Salah satu tujuan pengaturan penyelenggaraan rumah sakit adalah memberikan perlindungan
terhadap keselamatan pasien. KNC dan KTD yang sering terjadi disebabkan oleh ketidaksesuaian
identifikasi pasien, kesalahan dalam pemberian obat, sampel darah pasien tertukar, dan pasien jatuh.
Sasaran Keselamatan Pasien merupakan salah satu major bab dalam SNARS Edisi 1. Tujuan penelitian
yaitu melihat gambaran penerapan sasaran keselamatan pasien di RSUD Kota Makassar dengan
menggunakan SNARS Edisi 1.
Jenis penelitian ini adalah kualitatif dengan pendekatan studi kasus. Instrumen penelitian dan
penentuan informan menggunakan instrumen survey akreditasi SNARS Edisi 1 dan diperoleh 20
informan. Pengumpulan data dilakukan dengan telusur dokumen, observasi, dan wawancara mendalam.
Keabsahan data dilakukan triangulasi sumber dan metode.
Hasil penelitian diperoleh identifikasi pasien telah dilakukan sesuai regulasi menggunakan enam
identitas sebelum dilakukan tindakan. Peningkatan komunikasi efektif dilakukan dengan membuat SPO
dan mengadakan pelatihan, pesan secara verbal dan verbal lewat telpon dilakukan dengan teknik SBAR
tetapi tidak ada sosialisasi penanggung jawab pelaporan nilai kritis, bukti catatan dan evaluasi tentang
hal-hal kritikal belum dipenuhi seluruhnya. Meningkatkan keamanan terhadap obat yang perlu
diwaspadai dan elektrolit konsentrat dilakukan dengan menerapkan SPO, menyediakan daftar serta
diatur di tempat aman. Tepat-lokasi, prosedur dan pasien yang menjalani tindakan dilakukan sesuai
surgical safety checklist WHO, penandaan di lokasi operasi melibatkan pasien, persetujuan atas operasi
dan konfirmasi verifikasi sudah lengkap dilakukan. Pengurangan risiko infeksi terkait pelayanan
kesehatan dilakukan dengan hand hygiene tetapi hanya dua dari lima langkah yang dilakukan.
Pengurangan risiko cedera akibat pasien jatuh dilakukan proses asesmen sesuai SPO dan diadakan
langkah untuk mengurangi risiko pasien jatuh.

One of the goals of organizing hospitals is to protect patient safety. Near miss and adverse
events that often occur were caused by the mismatch of patient identification, errors in the drug
administration, the patient's blood sample swapped, and the patient falls. Patient Safety Goals was one
of the major chapters in SNARS Edition 1. This study looked at the patient safety goals implementation
at RSUD Kota Makassar by using patient safety goals on the accreditation version of SNARS First Edition.
This study was qualitative with a phenomenology approach. The instrument and
determination of informants used the accreditation survey of SNARS First Edition and obtained 20
informants. Data collected by reviewing the documents, observations, and in-depth interviews. The data
validity used triangulation of sources and methods.
The results of this study obtained patient identification according to regulations and used
six identities before action was taken. To improve effective communication is carried out by making SOP
and conducting training for effective communication, verbal and verbal messages via telephone
conducted with SBAR techniques but there was no socialization about who has the responsibility for
reporting critical values, proof of evidence and evaluation of critical matters has not been fully fulfilled.
To improve the safety of the high alert medication and electrolyte concentrates is carried out by
applying SOP, providing lists and arranged in a safe place. To ensure correct-site, correct-procedures,
and correct-patient surgery carried out according to the WHO surgical safety checklist, marking on-site
operations involves patients, informed consent and confirmation of verification were completed. To
reduce the risk of Health Care-Associated Infection carried out by hand hygiene but only two of the five
moments were taken. To reduce the risk of patient harm resulting from falls is carried out by the SOP,
assessment process and steps were taken to reduce the risk of patients falling.
