1. TUJUAN
Untuk memberikan standar dan pedoman yang aman dalam
melakukan isolasi plant/ equipment bergerak yang tidak dapat
digunakan atau sedang diperbaiki, bagi seluruh Mitra Bisnis
dan PT RMP
Untuk memenuhi spesifikasi Pabrik Pembuat Peralatan
Asli/Original Equipment Manufacturer (OEM).
Untuk mematuhi standar Isolasi perusahaan.
2. Definisi
Barikade Peralatan yang digunakan untuk
membatasi akses ke plant/area kerja
yang sedang diperbaiki.
1. PURPOSE
To provide safe guidelines and standards for isolation of
mobile plant /equipment that is unserviceable or under
repair, for all PT RMP and Business Partners.
To comply with Original Equipment Manufacturer (OEM)
specifications.
To comply with Company standards for Isolation.
2. DEFINITIONS
Barricade Positive means of restricting access to
plant/ work areas under maintenance.

4.2.3 Memeriksa perlengkapan isolasi dalam kondisi
baik dan terawat.

4.2.1 Conduct hazard analysis and risk assessments to
identify general and / or specific isolation
requirements.
4.2.2 Stop equipment for maintenance and repair to
reduce the risk of hazards with:

Note: Refer to equipment specific STPs for Rope Cranes for
isolation requirements.
4.4 Conduct safe testing:
Trades/ Non Trades:
4.4.1 Barricade the area:
- For stationary equipment.
- Install signage for mobile testing.
4.4.2 Get each LOTO device removed by its owner.
4.4.3 Check all unauthorized personnel are clear of
the area.
4.4.4 Attach Testing tags and check all unauthorized
personnel are clear and remove all hand tools
and equipment from the work area.
4.4.5 Conduct the tests/adjustments.
4.4.6 Remove test tags upon completion of
testing/commissioning (by tag owners).
4.4.7 Re-install LOTO as per (4.3)
4.4.8 Reinstall machine guards, and remove all hand
tools and equipment from the work area
4.4.9 Remove LOTO (check 4.4.2) when work has
been completed.

4.5.3 Memastikan apakah peralatan aman dan personel
telah menghentikan pekerjaan, sebelum melepas
gembok.
4.5.4 Menyerahkan Formulir Insiden Kecelakaan kepada
HSE.
4.6 Mengelola fasilitas penyimpanan peralatan LOTO:
Supervisor:
4.6.1 Mengelola area penyimpanan perlengkapan LOTO.
4.4.10 Report to the Operator and Dispatch and leave
the area.

Apply (point 4.3) where maintenance and or repairs need to be
performed during testing/commissioning

4.4.11 Conduct Pre-start checks
4.4.12 Resume work safely
4.5 Remove locks where authorized personnel cannot
be located.

4.5.1 Make every possible attempt to locate the owner
of the lock (face-to-face, by phone, by email,
though work colleagues).

Incident Issue Reporting and Investigation.
4.5.3 Check equipment is safe and personal have been
removed from the job, before removing the lock.
4.5.4 Submit the Accident Incident Form to HSLP.
4.6 Manage LOTO equipment storage facilities:
