A.3 Kontraktor hendaklah memastikan keseluruhan tapak kerja berada dalam keadaan teratur,
kemas dan bersih pada setiap masa. Segala kekotoran dan sisa pembinaan hendaklah
dibersihkan serta merta, dan dikeluarkan dari kawasan hospital ke tempat yang dibenarkan.
Segala sisa-sisa pembuangan atau sampah sarap yang tidak dibuang akan menyebabkan
pihak kontraktor terpaksa menanggung segala kos yang dikenakan oleh hospital, dan ia mesti
dibayar oleh pihak kontraktor sebanyak 2 kali ganda daripada kos yang dikenakan.
A.4 Kontraktor hendaklah meneliti segala skop kerja dan lukisan-lukisan yang diberi dan
menentukan bahawa harga yang disebutkan akan merangkumi semua kerja-kerja yang
difikirkan perlu untuk menyempurnakan skop kerja yang dinyatakan. Maka harga yang
ditawarkan dianggap dapat menampung semua tanggungjawab untuk menyempurnakan skop
kerja yang ditetap dalam Sebutharga ini.
A.5 Kontraktor hendaklah menentukan jumlah harga yang disebutkan dalam Borang Q adalah
bersama dengan harga yang dicatitkan dalam Jadual Ringkasan Harga berikut. Jika terdapat
perbezaan harga, maka jumlah harga dalam Borang Q akan jadi muktamad, dan pecahan
harga dalam Jadual Ringkasan Harga akan diselaraskan.
A.6 Lawatan tapak pada hari yang ditetapkan adalah wajib. Sebutharga dari Kontraktor-kontraktor
yang gagal menghadiri lawatan tapak tidak akan ditimbangkan. Kontraktor yang menghadiri
lawatan tapak akan diberi penerangan mengenai skop kerja. Kontraktor hendaklah disifat
telah memeriksa dan meneliti keadaan tapakbina, jenis , takat kerja, tenaga, bahan dan
peralatan yang perlu untuk menyempurnakan kerja; dan semua risiko serta luarjangka yang
mempengaruhi perlaksanaan projek ini. Sebarang tuntutan yang timbul akibat daripada
kegagalan Kontraktor mematuhi kehendak ini tidak akan dipertimbangkan.
A.7 Kontraktor hendaklah menyerahkan katalog bagi setiap jenis pemasangan serta jadual kerja
bersama-sama dengan sebut harga ini.
A.9 Sebarang ukuran dan lukisan yang diberikan adalah sebagai panduan sahaja kepada
kontraktor dan pihak kontraktor adalah bertanggungjawab untuk mengukurnya semula.
A.10 Jika terdapat sebarang percanggahan atau kekeliruan, ‘spesifikasi piawai JKR’ akan jadi
muktamad. Semua kerja dalam sebutharga ini diwajib mempersembahkan dengan ‘Good
Engineering Practice’ dan mematuhi Peraturan-peraturan, Undang-undang, Undang-undang
kecil dan Pekeliling yang dikeluar oleh Badan-badan berkuasa.
A.11 Harga yang disebutkan dalam sebut harga ini adalah harga pukal berdasarkan kepada
Unit Kejuruteraan Hospital Seberang Jaya HSJ/SPEC/15/AtspTb
lukisan, spesifikasi dan syarat-syarat kontrak, termasuk amalan pembinaan yang terkini. Ini
juga termasuk harga bagi segala peralatan, jentera, buruh, penyeliaan, rekabentuk struktur
dan pendawaian, bahan-bahan binaan, pembinaan, perancah, bayaran yuran yang berkaitan
dan lain-lain kos yang berkaitan dengan perlaksanaan dan penyiapan projek ini.
A.12 Pendawaian kabel PVC hendaklah sentiasa dilengkapi dengan konduit bergalvani kelas B dan
dipasang tertutup (concealed) sejauh mana yang mungkin. Bagi pemasangan dalam
‘trunking’, pita kuprum 3mm x 25mm hendaklah dipasang di setiap penyambungan ‘trunking’.
A.13 Paip yang menembusi dinding hendaklah dilengkapi dengan ‘pipe sleeve’. Ruang anular
antara paip dan ‘pipe sleeve’ tersebut hendaklah dipenuhi dengan bahan penebat kebakaran.
A.14 Bagi kerja-kerja sivil yang melibatkan pemecahan/pemotongan batu-bata, dinding, ‘partition’,
siling atau mana-mana bahagian bangunan, pihak kontraktor hendaklah mendapatkan
persetujuan daripada pihak hospital terlebih dahulu. Kontraktor dikehendaki mengemas kini
semua kerja pemecahan/pemotongan yang tersebut di atas supaya kekemasan bangunan
terpelihara.
A.15 Kontraktor dikehendaki berhubung dengan (liaise) dengan Syarikat Konsesi iaitu Faber
Mediserve Sdn. Bhd. untuk mendapatkan bekalan utiliti seperti elektrik, air dan sebagainya.
A.16 Segala kerja yang dijangka akan mengganggu operasi hospital hendaklah dimaklumkan
kepada pihak pengurusan hospital secara bertulis sekurang-kurangnya 7 hari sebelum kerja
tersebut dilaksanakan.
A.17 Tempoh Liabiliti Kecacatan untuk projek ini adalah 12 bulan kalender selepas tarikh
penyiapan kerja yang akan diperakukan oleh Pegawai Penguasa.
A.18 Pihak pengguna, Syarikat Konsesi bagi Perkhidmatan Sokongan Hospital yang telah
diswastakan, serta pengurusan Hospital akan melaporkan apa-apa jua kecacatan, yang
hendaklah disenaraikan dalam Jadual Kecacatan dan diserahkan kepada Kontraktor dalam
tempoh 14 hari sebelum penamatan Tempoh Liabiliti Kecacatan. Kontraktor hendaklah
menyempurnakan segala kecacatan yang dilaporkan, seperti yang diarahkan dan diluluskan
oleh Pegawai Penguasa.
A.19 Pegawai Penguasa untuk projek ini adalah Pengarah Hospital/Institusi yang berkenaan, dan
akan diwakili oleh Jurutera Hospital.
A.20 Kontraktor diingatkan supaya sentiasa mematuhi peraturan Bomba atau Pihak Berkuasa
Tempatan yang mempunyai kuasa ke atas sistem pencegah kebakaran dalam segala kerja
pemasangannya. Adalah menjadi tanggung jawab kontraktor untuk mendapatkan kelulusan
pihak berkuasa yang berkenaan sebelum penyerahan bangunan (‘building hand over’).
Segala kos pengubahsuaian bagi memenuhi permintaan pihak berkuasa hendaklah
ditanggung oleh Kontraktor.
A.22 Pihak hospital berhak menyenarai hitamkan kontraktor yang didapati bermasalah dan lewat
menyiapkan kerja.
A.23 Jika terdapat pencanggahan antara spesifikasi dalam dokumen sebutharga dan lukisan akitek
yang dilampirkan, maka lukisan akitek adalah rujukan yang muktamad.

A.24 Jika terdapat kerja yang ditumjukkan dalam lukisan tetapi tidak disebutkan dalam sebutharga,
maka kontraktor hendaklah menulisnya dalam seksyen Kerja-Kerja Tambahan, jika tidak, ia
dianggapi bahawa kontraktor sudah mengira masuk kos kerja tersebut.
A.25 Sebutharga dan tender ini adalah secara harga pukal dan projek ini dikira secara ‘design &
build’ oleh kontraktor, maka sebarang perubahan harga (variation order) daripada kontraktor
adalah tidak dilayan oleh pihak klien.
A.26 Kontraktor dikehendaki mengeluarkan bahan robohan kerja ubahsuai ini keluar dari kawasan
hospital dari masa ke masa di tempoh projek ini.
A.27 Kontraktor perlu menyediakan insuran untuk kemalangan dan kerosakan harta denda,
pampasan pekerja, serta membaik pulih segala struktur, sistem dan sebagainya yang
terganggu atau rosak di tapak setelah siap kerja, (sila sertakan salinan ‘cover note’)

A.28 Kontraktor perlu mematuhi segala peraturan keselamatan dalam hospital seperti larangan
merokok dan sebagainya.
B.1 Kerja-kerja menanggal 2 unit autopsy table yang lama dan memasang autopsy table yang
baru bersama kerja-kerja pemasangan utiliti sepertu elektrik dan perpaipan. Autopsy table
tidak perlu dibekalkan.
Sila rujuk jadual kerja/harga di BAHAGIAN C untuk kerja-kerja terperinci berserta lampiran untuk
maklumat-maklumat teknikal/lain.
GENERAL CONDITIONS OF CONTRACT
1.1. In construing these general conditions and the specifications the following works
shall have the meanings herein assigned to them unless there is something in
the subject or context inconsistent with such works.
