1 Tujuan
Untuk membuat prosedur kerja yang aman dengan
menerapkan system isolasi energy dan pemasangan serta
pelepasan padlock, Danger tag Pribadi, Scissor dan Label
Out of service selama konstruksi, perbaikan, perawatan,
pengujian dan pra-commissioning.
2 Ruang Lingkup
Prosedur ini berlaku bagi seluruh karyawan Lamipak
Indonesia dan karyawan kontraktor yang bekerja di setiap
lingkungan kerja dari seluruh lini Proyek Lamipak Indonesia.
3 Definisi
Isolasi energi Pemutusan atau penghambatan dari semua
sumber penyuplai energi ke peralatan. (Listrik,
mekanik, hidrolik, udara bertekanan, kimia,
termal, termasuk gaya berat atau sumber
energy lainnya yang berpotensi cidera pada
manusia)
Permit Isolasi Formulir standar yang ditandatangani oleh
Superintendent/Deputy (Atau ke atas) dan
pengawas yang bertanggung jawab untuk
memastikan pekerjaan bertegangan tinggi
dilakukan secara aman.
Pemegang Orang yang bertanggung jawab menyelesaikan
Permit pekerjaan bertegangan tinggi di tempat kerja
Pemohon Orang yang yang hendak melakukan pekerjaan
Isolasi pada sebuah peralatan atau keseluruhan
sistem bertegangan tinggi yang harus diisolasi
untuk alasan keselamatan
Perangkat Alat mekanis yang secara fisik mencegah
isolasi energi transmisi atau pelepasan energi. Metode
pencegahan mungkin dengan memutus sumber
energi atau dengan memblokir arus energi
Titik Isolasi Mekanisme pengontrolan yang ketika dimatikan
atau dilepaskan menjadikan area kerja terisolasi
dari sumber energi atau energy sisa.
Penguncian Penempatan alat pengunci pada perangkat
pengiisolasi energi, sesuai dengan prosedur
yang ditetapkan, yang memastikan bahwa
perangkat pengisolasi energi dan peralatan
yang dikontrol tidak dapat dioperasikan sampai
perangkat pengunci dilepas.
Peralatan Alat pengunci mekanis yang menggunakan
penguncian pengunci secara individual untuk
mengamankan perangkat pengisolasi energi
dalam posisi mencegah pemberian energi pada
mesin, peralatan, atau proses.
Penandaan Penempatan alat penanda pada perangkat
pengisolasi energi, sesuai dengan prosedur
yang ditetapkan, untuk menunjukkan perangkat
pengisolasi energi dan peralatan yang dikontrol
tidak dapat dioperasikan sampai perangkat
penanda dilepas
Danger tag Label yang diterapkan oleh orang yang bekerja
Pribadi pada, atau di mesin/ peralatan untuk
(Merah dan menunjukkan bahwa mesin/peralatan tersebut
Hitam pada tidak boleh dioperasikan. Disebut juga Label
penanda bahaya pribadi yang tujuan utamanya adalah

1 Purpose
To describe the safe work procedures with apply the
Energy isolation system and placement also removal of
Personal padlock, Danger Tags, Scissor and Out of
Service tags during construction, repairing, servicing,
testing and pre-commissioning.
2 Scope
The procedure is applicable for all employees of
Lamipak Indonesia and Sub-contractors that they are
working in all business line work area.
3 Definitions
Energy Disconnection or inhibition of all energy
Isolation supplies to equipment. (Electrical,
mechanical, hydraulic, compressed air,
chemical, thermal, include gravity or other
energy adapted to injury to humans)
Isolation Permit A standard form signed by the
Superintendent/His Deputy (Or Above) and
the Supervisor in charge to ensure the high
voltage job is carried out safely.
Permit Holder The person at the job site responsible for
completing the high voltage task.
Isolation The Isolation Requestor is the person who

of equipment or a total system of high
voltage which is to be isolated or de-
energised for safety reasons.
Energy Isolation Mechanical devices that physically prevent
device transmission or release of energy.

