1. Tujuan PIC
Objective
Prosedur ijin kerja Log out Tag out dibuat sebagai panduan di pabrik PT
Sinar alam permai agar :
Work permit procedures in hazardous areas is created as a guide in the
factory of PT Sinar Alam Permai in order to :
1.1 Seluruh karyawan/kontraktor pekerjaan dapat lebih terjaga
keamanannya dari bahaya kecelakaan kerja yang menimpanya
karena kelalaian dan kecerobohan kerja.
1.1 All employees and contractors can be gated from the dangers of
workplace accidents that happened because of the negligence and
carelessness of work.
1.2 Memastikan bahwa isolasi energi dapat dilakukan dengan benar dan
sesuai dengan standard.
1.2 Ensure that energy isolation can be carried out correctly and in
accordance with standards.
2. Definisi
Definition
2.1. Isolasi adalah penghalang fisik untuk mencegah transmisi atau
pelepasan energi seperti, sirkuit pemutus listrik manual, saklar
pemutus, atau pun dalam bentuk lainnya yang berfungsi untuk
mencegah Energi menuju ke sesuatu lokasi.
2.1. Isolation is a physical barrier to prevent the transmission or
release of energy such as, a manual electrical circuit breaker, a
breaker switch, or any other form that serves to prevent energy
from going to a location.
2.2. Lock Out adalah Penempatan perangkat pengunci pada alat
pengisolasi energi, sesuai dengan prosedur yang ditetapkan,
memastikan bahwa perangkat dan peralatan isolasi energi yang

dikontrol. Perangkat dan Peralatan tidak dapat dioperasikan

sampai Lock Out / Pengunci dilepas.
2.2. Lock Out is the placement of a locking device on an energy
insulating device, in accordance with established procedures,
ensuring that the device and energy isolation equipment are
controlled. The device and equipment cannot be operated until the
Lock Out / Lock is removed.
2.3. Tag Out adalah Penempatan penanda pada peralatan menunjukkan
bahwa peralatan yang dikontrol tidak dapat dioperasikan sampai
penanda tersebut dilepas. Tag berisi tanggal isolasi, petugas yang
memasang Isolasi, tag hanya dapat dibuka oleh petugas yang
memasang.
2.3. Tag Out is a marker placement on the equipment indicating that
the controlled equipment cannot be operated until the marker is
removed. The tag contains the date of isolation, the officer who
installed Isolation, the tag can only be opened by the officer who
installed it.
3. Ruang Lingkup
Scope
Prosedur izin kerja isolasi / LOTO ini berlaku di PT Sinar Alam Permai
Isolation / LOTO work permit procedures is applicable in PT Sinar Alam
Permai.
4. Kebijakan
Policy
4.1. Energi yang harus teridentifikasi pada saat pelaksanaan LOTO
seperti Energi Mekanik, Energi Putaran, kimia, listrik, oli, biologi,
radiasi, korosif, uap, gas, hidrolik, tekanan angin, udara, katup,
gravitasi, air atau energi lainnya yang dapat mengakibatkan
kecelakaan/kerugian apabila tidak dikendalikan pada saat

pekerjaan.
4.1. Energy that must be identified during LOTO implementation such
as Mechanical, Rotational, Chemical, Electrical, Oil, Biological,
Radiation, Corrosive, Steam, Gas, Hydraulic, Pneumatic, Air,
Valve, Gravity, Water, or other energy that can resulting in
accidents / losses if not controlled at work.
4.2. LOTO dapat dilakukan bersamaan dengan ijin kerja lainnya.
4.2. LOTO can be done in conjunction with other work permits.
4.3. Sebelum melakukan LOTO, seluruh energi yang akan di Isolasi
harus teridentifikasi. Tidak boleh melakukan pekerjaan apabila
terdapat Energi yang belum terisolasi.
4.3. Before conducting LOTO, all energy that will be isolated must be
identified. Do not work if there is Energy that has not been
isolated.
4.4. Tag Out harus berisikan informasi Operator / Karyawan yang
memasang LOTO tersebut.
4.4. Tag Out must contain information on the Operator / Employee
who installed the LOTO.
4.5. Kunci LOTO hanya diperbolehkan dibawa oleh Operator /
Karyawan yang memasang LOTO tersebut.
4.5. The LOTO key is only allowed to be carried by the Operator /
Employee who installed the LOTO.
4.6. Pengesahan ijin kerja harus dilakukan di Area Kerja, tidak
diperbolehkan dilakukan di dalam Kantor.
4.6. Approval of work permits must be made in the Work Area, not
permitted within the Office.
4.7. Pemasangan LOTO harus dipastikan terpasang sempurna dan
energi benar – benar terisolasi.
4.7. LOTO installations must be ensured to be perfectly installed and

energy completely isolated.

