ABSTRAK Ria Nurrohmah Utomo Evaluasi Input Program Pengelolaan Penyakit Kronis (Prolanis) BPJS Kesehatan di Puskesmas Pandanaran dan Puskesmas Karanganyar Kota Semarang XIX + 162 Halaman + 20 Tabel + 2 Gambar + 30 Lampiran
Puskesmas Pandanaran memiliki jumlah peserta Prolanis terbanyak pada
tahun 2017 yaitu 409 orang. Jumlah kunjungan peserta Prolanis pada bulan Agustus sampai November 2017 berturut-turut yaitu 50,13%, 56,1%, dan 68,95%. Sedangkan salah satu puskesmas dengan jumlah peserta paling sedikit terdapat di Puskesmas Karanganyar, yaitu 79 peserta pada tahun 2017. Jumlah kunjungan peserta Prolanis pada bulan Agustus sampai Desember 2017 berturut-turut yaitu 7,2%, 7,6%, 8%, 40%. dan 38,47%. Tujuan penelitian ini adalah untuk mengevaluasi input Program Pengelolaan Penyakit Kronis (Prolanis). Jenis penelitian adalah penelitian deskriptif kualitatif, dilengkapi studi evaluasi. Informan penelitian berjumlah 13 orang yang dipilih dengan teknik purposive sampling. Teknik pengumpulan data menggunakan wawancara dan observasi. Data dianalisis secara kualitatif. Hasil penelitian menunjukkan tenaga pelaksana terlatih Prolanis masih kurang, kegiatan sudah sesuai dengan pedoman penggunaan dana, ketersediaan obat mengacu Formularium Nasional (Fornas), ketersediaan peralatan pemeriksaan kesehatan mengacu Kompendium Alat Kesehatan, ketersediaan buku pedoman Prolanis belum sesuai, formulir kesediaan bergabung dalam Prolanis belum dan buku pemantauan status kesehatan terdistribusi menyeluruh. Input Prolanis belum tersedia secara optimal. Direkomendasikan agar perencanaan program dilakukan kembali dengan perbaikan petunjuk teknis yang disesuaikan dengan pedoman.

The number of Prolanis participants visit from August to December 2017 is 7.2%, 7.6%, 8%, 40% . and 38.47%. The study aims to evaluate the input of Program of Chronic Disease Management (Prolanis). The type of research is qualitative descriptive approach with evaluation study. Informant of the study amounted to 13 people, selected by purposive sampling technique. Types of data collected by interviews and observations. Data were analyzed qualitatively. The results showed that the trained personnel of Prolanis are still inadequate, the activities are in accordance with the guidance of the use of funds, the availability of medicines referring to the National Formulary (Fornas), the availability of health inspection apparatus referring to the Compendium of Medical Devices, the availability of Prolanis manuals not yet appropriate, a comprehensive distributed health status monitoring book.. The Prolanis input is not yet available optimally. It is recommended that the program planning be repeated with the improvement of technical guidelines adapted to the guidelines.