1.3 The `Purchaser’ shall mean Hospital Seberang Jaya and shall include the
Purchaser’s heirs, successors and assignees.
1.4 The term `Sub-Contractor’ shall mean the firm or persons named in the contract
for any part of the work or any person to whom any part of the work has been
sublet with the consent in writing of the Engineer-In-Charge and shall include
his heirs, successors and assignees approved by the Purchaser.
1.5 The Term `Inspector’ shall mean any person appointed by/or on behalf of the
Purchaser to inspect supplies, stores or work under the contract or any
person deputed by the Inspector for the purpose.
a) Specifications
b) Drawing
c) Sealed Pattern denoting a pattern sealed and signed by the Inspector.
d) Proprietary make denoting the produce of an individual firm.
e) Any other details governing the construction, manufacture and/or
supply as existing for the contract.
1.7 The term `Engineer’ shall mean Engineer, Engineering Unit, Hospital Seberang
Jaya or some other person for the time being or from time to time duly
appointed in writing by the Owner to act as Engineer for the purpose of the
Contract or in default of such appointment the Purchaser.
1.8 The term `Specification’ shall mean the specifications annexed to or issued
with these Conditions of Contract.
1.9 The term `Site’ shall mean the place or places at which the Equipment is to be
delivered or work done by the Contractor shall include where applicable the
lands and buildings upon or in which the works are to be executed and shall
also include the place or places at which fabrication and other work is being
carried out by the Contractor.
1.10 `Electrical Equipment’, `Stores’, `Work’ or `Works’ shall mean and include
equipment and materials to be provided and work to be done by the Contractor
under the Contract.
1.11 `Tests on Completion’ shall mean such tests are prescribed by the specifications
or have been mutually agreed to between the Contractor/Supplier and the
Purchaser to be made before the equipment is taken over by the Purchaser.
1.12 `Writing’ shall include any manuscript, typewritten or printed statement under
or over signature or seal as the case may be. Words importing `person’
shall include firms, companies, corporations and association of individuals
whether incorporate or not.
1.13 Words importing singular shall also include plural and vice versa where context
requires.
2.1 Contract:
3.1 The contractor should ensure that his people follow safe practices at work.
3.2 Access to the works shall be allowed only to the Contractor/Supplier, Sub-
Contractors or his duly appointed representatives. The Contractor/ Supplier
shall not object to the execution of work by other contractors or tradesman
and shall afford them every facility for execution of their several works
simultaneously with his own.
3.3 Work at the Purchaser’s premises shall be carried out at such time as the
Purchaser may approve but the Purchaser shall give the Contractor/ Supplier all
reasonable facilities for the same. The Contractor/Supplier shall provide
sufficient fencing, notice boards etc. to guard the works and warn the public.
3.4 The Contractor shall obey all Federal, local and State regulations and
enactment pertaining to workmen and labour and the Engineer shall have
the right to enquire into and decide all complaints on such matters.
4.1 Delays:
4.2 The Contractor shall not be entitled to any compensation for any loss suffered by
him on account of delays in commencing or executing the work, whatever the
cause for such delays may be, including delays in procuring Government
controlled or other materials and delay in obtaining instructions and decisions
from Engineer-In-Charge. The Contractor shall, however, merit extension of time
as hereinafter mentioned.
6.2 If the Contractor is delayed in the progress of work by changes ordered in the
work, or by any cause, which the Engineer shall decide to justify the delay, then
the time of completion shall be extended by a reasonable time. No such
extension shall be allowed unless requests for extension are made in writing
by the Contractor/Supplier to the Engineer within 15 days from the date of
occurrence of the delay.
7.2 The Contractor/Supplier shall be responsible for all injury to persons, animals or
things and for all damage to the works, structure of, and decorative work in the
property which may arise from operation or neglect of himself or any of his
Sub-Contractor or of his or Sub-Contractor’s employees, whether such injury or
damage may arise from carelessness, accident or any other cause whatever in
any way connected with the carrying out of this contract. This clause shall be
held to include any damage to buildings, whether immediately adjacent or
otherwise, any damage to roads, streets, foot paths, as well as all damage
caused to the works forming the subject of this contract by frost or other
inclemency of weather. The Contractor/Supplier shall indemnify the Purchaser
and hold him harmless in respect of all and any expenses or property as
aforesaid and also in respect of any claim made in respect of injury or damage
under any acts of Government or otherwise and also in respect of any award
of compensation or damages consequent upon such claim.