source.
Lock Out The placement of a lockout tool on an
energy isolating device, in accordance with
an established procedure, ensuring that the
energy isolating device and the equipment
being controlled cannot be operated until
the lockout device is removed.
Lock out Device A mechanical means of locking that uses an
individually keyed lock to secure an energy
isolating device in a position that prevents
energization of a machine, equipment, or a
process
Tag Out The placement of a tag out tool on an
energy isolating device, in accordance with
an established procedure, to indicate the
energy isolating device and the equipment
being controlled may not be operated until
the tag out device is removed
Personal A tag applied by a person working on, or in,
Danger Tag plant or equipment to indicate that operation
(Red and Black or energising of the plant or equipment is
in Hazard prohibited. Also called a Personal Danger
Marking) Tag as its primary purpose is to protect
people.
Out of Service A tag applied by an employee to indicate

5 Penanggung Jawab
5.1 Manajer Proyek
Memberi kewenangan terhadap Superintendent Departemen
untuk menilai dan menandatangani pengajuan permit isolasi,
dan, mendelegasikan seluruh Kepala Departemen untuk
memastikan pelaksanaan Prosedur Isolasi-LOTO dengan
maksimal.
5.2 Superintendent Departemen
Memastikan prosedur ini diterapkan oleh seluruh pekerja
termasuk sub-kontraktor saat bekerja untuk Lamipak
Indonesia. Juga, memastikan semua karyawan dan sub-
kontraktor mendapatkan pelatihan prosedur/instruksi kerja
isolasi energi sebelum melakukan pekerjaan di area kerja
Lamipak Indonesia. Juga menandatangani permit isolasi jika
ada pengajuan.
Superintendet memberikan surat peringatan pada Karyawan
(termasuk sub-kontraktor) yang melakukan pelanggaran
terhadap prosedur ini dan menyerahkan surat peringatan
yang telah dilengkapi ke Departemen HR.
5 Responsibilities
5.1 Project Manager
To give an authority for Superintendent Department or
Department Head to be able to approved and
assessed isolation permit requisition and delegate all
department head to ensure the good implementation of
Isolation Procedure-LOTO.
5.2 Department Superintendent
Ensure this procedure is implemented for all
employees including sub-contractor when working for
Lamipak Indonesia. And also, ensure all employees
and sub-
contractors adequately trained in isolation
procedure/work instruction before commencing work in
Lamipak Indonesia. Also, to approve isolation permit if
any permit requisition.
Superintendent to give the warning letter to employees
(include sub-contractor) who failure to follow the
requirement in this procedure and submit that

5.3 Foreman/Supervisor
Memastikan prosedur dan sistem isolasi - LOTO ditaati
dengan baik oleh seluruh bawahan dan sub kontraktor setiap
saat. Dan memastikan semua bawahan dan sub kontraktor
memiliki gembok dan label pribadi masing-masing. Juga
memastikan persyaratan Sistem Isolasi sudah dipatuhi
sebelum menandatangani permit isolasi (Jika ada
pengajuan).
Pengawas memberitahu Superintendent setiap terjadi
pelanggaran prosedur oleh pekerja (termasuk kontraktor)
sebagai proses pemberian surat peringatan.
5.4 Karyawan
Mengidentifikasi sumber energi dan melakukan
pengisolasian yang benar sebelum memulai pekerjaan.
Melakukan pengetesan terlebih dahulu pada sistem isolasi
energy untuk memastikan efektif atau tidak. (Dead Test).
Merawat label (bahaya) pribadi dan gembok Pribadi (Yang
dikeluarkan Lamipak Indonesia). Segera laporkan ke
Foreman/Supervisor jika terjadi kerusakan atau kehilangan
gembok dan label pribadi.
5.5 SHE Representative
Memberikan Induksi dan Memastikan peserta induksi
mengerti tentang Prosedur/Instruksi Kerja Penggembokan
dan Pelabelan (Sistem Isoalsi).
Memberikan Label Bahaya Pribadi dan Gembok standard
kepada personil yang telah mendapatkan pelatihan LOTO.
Melaksanakan random inspeksi LOTO (Sistem Isolasi)
minimal sekali dalam sebulan
6 Sistem Isolasi
6.1 Pengenalan
 Semua sumber energy harus diidentifikasi dan dilakukan
isolasi pada semua sumber energi (semua bagian/titik
energi), kemudian pengisolasian diuji untuk memastikan
pekerjaan tersebut dapat dilakukan dengan aman. Titik-titik
ini kemudian harus terkunci, dipasang label, ditutup atau
diamankan dan Permit Isolasi diimplementasikan untuk
energi listrik tegangan tinggi.
 Isolasi sumber energi sebelum melakukan perawatan,
pemeliharaan atau perbaikan merupakan elemen penting
untuk melindungi karyawan dari potensi terlepasnya energi
atau gerakan tak terduga, penyalaan mesin atau peralatan,
atau terlepasnya energi yang tersimpan.
 Perangkat Isolasi Energi, termasuk namun tidak terbatas
pada, hal-hal berikut:
- Pemutus sirkuit listrik yang dioperasikan secara manual;
- Sakelar pemutus;
- Sakelar yang dioperasikan secara manual dimana
konduktor suatu rangkaian dapat diputuskan dari semua
konduktor suplai yang tidak tersalur pembumian dan,
sebagai tambahan, tidak ada kutub (electroda) yang dapat
dioperasikan secara independen;
- Katup saluran;
- Penutup (misalnya Flange, block valve) dan perangkat
serupa yang digunakan untuk memblokir atau mengisolasi
energi
(Tombol tekan, sakelar pemilih, dan perangkat jenis
rangkaian kontrol lainnya tidak dianggap sebagai
perangkat pengisolasi energi)
 Sistem Izin Isolasi merupakan metode formal dan terperinci
untuk memastikan sistem kerja yang aman untuk tugas

