• Ganglia Basalis terdiri Striatum, Globus Palidus dan Nukleus subthalamikus. • Striatum merupakan target dari input korteks menuju ganglia basalis. Striatum dibentuk oleh nukleus kaudatus dan putamen. • Globus palidus merupakan sumber output terhadap thalamus dan dibagi menjadi segmen interna dan segmen eksterna. • Ganglia basalis menerima input dari korteks serebri di striatum input diteruskan ke globus palidus menuju substansia nigra diteruskan kembali ke korteks serebri melalui thalamus. • Fungsi ganglia basalis mempertahankan tonus otot yang diperlukan untuk menstabilkan posisi sendi. • Adanya kerusakan di pada struktur ganglia basalis menyebabkan gerakan yang tidak terkontrol seperti tremor. Autoregulasi Dopamin • Dopamin adalah katekolamin yang disintesis oleh tirosin di terminal neuron dopaminergik. • Dopamin melewati sawar darah otak melalui transport aktif. Proses perubahan L-tyrosin menjadi L-dihydroxyphenylalanine (L-dopa) dikatalis oleh enzim tyrosine hidroxylase yang ada dalam neuron katekolaminergik. • L-dopa diubah secara cepat menjadi dopamin oleh aromatic L-amino acid decarboxylase. • Didalam ujung saraf dopamin dibawa ke vesikel oleh protein pembawa dan dilepaskan dari ujung saraf melalui eksositosis, suatu proses yang dirangsang oleh depolarosasi akibat masuknya Ca ke dalam sel. • Kerja dopamin di celah sinaps dapat diakhiri dengan 2 cara, : 1. Dopamin dapat diambil kembali oleh protein karier membran 2. Dopamin dapat didegradasi oleh kerja DOPAC oleh enzim monoamine oxidase type B ( MAO-B ) • Kerja dopamin di otak diperantarai oleh reseptor protein dopamin, ada 5 reseptor dopamine yang berbeda, ke 5 reseptor dapat dibagi menjadi 2 kelompok : 1. Kelas reseptor D1 ( D1 dan D5 ) yang menstimulasi sintesis intraseluler cAMP 2. Kelas reseptor D2 ( D2,D3,D4) yang menghambat sintesis cAMP, menghambat arus Ca2+ da meningkatkan arus K+ Patofisiologi Parkinsonisme • Msasalah utama pada penyakit parkinson adalah hilangnya neuron di substansia nigra pars kompakta yang memberikan inervasi dopaminergik ke striatum ( putamen dan nukleus kaudatus ). • Penyakit parkinson terjadi karena penurunan kadar dopamin akibat kematian neuron di substansia nigra pars kompakta. • Substansia nigra pars kompakta (SNC) dihubungkan dengan striatum oleh dopamine sebagai neurotransmiter. Didalam striatum terdapat dua kelompok reseptor yaitu reseptor D1 yang akan mengaktifkan jalur langsung dan reseptor D2 yang akan mengakttifkan jalur tidak langsung • Jalur langsung dibentuk oleh neuron di striatum yang memproyeksikan langsung ke subtansia nigra pars retukulata (SNR) dan globus palidus interna (Gpi). Dari sini akan dilanjutkan ke ventroanterior dan ventrolateral thalamus, yang memberikan input rangsangan positif terhadap korteks. • Neurotransmiter yang digunakan di jalur langsung adalah GABA yang bersifat eksitatorik sehingga efek akhir dari stimulasi jalur langsung adalah peningkatan arus rangsangan dari thalamus ke korteks • Jalur tidak langsung terdiri dari neural striatal yang memproyeksikan ke globus palidus eksterna (Gpe) struktur ini langsung menginervasi nucleus subthalamikus (STN) yang akan dilanjutkan ke SNR dan Gpi.
• Proyeksi dari striatum ke Gpe, dari
Gpe ke nucleus subthalamikus menggunakan transmiter GABA yang bersifat eksitatorik, tetapi jalur akhir proyeksi dari NST ke SNR dan Gpi merupakan jalur rangsang negatif glutamatergik. Dengan demikian efek akhir dari jalur tidak langsung adalah berkurangnya arus rangsangan dari thalamus ke korteks. • Pada keadaaan dopamine menurun seperti pada penyakit parkinson terjadi efek sebaliknya yaitu inhibisi arus keluar dari SNR dan Gpi ke thalamus dan berkurangnya rangsang terhadap korteks motorik m Pathology and Pathogenesis • The most constant and pertinent finding in both idiopathic and postencephalitic Parkinson disease is a loss of pigmented cells in the substantia nigra and other pigmented nuclei (locus ceruleus, dorsal motor nucleus of the vagus). • The substantia nigra is visibly pale to the naked eye; microscopically, the pigmented nuclei show a marked depletion of cells and replacement gliosis, and some of the remaining cells have reduced quantities of melanin, findings that enable one to state with confidence that the patient must have suffered from Parkinson disease • Many of the remaining cells of the pigmented nuclei contain eosinophilic cytoplasmic inclusions, surrounded by a faint halo, called Lelvy bodies. • These are seen in practically all cases of idiopathic Parkinson disease. The mechanism by which MPTP produces the clini cal aspects of the Parkinson syndrome is unsettled.