7.3 The Contractor/Supplier shall reinstate all damage of every sort mentioned in this
clause, so as to deliver up the whole of the contract works complete and perfect
in every respect and so as to make good or otherwise satisfy all claims for
damage to the property of third parties.
7.4 The Contractor/Supplier shall indemnify the Purchaser against all claims which
may be made against the Purchaser, by any member of the public or other
party, in respect of anything which may arise in respect of the works or in
consequence thereof and shall, at his own expense, effect and maintain, until
the work has been ‘taken over’.
7.5 The Contractor/Supplier shall also indemnify the Purchaser against all claims
which may be made upon the Purchaser whether under the Workmen’s
Compensation Act or any other statute in force during the currency of this
contract or at common law in respect of any employee of the
Contractor/Supplier or of any of his sub-contractor and shall at his own
expense effect and maintain until the work has been `Taken Over’, with an
approved office.
7.6 The Purchaser, with the concurrence of the Engineer, shall be at liberty and is
hereby empowered to deduct the amount of any damages compensation costs,
charges and expenses arising or accruing from or in respect of any such claims
or damages from any sums due to or become due to the Contractor/Supplier.
GENERAL INFORMATION TO BIDDERS
2. The building shall be constructed at the given location. The location is situated on
an empty ground grassed-soil.
3. It is important that the bidder must satisfy himself before quoting that the space
provided is adequate for installation of his plant along with all equipments and
accessories as no extra space can be provided. The tenderers must visit the
site and take the actual measurement before quoting. The bidders shall furnish
a fully dimensioned layout for his offerings, with all equipment and accessories
along with quotation.
4. Bidder shall furnish the data called for in data sheets and the schedules in the
standard Proforma to facilitate correct evaluation of his bid in a most
expeditious manner. It is in the interest of the bidder to submit the bid in
above manner with complete technical details, failing which it is likely that his
bid may not be considered.
7. Bidder shall clearly specify all the deviations with respect to this specification
in the appropriate schedule.
8.1 All equipments mentioned in the enclosure should conform to the respective
technical specifications. Only main items of the tender have been brought out
specifically in this schedule, however all accessories as per specification or
otherwise should be furnished at the time of quotation with cost of individual
item.
8.2 Tenderers are required to fill in unit rates for all the items mentioned in the
enclosure and any additional items quoted by them in the Financial Bid. In
absence of the unit rates, the offer may be considered as invalid.
The purchaser will provide for the contractor following services during the
performance of work.
10.1 Temporary electric power at 415 V, 3 ph., 50 Hz at one point only which will be
within 50 M of location of works free of cost. Water shall be provided free of
cost for testing and commissioning.
10.2 The contractor shall make his own arrangement for supply, erection and
dismantling on completion of works of his temporary distribution system,
distribution panels and other equipments he may require to take the power
from the purchaser's supply points.
10.4 The purchaser will not hold himself responsible for the consequences of
any interruptions to the continuity of the power supply or power system voltage
and frequency fluctuations.
10.5 The electric power shall not be used for heating purposes.
10.6 Electrical power made available to the contractor, during testing and
commissioning for the refrigeration unit is free of charge.
10.7 There is no EOT crane, skylift or mono rail to assist in erection work. Tenderer
should provide suitable derricks & tripods for lifting heavy load.
11. SERVICE SUPPORT, SPARES & TOOLS:
11.2 The tenderer shall furnish complete set of tools & wrenches for making
adjustments, repairs & preventive maintenance including those required for
erection.
12.1 The contractor shall submit tracings in original with three prints of all
installation drawings based on which equipments are installed &
commissioned. A set of soft-copy is mandatory.

INSTRUCTIONS TO BIDDERS & SPECIFIC
NON-TECHNICAL REQUIREMENTS
1. Prices and rates quoted shall include cost of all materials, labour, plant,
equipment, temporary work, tools, setting out, supervision, transport, taxes,
royalties, octroi and any local tax or levy payable on all transactions, insurance
and everything necessary for due performance of work under this contract.
2. Prices and unit rates shall be valid even if the contract is split.
4. a) Site Visit:
The bidders may like to visit the work site before submitting their bids
to get conversant with site conditions, which may affect the work.

5. The Contractor should plan and give his daily program for execution of work in
advance so that no disturbance is created in the routine working of the Institute.
8. The contractor shall not without the consent in writing of the Owner assign or
sublet the contract.
9. No labour below the age of 18 years shall be employed at site.
10. Contractor shall be liable to pay workman compensation to his labour as per
prevailing rules for any accident occurring at our site.