5.3 Foreman/Supervisor
Ensure that the correct isolation procedures and
systems are adhered by subordinate and sub-
contractors at all times. And ensure each subordinate
and subcontractor has a pad lock and personal danger
tag. Also, ensure all required by Isolation permit are
comply before signing off in Isolation permit (If any
requisition).
Supervision to report to Superintendent if any breach
the rules by employees (include subcontractor) as part
of punishment process.
5.4 Employees
To identify energy sources and follow correct isolation
prior to commence of work.
Conduct prior testing of isolation systems to ensure
they are effective or not (Dead Test).
Maintain and care for their Personal Danger Tag and
Pad Lock (as issued by Lamipak Indonesia). Reporting
to Foreman/Supervisor directly if broken or missing.
5.5 SHE Representative
Conduct SHE Induction and ensuring that induction
participant understand about LOTO Procedure/Work
Instruction.
Issued Personal Danger Tags and Pad Locks to the
person who getting done for training LOTO.
Conduct random inspections LOTO (Isolation System)
at least once a month.
6 Isolation System
6.1 Introduction
All energy sources shall be identified and isolated at all
energy sources (all parts / energy point), then isolation
is tested to make sure the work can be done safely.
Those point shall be locked, labeled, closed or secured
and Isolation Permit is implemented for high voltage
Isolated all energy sources prior to servicing,
maintenance or repairing is an essential element of
protecting employees from the unexpected
energization or motion, start up of the machine or
equipment, or release of stored energy
 Energy Isolation Device including, but not limited to,
the following:
- A manually operated electrical circuit breaker;
- A disconnect switch;
- Manually operated switches by which the conductors
of a circuit can be disconnected from all ungrounded
supply conductors and, in addition, no pole can be
operated independently;
- A line valve;
- A block (e.g. blind flange, block valve) and any
similar device used to block or isolate energy.
(Push buttons, selector switches and other control
circuit type devices are not considered energy isolating
devices).
The Isolation Permit System constitutes a formal and
detailed method of ensuring safe systems of work for
task with high voltage electric. LMIND-00-HS-FOR-
0018; ISOLATION PERMIT is used where the
personell involved and the tasks are exposed by
high voltage

