- Daily permit to Work dilakukan setiap shift kerja. Supervisor/ Pelaksana memaparkan rencana kerja, potensi bahaya
dan pengendaliannya. Safety Officer menyampaikan temuan Daily Patrol kemarin dan
- JSA tindakan perbaikannya.

KIKEN YOCHI MEETING


Ada 3 level KY Meeting (TBM)
1. Supervisor / SO berbicara di tengah lingkaran, menyampaikan potensi bahaya dari
pekerjaan yang akan dilakukan secara umum
2. Dalam 1 lingkaran (max. 20 orang), Leader (bisa Spv atau Foreman), menunjuk
random anggotanya, pekerja yang ditunjuk harus bisa menyampaikan : pekerjaan
yang akan dia dilakukan, material dan alat yang akan digunakan, bahaya dari
pekerjaan dan pengendaliannya
3. Memberi tepukan psikologis dan membuat Laporan KYM dan Leader mengisi
Papan KYM

1. Total beban yg diangkat melebihi ambang batas standar . Contoh angkatan > 80% dari
SWL load chart.
2. Pengangkatan orang
3. Material khusus/ Abnormal Size
4. Pengangkatan menggunakan dua crane atau lebih
5. Pengangkatan material yg khusus, mudah rusak, dan bila rusak tidak bisa diperbaiki,
mengganggu proses kerja, dll.
6. Pengangkatan material yg akan dibongkar/demolish, ketidaktahuan berat beban yg
akan diangkat
7. Pengangkatan di area power lines, over active work areas, occupied buildings, public
roadways, transportation systems, etc.
8. Operasional pengangkatan di area terbatas (confined space), keterbatasan ketinggian
boom, dll.