4.8. LOTO hanya menggunakan peralatan LOTO yang telah
disediakan.
4.8. LOTO only uses the LOTO equipment provided.
4.9. Ijin kerja hanya berlaku selama 8 Jam dan 4 Jam perpanjangan,
setelah itu Ijin Kerja wajib untuk ditutup dan Lembar putih
dikembalikan kepada EHS.
4.9. The work permit is only valid for 8 hours and 4 hours of extension,
after which the work permit must be closed and the white sheet
returned to EHS.
5. Tahapan Kegiatan
Stages of Activity
5.1. Identifikasi jenis pekerjaan dan potensi bahaya, persiapkan Ijin
Kerja lainnya apabila memerlukan, Ijin kerja LOTO dapat
digabungkan dengan Ijin Kerja lainnya.
5.1. Identification of type of work and potential danger, prepare
another work permit if needed, LOTO work permit can be
combined with other work permits.
5.2. Identifikasi seluruh energi yang membahayakan pekerjaan. Pekerja

5.3. Apabila pekerjaan dilakukan oleh lebih dari satu Departemen
dalam satu lokasi, maka setiap departemen memasang LOTO.
Tidak boleh beberapa Departemen hanya menggunakan 1 Gembok
LOTO.
5.3. If the work is carried out by more than one Department in one
location, each department installs LOTO. It may not be possible
for some Departments to only use 1 LOTO Padlock.
5.4. Pastikan saklar, valve, tuas atau sumber energi lainnya dalam
posisi tertutup sebelum pemasangan LOTO.

5.4. Make sure the switch, valve, lever or other energy source is in a
closed position before installing LOTO.
5.5. Penyusunan JSEA dibutuhkan untuk mengetahui langkah –
langkah pekerjaan yang akan dilakukan dan seluruh energi yang
akan diisolasi telah teridentifikasi.
5.5. Preparation of JSEA is needed to know the steps - the work to be
carried out and all the energy to be isolated has been identified.
5.6. Setelah pemasangan LOTO, pastikan seluruh energi telah
terisolasi dan pastikan tidak ada energi tersisa.
5.6. After LOTO installation, make sure all energy is isolated and
make sure there is no energy left.
5.7. Kunci Gembok LOTO harus dibawa oleh Operator/Karyawan
yang memasang LOTO.
5.7. LOTO lock must be carried by the operator / employee who
installs LOTO.
5.8. Ijin kerja LOTO harus tersedia sebelum memasang LOTO dan
mulai pekerjaan.
5.8. LOTO work permits must be available before installing LOTO and
starting work.
5.9. Apabila dalam identifikasi langkah pekerjaan dibutuhkan Izin
Kerja lainnya, maka izin kerja tersebut harus dimiliki pekerja
sebelum melakukan pekerjaan sesuai dengan Standar Operasional
Prosedur.
5.9. If the identification of work steps requires another Work Permit,
then the work permit must be owned by the worker before carrying
out work in accordance with Standard Operating Procedures.
5.10. User dan EHS harus memeriksa peralatan dan kondisi area
kerja sebelum memberikan Ijin Kerja. Tidak diperbolehkan
melakukan pengesahan ijin kerja di dalam kantor.

5.10. User and EHS must check the equipment and conditions of the
work area before giving a Work Permit. Not permitted to
authorize work permits in the office.
5.11. Uji alat atau sumber energi yang telah diisolasi atau LOTO
untuk memastikan Isolasi telah berhasil dengan sempurna.
5.11. Test the device or energy source that has been isolated or
LOTO to ensure that the Isolation has succeeded perfectly.
5.12. Hilangkan seluruh energi yang masih tersisa.
5.12. Eliminate all remaining energy.
5.13. Jangan melepas LOTO apabila pekerjaan masih dikerjakan.
5.13. Don't release LOTO if the work is still being done.
5.14. Apabila pekerjaan telah selesai, pastikan seluruh kondisi alat
aman dan tidak ada pekerja lain yang sedang melakukan pekerjaan
pada Alat atau Mesin tersebut.
5.14. When the work has been completed, make sure all conditions
of the equipment are safe and no other workers are doing work on
the Tool or Machine.
5.15. Lepas LOTO, dan laporkan kepada pengawas.
5.15. Remove LOTO, and report to supervisor.