 dengan listrik tegangan tinggi. LMIND-00-HS-FOR-0018;
IZIN ISOLASI digunakan di mana personel yang terlibat
dan pekerjaan – pekerjaan terpapar oleh bahaya listrik
tegangan tinggi (1000 Volt atau lebih).
 Sebelum mengeluarkan Permit Isolasi,
Superintendent/Deputy (Atau lebih tinggi) dan pengawas
harus memastikan bahwa semua orang yang terlibat sudah
mendapatkan pelatihan LOTO serta memiliki Danger tag
Pribadi dan Pad lock.
 Dalam hal ini harus memasang Danger tag Pribadi, Pad lock,
dan Scissor di setiap titik isolasi. Sistem Isolasi harus dilepas
sebelum permit isolasi ditutup atau dibatalkan.
 Pada pekerjaan mekanik dan auto elektrik (low voltage) yang
tidak memerlukan permit, semua pekerja yang terlibat
tetap harus mengidentifikasi semua sumber energi yang ada
pada mesin/peralatan dan diisolasi untuk menjamin bahwa
energi apapun yang tersimpan tidak terlepas dan tidak
terpapar dengan pekerja. Sehingga pekerjaan dapat
dilakukan dengan aman.
 Perangkat penguncian harus dipasang pada perangkat
isolasi energi selama proses pekerjaan dilakukan, perangkat
penguncian harus dilengkapi dengan penandaan baik
penandaan personal maupun status dari peralatan
 Selain pemasangan gembok, label-label, dan Scissor, juga
harus memasang pengunci/pengaman lain seperti Pin/Safety
line untuk dump body truck, merilis tekanan angin yang
tersimpan, mengganjal ban untuk mencegah pergerakan
mesin/peralatan, menutup bagian-bagian yang panas, dan
lain-lain.
 Secara umum metode pengisolasian Energi berbahaya
dapat dilihat pada lampiran 10.2.
6.2 Pengajuan Permit Isolasi
Setiap pekerja yang akan bekerja pada peralatan
bertegangan tinggi, harus mendapatkan izin dari
Superintendent/Deputy (Atau lebih tinggi) dan melengkapi
Permit Isolasi (LMIND-00-HS-FOR-0018). Permit Isolasi
akan menjelaskan semua bagian yang harus diisolasi dan
menentukan batas-batas yang tepat dari isolasi, termasuk
switch, pemutus sirkuit, katup dll
Superintendent/Deputy (Atau lebih tinggi) akan mencatat
rincian pekerjaan pada Permit Isolasi dan harus
menandatangani Permit Isolasi tersebut. Salinan permit
isolasi diberikan kepada pemegang permit dan salinannya
tersedia di setiap titik isolasi.
Semua keterangan yang diperlukan harus diisi secara rinci
pada label yang dipasang.
 Berapapun jumlah pekerja yang bekerja pada pekerjaan
tegangan tinggi, mereka harus menandatangani Permit
Isolasi dan memasang Danger tag Pribadi, Pad lock dan
Scissor
 Permit Isolasi ini hanya berlaku selama 12 jam. Apabila
pekerjaan masih akan dilanjutkan pada shift berikutnya,
pemegang permit harus mengakhiri dan menyelesaikan
permit Isolasi pada akhir pergantian shift dan Permit Isolasi
diperbaharui oleh crew pada shift berikutnya dan
ditandangani kembali oleh pengawas dan
Superintendent/Deputy

electrical hazard (1000 Volt or Above).
Prior to the Isolation Permits becoming effective, the
Superintendent/His Deputy (Or Above) and Supervisor
Shall be ensuring that persons involve has been
trained and have a personal danger tags and pad
locks.
 This will generally involve the fixing of a Personal
Danger tag, Pad lock, and Scissor to every point of
isolation. Isolations must be de-isolated and every tag,
pad lock, and scissor removed before the permit is
closed out.
 For mechanic job and auto electrician job (low voltage)
which is don’t need the isolation permit, involves
person are still have to identify all energy sources at
the machine/equipment and isolated to ensure that all
stored energy are relieve and not exposure to people in
place. The work can be conducted safely.
 Locking devices shall be installed on Energy isolation
devices during work performed, locking devices must
be equipped with personal tagging personal tagging
and equipment status
 Besides involve person to place their pad locks, tags,
and scissor lock, they are shall install another
locks/securing like a dump pin/safety line for trucks,
relieve stored air pressure, Wheel chocked to prevent
that machine/equipment move, protected hot are, etc.
General Methods of hazardous Energy isolation can be
seen on appendix 10.2
6.2 Requesting Isolation Permit
 Any employee, who is required to work on high voltage
equipment, must obtain permission from an
Superintendent/His Deputy (Or Above) and complete
an Isolation Permit (LMIND-00-HS-FOR-0018). The
Isolation Permit will describe all point to be isolated
and define the exact boundaries of the isolation,
including switches, circuit breakers, valves etc.
 The Superintendent/His Deputy (Or Above) will record
the details of the work on the Isolation Permit and sign
the Isolation Permit. A copy of permit isolation is given
to the permit holder and available at each isolation
point.
 All required information shall be filled in detail on the
label installed.
 Regardless of the number of workers who dedicated
on high-voltage jobs, they shall sign the Isolation
Permit and install the Personal Danger tag, Pad lock
and Scissor
 Isolation Permit is only valid for 12 hours. If the
work is still to be continued in the next shift, the permit
holder shall terminate and complete the Isolation
permit at the end of the shift change and the Isolation
Permit is updated by the crew on the next shift and re-
appointed by the Supervisor and Superintendent /
Deputy

7 Penguncian dan Penandaan
7.1 Label Peralatan Rusak (Label Out of service)
 Apabila ada plant, peralatan atau mesin, yang ditemukan
rusak atau bermasalah, maka harus segera ditarik dari
pengoperasian dan memasang label Out of service, untuk
menunjukkan bahwa plant, mesin atau peralatan tersebut
tidak layak untuk digunakan atau dioperasikan. Service tag
tersebut harus diisi dan dipasang ke titik isolasi utama
peralatan.
 Pengawas atau Superintendent/Deputy (Atau lebih tinggi)
Mekanik atau Elektrik harus diberitahu sesegara mungkin
untuk melakukan perbaikan
 Mesin atau peralatan yang terdapat label Out of service
tidak dapat digunakan, dipindahkan atau dioperasikan
kecuali untuk keperluan perbaikan dan pengetesan.
 Aturan tentang label Out of service
Out of service Tag ditempatkan pada:
- pekerjaan yang belum selesai
- Perlengkapan yang salah, rusak, tidak standard atau
berbahaya
- Peralatan berstatus “Out of service” karena alasan
operasional
 Service tag TIDAK untuk digunakan sebagai pengganti
Personal Danger tag. Peralatan yang telah dipasang label
Out of service tidak akan dijalankan atau dioperasikan.
Label Out of service harus selalu mencamtumkan status
terkini akan mesin atau peralatan.
7.2 Gembok dan Master Scissor
 Gembok / Pad lock yang digunakan harus terpasang
bersamaan dengan Danger tag Pribadi.
 Master scissor dapat juga digunakan jika jumlah pekerja
yang terlibat lebih dari 1 atau titik pemasangan gembok /
padlock tidak mencukupi untuk jumlah pekerja yang terlibat.
 Gembok / Pad lock hanya dapat dilepas oleh orang yang
memasang atau pemiliknya. Aturan pelepasan gembok
sama halnya dengan pelepasan Danger tag Pribadi.
 Apabila ada pekerja yang lupa/tidak melepas Gembok,
maka harus kembali ke tempat kerja untuk melepas
Gembok tersebut. Apabila pemiliknya tidak dapat kembali
ke tempat kerja, maka harus dilakukan penyelidikan dan
Gembok hanya boleh dilepas atas seijin Superintendent
area atau lebih tinggi.
7.3 Pelepasan label-label
 Danger Tag Pribadi
Tidak diijinkan seseorang untuk melepas personal Danger
tag milik orang lain. Personal Danger tag hanya boleh
dilepas oleh orang yang memasang dan pemilik label
tersebut.
Apabila pemilik label tidak ada, label tersebut hanya dapat
dilepas atas persetujuan dari Manajer/Superintendent
departemen terkait setelah penyelidikan menyeluruh
dilakukan.
Kegagalan dalam mematuhi persyaratan ini akan dianggap
sebagai pelanggaran serius.
 Label Peralatan Rusak
Hanya pengawas atau orang yang ditunjuk atau orang yang
memiliki kewenangan selama perbaikan peralatan yang
dapat melepas label Out of service.

7 Lock Out And Tag Out
7.1 Out of Service Tags
 Any plant, equipment or machinery, is found to be
damaged or defective, it shall immediately be
withdrawn from service and an Out of Service tag
placed on it, to indicate it is unsafe to use. The tag
shall be filled in and fixed to the main isolation point of
the equipment. The Supervisor or authorised repairer
shall be notified as soon as practicable.
 Mechanic or Electric Supervisor or
superintendent/Deputy (or Above) must informed
immediately to conduct repair.
 Machinery or equipment displaying an Out of Service
Tag is not to be used, moved or operated except to
maintenance or testing purposes.
 Out of Service Tags Rules
Out of Service Tags are placed on:
- Incomplete jobs
- Faulty, damage, substandard or dangerous
equipment
- Equipment to be kept out of service for
operational reasons
 Out of service NOT to be used as a substitute for
Personal Danger Tags. Equipment that has been
tagged Out of Service shall not be started or operated.
Out of Service Tags shall always show the current
status of the equipment.
7.2 Padlock and Master Scissor
 The used padlock shall be installed with the Personal
Danger tag.
 Master scissors can also be used if the number of
workers involved more than 1 or padlock mounting
points is insufficient for the number of workers
involved.
 Padlock can only be removed by the person who
installed it or the owner. The lock release rule is the
same as Danger's Personal tag release.
 If there is a worker who forgot / did not remove the
Padlock, then the worker must return to the workplace
to remove the Padlock. If the worker is unable to
return to the workplace, investigations must be
perform and Padlock may only be removed upon the
permission of the Superintendent area or higher
7.3 Removal of Tags and Locks
 Personal Danger Tags
No person is permitted to remove personal Danger
tags belonging to others. Personal Danger tags may
only be removed by the person who installed and the
owner of the label
If the label owner is not present, the label may only be
removed upon approval from the Manager /
Superintendent of the relevant department after a
thorough investigation has been conducted.
A Failure to comply with these terms will be
considered a serious violation.
 Out of Service Tags
Only the authorised repairer or supervisor or

