HANDOVER

Pada pukul 14.00 WIB di ruangan Melati RS Karyadi, para perawat akan melaksanakan kegiatan asuhan
keperawatan

Setelah dinas pagi berakhir para perawat pelaksana akan memberi laporan terkait pasien yang ditangani
kepada Ketua Tim dan Karu dan akan melakukan handover dengan perawat pelaksana sore di nurse
station

A : asalamualaikum, selamat siang semuanya, Karena dinas pagi sudah berakhir, kita akan
melakukan handover yang dipimpin oleh saya sendiri. Untuk mengawali handover pada siang hari ini kita
akan mendengarkan asuhan keperawatan dari tim sift pagi, silahkan untuk ners fitri.

B : Terimakasih. Baik, disini saya akan melaporkan kondisi pasien sift pagi. Sesuai dengan pasien
yang saya tangani Disini saya merawat Tn. D dengan diagnosa DM Luka pada ekstremitas kanan bawah,
sesuai anjuran dari dokter saya telah memberi obat-obatan dan melakukan perawatan luka TTV Tn D
terlihat normal, hanya saja KGD masih tetap yaitu 256 mg/dl. keadaan luka masih ada pus dan jaringan
nekrotik disekitamya. Untuk kondisi pasien saat ini mengatakan tidak bisa tidur karena Luka di kakinya,
kemudian merasa lebih nyaman tidur dengan posisi semi flowler. Tn. D ini berusia 45 tahun, masuk
rumah sakit pada 15 November kemarin pukul 17.00, keluhannya kaki merasa sakit karena luka tidak
sembuh-sembuh.

A : Baik terimakasih Ners Fitri apakah masih ada yang ingin disampaikan oleh Tim sift Pagi?.

A : Oke, silahkan Tim sift Perawat sore kepada ners fatim Apakah ada yang ingin diklarifikasi dari
tim sift sore?

C : Baik Terimakasih, jadi untuk Tn D dengan diagnose DM+Luka Geggren pada ekstremitas kanan
bawah, sesuai anjuran dari dokter telah diberikan obat-obatan dan melakukan perawatan luka. TTV Tn D
terlihat Normal, Hanya saja KGD masih tetap Yaitu 256 mg/dl, Keadaan luka masih ada pus dan jaringan
nekrotik di sekitarnya. Untuk kondisi pasien saat ini mengatakan tidak bisa tidur karena Luka di kakinya,
kemudian merasa lebih nyaman tidur dengan posisi semi flowler. Tn. D ini berusia 45 tahun, masuk
rumah sakit pada 15 November kemarin pukul 17.00, keluhannya kaki merasa sakit karena luka tidak