11. The contractor shall pay wages to his workmen as per the Statutory Minimum
Wages Act.
12. The Contractor shall provide all risk insurance for his men, material and
machines and absolve us of all risk and liabilities whatsoever pertaining to
his men, material and machines.
13. The Contractor will arrange for photo passes of his personnel and get the same
endorsed by our Establishment/Security Department before beginning the work.
14. The Contractor will ensure that his workmen follow safe practices at Work.
15. The Contractor will work from 8.00 a.m. to 6.00 p.m.
16. No labour camp etc. will be allowed to be put up inside or in the vicinity of the
Hospital.
17. The contractor should ensure that his engineer/supervisor is present at
site throughout the progress of works till completion.
18. The bidder should submit technical data sheet, drawings, any other technical
detailed asked for in the tender with Part II i.e. Technical Bid of the Tender.
19. The schedule of quantities and rates shall be read in conjunction with the
specification, tender drawings, other tender documents, relevant MS Codes,
Engineering hand books, etc. the Contractor shall not rely merely on the
description given in the Schedule of Quantities.
18. Contractor will be fully responsible for lifting, shifting and handling Hospital’s
properties such as any articles and materials lying nearby to proper place as
instructed by Site In- Charge.
19. All the materials used shall be used only after approval and tests of the
Engineer-In- Charge as per specifications.
20. All the debris shall be removed by the Contractor at his cost to any nearest
BMC approved dumping yard.
21. Storage and custody of material, till the time of handing over to HSJ, shall
be the responsibility of contractor.
22. The contractor after award of work, should submit detailed schedules showing
program and order in which contractor proposes to carryout the work with
dates and estimated completion time for various parts of work. This should be
submitted in the form of Bar- chart.
23. The Engineer can instruct the contractor to make any alteration, omission, or
variation in work. The difference in cost due to instruction shall be added or
deducted from contract price as the case may be in accordance with the rate
applicable in the contract.
24. DEVIATION FROM THE SPECIFICATIONS:
The Contractor must tender for the equipment as herein specified but
may submit alternate proposals for modifications to details where such
modifications offer technical advantages and will make cost savings to the
owner. The basis and details of calculations of operating cost and the
net expected final saving shall be indicated wherever such a claim is
made. The acceptances or otherwise of alternate proposal/deviations/
modifications etc. will be decided solely by the owner.
25. Inspection of Material & Equipment:
Supplier shall arrange for inspection/testing of all systems at his works.
HSJ will have liberty to inspect/test the parts of any new installations
during the manufacturing/assembly. No charges will be paid extra
towards inspection/testing. The bidder shall make necessary
arrangements for travel, boarding & lodging for 2 engineers of HSJ
deputed for inspection & testing of any relevant equipment at
manufacturer’s premises if applicable.

B. TEST PROCEDURES
It should be noted that the continuity and insulation resistance tests for lighting circuit described below
can only be applied to a 'loop-in' wiring system only.
1.1 Test method 1 (CPC conductor terminated at the last point of the circuit and using the neutral
conductor as a return lead).
1.1.1 Connect the neutral conductor to the CPC conductor al the Iast point of the circuit under test.
1.1.2 Test between neutral and earth of the circuit at the DB.

1 Semua kerja-kerja pengujian perkara-perkara diatas perlulah dilakukan oleh orang kompeten sahaja, iaitu
pendawai/penjaga jentera bertauliah dan berdaftar dengan Suruhanjaya Tenaga Malaysia.
2 Penguji WAJIB menyertakan sijil kekompetenan semasa penyerahan borang ini dan menandatangani setiap helaian
borang yang disi (disisi bawah kiri).
3 Tindakan yang sewajarnya akan diambil sekiranya perkara-perkara diatas tidak dipatuhi.
NOTES:
1 Regulation 713-09-01 colls for a polarity test to be carried out to establish that:
1.1 All fuses, circuit breakers and single pole switches are connected in the phase conductor only.
1.2 The centre contact of Edison Screw (ES) lampholders are connected to the phase conductor.
1.3 The polarity of socket outlets, other accessories and points of utilisation (eg luminaires) is correct
2 The polarity of all circuits must be verified before connection to the supply.
3 Polarity testing must be carried with the supply isolated from the installation.
4 There are a number of ways in which polarity can be tested.
4.1 Polarity test on lighting circuits, for example, can be conducted at the same time as that for continuity of CPC.
5 After connection of the supply, polarity should be confirmed using an approved voltage indicator.