 Pelepasan label Out of service hanya dapat dilakukan

dikembalikan untuk dioperasikan secara normal dan aman.
Personil yang melakukan perbaikan harus memastikan

 Pengunci / Gembok dan master Scissor
Pad lock yang digunakan harus terpasang bersamaan
dengan Danger tag Pribadi dan Scissor lock.
Gembok hanya dapat dilepas oleh orang yang memasang
atau pemiliknya. Aturan pelepasan gembok sama halnya
dengan pelepasan Danger tag Pribadi
7.4 Tanda-Tanda dan Penghalang (Barikade)
 Pengisolasian yang efektif memerlukan tindakan lebih lanjut
dalam bentuk tanda-tanda (rambu) dan
penghalang/barikade. Orang-orang yang terlibat dapat
menempatkan tanda-tanda/rambu yang sesuai pada
peralatan.
 Pengawas atau orang yang terlibat dapat memasang
penghalang/barikade sebagai peringatan yang terlihat untuk
menandakan bahwa area atau sistem atau mesin dan
peralatan tersebut sedang diisolasi atau dilakukan
pengetesan.
7.5 Pelaksanaan Pekerjaan
 Sebelum memulai pekerjaan, karyawan harus memasang
Label Out of service pada peralatan, atau mesin
 Pekerja yang terlibat dapat memasang Pad lock masing-
masing, Danger tag Pribadi, dan Scissor lock setelah label
Out of service telah terpasang.
 Semua keterangan yang diperlukan harus diisi secara rinci
pada label yang dipasang.
 Energi apapun yang tersimpan harus diisolasi dan dilepas.
Pengisolasian tidak hanya terbatas pada penggembokan
dan pelabelan.
 Jika lebih dari satu pekerja melakukan pekerjaan pada
suatu peralatan pada saat yang sama, maka setiap pekerja
yang terlibat harus menerapkan kunci pribadinya ke
perangkat lockout. Jika lebih dari satu orang menerapkan
kunci ke perangkat lockout, pengait atau kotak kunci atau
Master Scissor Lock dapat digunakan.
 Setelah pengisolasian dilakukan, sebelum memulai
pekerjaan, pekerja harus melakukan pengetesan pada
sistem isolasi yang ada (tes mati) untuk memastikan bahwa
mesin atau peralatan sudah terisolasi dengan baik. Dan
memastikan semua energi yang tersimpan sudah terlepas
supaya pekerjaan dapat dilakukan dengan aman.
Pengetesan dapat dilakukan melalui inspeksi atau
mengamati alat ukur atau indikator, mencoba secara
manual, menguji sirkuit, memantau gerakan atau aliran, dan
pengukuran.
7.6 Penerapan Pada Kondisi Khusus
 Dimungkinkan ada situasi ketika perbaikan, pembersihan,
pelumasan atau penyetelan peralatan harus diselesaikan
saat beroperasi, dalam gerakan, atau teraliri suatu energi.

