1. Fan ventilasi beroperasi untuk minimum 20 menit / Ventilation fans in operation for at least 20
mins.
2. Pengaturan dibuat untuk kelanjutan ventilasi / Arrangements made for continuous ventilation.
3. Penerangan yang cukup di ruang pompa / Adequate pump room lighting.
4. O.O.W diinformasikan rencana ini / O.O.W. informed of planned entry.
5. Sitem komunikasi telah disetujui / System of communication agreed.
6. Personal gas monitor diperkisa dan dapat bekerja dengan baik / Personal gas monitors checked and
operational.

shall inform the person(s) entering pump room of the following precautions
1. Segera evakuasi jika terjadi kegagalan ventilasi / Evacuate immediately in the event of a
ventilation failure
2. Simpan prosedur komunikasi dan lapor setiap saat / Keep communication procedure and report at
intervals not exceeding 10 mins.
3. Simpan personal gas monitor / Keep personal gas monitor
4. Segera lapor langsung jika keadaan darurat terjadi / Report directly in the event of an emergency

* Periksa H2S untuk Tanker minyak dan catat dalam kolom Toxic Gas (ppm) / Should Check H2S for
Oil Tanker and Record in Toxic Gas(ppm) Column.
