pertama
untuk mengurangi nilai kode dalam RTS (Sartorius D dkk, 2010). GCS, Usia
dan Tekanan Arteri Sistolik (GAP) adalah sistem penilaian yang
disederhanakan dengan menghapus mekanisme dari MGAP (Kondo Y dkk,
2011). MGAP dan GAP terbukti lebih unggul dari RTS dalam prediksi
mortalitas pasien trauma. Namun, mereka tetap kalah dengan TRISS.

diidentifikasi sebagai cut off untuk hipotensi, di mana peningkatan signifikan
dalam kematian terjadi (Hasler RM dkk, 2011).
RTS adalah sistem penilaian yang dinilai luas tetapi agak ketinggalan
jaman untuk prediksi kematian trauma. Oleh karena itu, ada modifikasi dari
RTS dan rancangan skor trauma baru berdasarkan perkembangan terakhir
dalam pengaturan trauma. Gagasan utama meliputi penerapan skor GCS aktual
alih-alih kode GCS, revisi interval tekanan darah sistolik yang digunakan untuk
nilai kode dan penggabungan SpO2dan bukan RR . Ukuran ini disebut Skor
Trauma Baru / New Trauma Score (NTS).

LANDASAN EBNP
A. Konsep Teori
1. Pengertian
Revised Trauma Scale (RTS) adalah salah satu skor fisiologis yang
lebih umum. Menggunakan 3 parameter sebagai berikut: (1) skala Glasgow
koma (GCS), (2) (SBP) tekanan darah sistolik dan (3) frekuensi pernafasan
(RR). Pada parameter tekanan darah yang digunakan adalah sistolik karena
tekanan sistolik sendiri adalah tekanan darah pada saat terjadi kontraksi otot
jantung, sehingga dapat disimpulkan bahwa kontraksi otot jantung bisa
dilihat dari tekanan sistolik.RTS diperkenalkan oleh Champion et al (1983),
sistem ini paling banyak digunakan sistem penilaian fisiologis. Sistem RTS
ini menggabungkan nilai GCS dengan laju respirasi dan tekanan darah
sistolik (Salim, 2015). Respiratory rate biasanya digunakan dalam
pengukuran klinis tetapi tidak mempunyai keakuratan yang tinggi karena
untuk pasien tertentu seperti nyeri atau stress psikologis akan
mempengaruhi jumlah RR.
New Trauma Score (NTS) adalah perbaruan dari Revised Trauma
Score dengan menggunakan 3 parameter yaitu skala Glasgow Coma (GCS),
tekanan darah sistolik dan saturasi oksigen (SPO2). New Trauma Score
menggunakan parameter saturasi oksigen salah satunya karena saturasi
oksigen merupakan metode pemantauan yang objektif, efisien dan akurat
dalam skrinning paru (Jeong et al, 2017).

mengindikasikan pasien perlu dirujuk ke trauma centre atau dilakukan
tindakan medis segera, sedangkan nilai ≤ 11 menandakan harus segera

P : This was a prospective observational study using data from the trauma
registry of a tertiary hospital. Injury mechanisms were categorized as blunt
trauma, penetrating trauma, burn, drowning, hanging, asphyxia, poisoning
and heat or cold-related injury.
I :The trauma registry was originally developed as a part of Emergency
Departmentbased Injury In-depth Surveillance conducted by the Korea
Centers for Disease Control and Prevention. Informed consent was not
needed because the data were collected without identifiable personal
information. Data used for derivation were obtained from patients arriving
in the ED between July 1, 2014 and June 30, 2016. The inclusion criteria
consisted of (i) patients categorized with blunt or penetrating mechanisms

and (ii) age ≥ 15 years. The exclusion criteria consisted of (i) patients who
died before ED arrival, (ii) patients discharged or transferred from the ED.
We used the data from patients arriving at the same hospital between July 1,
2011 and June 30, 2013 for external validation purposes. This study was
approved by the Gyeongsang National University hospital institutional
review board (number 2016–09-008).

O :The NTS predicts in-hospital mortality substantially better than the RTS
and not inferior to the MGAP and GAP. We hope that the NTS will be a
useful tool for triage in trauma patients and will lead to an improvement in
trauma management.

P : This study was a diagnostic test evaluation conducted on data of 1185
traumatic patients transferred through EMS to Poursina Hospital of Rasht
between March 2012 and March 2013.
I :Our outcome measure was necessity of transfer and admission to hospital
in trauma patients according to the severity of trauma. According to a search
on literature, this study used seven variables extracted from a
researcher‑made questionnaire as follows: age, SBP, GCS, PR, ability to
walk, O2S, and mechanism of injury.the questionnaire was filled out by the

EMS staff at the scene or during transfer to the hospital with respect to
clinical observations. Thus, age, SBP, GCS, PR, ability to walk, O2S, and
mechanism of injury were analyzed to calculate the score. All the predictors

O :Using obtained results of the study and similar studies in policy‑making
and management of health system as well as utilizing this tool in
surveillance system can have key role in patient's situation evaluation in
pre‑hospital and hospital phase, appropriate planning for right and timely
providing hospitalization services for patients (secondary level of
prevention), and reduced hospital costs.

admitted to our University Hospital Emergency Department during an 8-

BAB III
Penerapan sistem NTS ini dapat digunakan ketika awal pasien yang
mengalami trauma masuk ke IGD, dengan penghitungan NTS pada triage
yang dilakukan dengan menjumlahkan coded value dari 3 parameter yaitu
Glasgow Coma (GCS), tekanan darah sistolik dan saturasi oksigen
(SPO2). Masing-masing parameter mempunyai nilai dari 0-4 sehingga
NTS triage mempunyai sebaran nilai dari 0-12. Nilai RTS = 11
mengindikasikan pasien perlu dirujuk ke trauma centre. Tabel penilaian
RTS adalah sebagai berikut :

kali pemeriksaan maka dapat dikatakan hasil tersebut tidak valid karena
pasien selalu mengalami perubahan baik perubahan nilai GCS, tekanan
darah sistolik, dan SPO2. Sehingga diharapkan dengan pemeriksaan secara
berkala, petugas kesehatan mampu mengetahui dengan cepat kamajuan
atau bahkan perburukan keadaan yang terjadi pada pasien karena tindakan
tindakan yang tepat akan sangat berpengaruh kepada prognosis pasien.