delegated person of the plant or equipment shall
remove an Out of Service tag.
Removal of the tag shall occur only after the
appropriate repair has been made and the repairer is
satisfied the equipment can safely be returned to
normal use.
Repairers shall ensure that the tag has been removing
prior to relocating or return to operate.
 Locks and Master Scissor
The pad lock that used must be installed along with
Personal Danger tag and Scissor lock.
The padlock can only be removed by the person who
installed it or the owner. The lock release rule is the
same as Personal Danger tag release
7.4 Signs and Barriers
 Effective isolation may require further precautions in
the form of signs and barriers. Person Involves may
place appropriate signage on equipment.
 The Supervisor or the person Involves may install
barriers or signage as a visual warning to delineate
areas or systems or machine/equipment that are being
isolated or tested
7.5 Performing the Work
 Prior starting the job, the employee shall install Label
Out of service on equipment, or machine
 The worker who involved can install their own Pad
lock, Personal Danger tag, and Scissor lock after the
Out of service label has been installed.
 All required information shall be filled in detail on the
label installed.
 Any stored energy shall be isolated and released.
Isolation not only limited to locking and labelling.
 If more than one person is working on a piece of
equipment at the same time, each person must apply
his/her personal lock to the lockout device. If more
than one person is applying a lock to a lockout device,
a hasp or lock box may be used. Specific lockout
procedures should include the proper use of lockout
devices such as lock boxes
 After isolation is done, prior starting the work, the
worker shall perform testing on the installed isolation
system (dead test) to ensure that the machine or
equipment is well isolated. And ensure that all stored
energy is released so that work can be done safely.
Testing can be done through inspection or observing a
measuring instrument or indicator, manual testing,
circuit testing, motion or flow monitoring, and
measurement.
7.6 Application Under Specific Condition
 There may be situations where repairing, cleaning,
lubrication or equipment adjustment must be

 Rencana pengembangan pengendalian energi yang
berbahaya harus mencakup unsur-unsur berikut
a. Mendokumentasikan penilaian risiko;
b. Implementasi langkah-langkah pengurangan risiko
sebagaimana ditentukan dari penilaian risiko yang
mungkin termasuk;
- Merancang perubahan, substitusi bahan yang
kurang berbahaya atau penerapan prinsip
ergonomi;
- Penerapan safeguarding dan pelengkap tindakan
perlindungan;
- Praktik kerja aman yang didokumentasikan; dan /
atau Pelatihan tambahan..
 Jika prosedur penguncian tidak dapat digunakan dan
pengurangan risiko yang memadai tidak dapat dicapai
dengan menggunakan metode kontrol lainnya, aktivitas
atau pekerjaan tidak boleh dilakukan.
7.7 Penyelesaian Pekerjaan
 Apabila pekerjaan selesai pada shift itu juga, semua
personil yang terlibat melepas semua gembok, Danger tag
Pribadi, dan Scissor lock. Kemudian melaporkan kepada
pengawas bahwa pekerjaan tersebut telah selesai
 Untuk Pekerjaan dengan permit isolasi maka pekerja
melapor kepada pengawas dan superintendent/ deputy
yang mengeluarkan permit untuk melakukan
pengetesan/pemeriksaan mesin/peralatan yang telah
dikerjakan. Penandatanganan Permit Isolasi (PTP-SHE-FO-
G-0019 ) menunjukkan bahwa permit isolasi telah selesai
masa berlakunya dan dibatalkan.
 Ketika pekerjaan telah selesai dilakukan, Kru mekanik atau
elektrik atau yang memiliki otoritas harus melakukan
pengetesan bahwa mesin atau peralatan sudah terlepas
dari isolasi dengan sempurna.
 Pengawas dan atau orang yang ditunjuk melepas label Out
of service yang terpasang serta mengembalikan
mesin/peralatan untuk dioperasikan sebagaiamana
mestinya.
 Apabila pekerjaan harus dilanjutkan pada shift berikutnya,
semua personil yang terlibat melepas semua gembok,
Danger tag Pribadi, dan Scissor lock. Kemudian
memastikan label Out of service masih terpasang dan
informasi yang tertulis masih relevan dan atau diperbaharui.
 Personil yang melakukan pekerjaan pada shift berikutnya
harus membaca dan memahami informasi yang tertulis
pada label Out of service yang masih terpasang. Kemudian
memasang gembok, Danger tag Pribadi, dan Scissor lock
serta dilakukan pengujian pada sistem isolasi untuk
memastikan pekerjaan dapat dilanjutkan dengan aman.
7.8 Pemutusan/Pengalihan listrik tegangan tinggi
Hanya orang yang memiliki kewenangan (misalnya Petugas
listrik bertengan tinggi yang berkompeten) yang diizinkan
untuk melaksanakan pekerjaan atau pengalihan listrik
bertegangan tinggi.

completed when on operating, in motion, or in an
energy level. And or when work should be done on
equipment where physical locking is not possible

 In this situation, risk assessments and hazardous
energy control plans shall be developed to ensure
work activities are conducted in a safe manner
 The development plans for hazardous energy control
shall include the following elements :
a. Documenting risk assessments;
b. Implementation of risk reduction measures as
determined from possible risk assessments including;
- Designing changes, substitution of less harmful
materials or application of ergonomic principles;
- Implementation of safeguarding and
complementary safeguards;
- Documented safe practices; and / or additional
Training ..
 If locking procedures are unusable and adequate risk
reduction can not be achieved by using other control
methods, activity or work must not be performed
7.7 Completion of Works
 If work is completed within the shift, the Permit Holder
removes the Personal Danger Tag, and takes off the
padlock and scissors and notifies the Supervisor that
the work already completed.
 The work with isolation permit, the dedicated worker
shall reporting to supervisor and superintendent /
deputy (or above) that signed the permit to conduct
testing/inspection to machine/equipment that has been
done. Signing the isolation permit (PTP-SHE-FO-G-
0019) indicate that the isolation permit has been
completed its validity and cancelled
 Once the work has been completed, an authorized
mechanical or electrical crew shall test that the
machine or equipment is completely detached from
the insolation.
 The supervisor and /or designated person release the
label of the installed Out of service and return the
machine / equipment to be operated as appropriate.
 If the work to go over more than one shift, all involved
worker removed padlock, personnel danger tag, and
scissor lock. Then ensure the Out of Service tag still
attached and its information still relevant and or up-
dated
 Personals who will performing work on the next shift
shall read and understand the information written on
the Out of service label that is still installed. Then
install a padlock, Personal Danger tag, and Scissor
lock as well as testing on the insolation system to
ensure work can proceed safely
7.8 High Voltage Switching
Only authorised personnel (e.g. the high voltage
electrician) shall be permitted to carry out high
voltage switching.

8 Tinjauan Ulang Prosedur
8.1 Tinjau Ulang Manajemen
Tinjau ulang kesesuaian prosedur ini, dan pelaksanaannya,
harus dilakukan apabila ada kecelakaan serius yang terkait,
kekurangan yang diamati dari hasil audit, inspeksi, dan
umpan balik karyawan, atau ada perubahan perundang-
undangan/regulasi, dan atau ada jadwal tinjau ulang
berkala terhadap dokumentasi.
Persetujuan perubahan adalah tanggung jawab Manajer-
Manajer terkait.
8.2 Pelatihan
Pelatihan awal akan diberikan kepada seluruh karyawan
pada saat induksi site. Ini mencakup penggunaan dan
pemasangan Danger tag Pribadi, label Out of service,
Gembok dan Scissor.
Pelatihan khusus harus diberikan kepada personil yang
bekerja pada mesin atau peralatan yang mensyaratkan
untuk diisolasi dan catatan harus dipelihara. Selain itu,
pekerja yang pekerjaannya tidak terlibat langsung dalam
prosedur ini akan disampaikan pada tool box
meeting/safety meeting/sosialisasi umum
8.3 Pelanggaran Prosedur
Karyawan yang tidak melepas LOTO pada akhir shift kerja,
dan atau melanggar atau gagal mengikuti persyaratan di
prosedur ini dikategorikan sebagai Pelanggaran Berat yang
dapat diberikan tindakan disiplin level III atau PHK ; sesuai
dengan Matriks Pelanggaran Golden Rules dan Peraturan

8 Review Of Procedure
8.1 Management Review
Review of the suitability of this procedure, and it’s
implementation, shall be undertaken as a result of
serious incidents, observed deficiencies, audits,
inspections, employee feedback, changing legislation
and/or regular documentation review. Approval of
changes is the responsibility of the related Managers
8.2 Training
Initial training will be provided to all employees during
the site induction. This covers the use and the
recognition of Personal Danger Tags, Out of Service
Tags, Pad Locks and Scissor lock.
Specific LOTO training shall be provided to personnel
who have working in plant, machine and equipment
that required to isolated and records shall be
maintained. In addition, personnel that their jobs are
not directly involved in this procedure will be advised
at Toolbox meetings/safety meeting/general
socialization
8.3 Procedure Violation
Employees who do not release LOTO at the end of the
work shift, and / or violate or fail to comply with the
requirements of this procedure are categorized as
Serious Violations which may be subject to disciplinary
action level III or determination; in accordance with the
Golden Rules Violation Matrix and Company
Regulations
