 Sistem Layanan Kesehatan
Sistem pelayanan kesehatan merupakan salah satu struktur
multidisipliner yang bertujuan untuk mencapai derajat kesehatan
optimal. Keperawatan merupakan bagian integral dari sistem
pelayanan kesehatan yang mempunyai tujuan untuk meningkatkan
kesejateraan manusia. Pelayanan keperawatan adalah upaya untuk
membantu individu baik yang sakit maupun yang sehat, dari lahir
hingga meninggal dalam bentuk pengetahuan, kemauan dan
kemampuan yang dimiliki.
Faktor-Faktor yang mempengaruhi
Sistem Pelayanan Kesehatan
1. Ilmu pengetahuan dan teknologi baru
Pelaksanaan sistem pelayanan kesehatan dapat dipengaruhi oleh ilmu
pengetahuan dan teknologi baru, mengingat perkembangan ilmu
pengetahuan dan teknologi, maka akan diikuti oleh perkembangan
pelayanan kesehatan atau juga sebagai dampakya pelayanan kesehatan
jelas lebih mengikuti perkembangan dan teknologi seperti mengatasi
masalah penyakit-penyakit yang sulit Berdasarkan itu maka pelayanan
kesehatan membutuhkan biaya yang cukup mahal dan pelayanan akan
lebih professional dan butuh tenaga-tenaga yang ahli dalam bidang
tertentu.
2. Pergeseran nilai masyarakat
Berlangsungnya sistem pelayanan kesehatan juga dapat dipengaruhi
ole nilai yang ada di masyarakat sebagai pengguna jasa pelayanan,
di mana dengan beragamnya masyarakat, maka dapat menimbulnya
pemanfaatan jasa pelayanan kesehatan yang berbeda. Masyarakat
yang sudah maju dengan pengetahuan yang tinggi, maka akan
memiliki kesadaran yang lebih dalam penggunaan /pemanfaatan
pelayanan kesehatan, demikian juga sebaliknya.
3. Aspek legal dan etik
Dengan tingginya kesadaran masyarakat terhadap penggunaan
atau pemanfaatan jasa pelayanan kesehatan, maka akan semakin
tinggi pula tuntutan hukum dan etik dalam pelayanan kesehatan,
sehingga pelaku pemberi pelayanan kesehatan harus dituntut
untuk memberikan pelayanan kesehatan secara professional
dengan memperhatikan nilai-nilai hukum dan etika yang
berlaku.
Ruang lingkup pelayanan kesehatan
1. Ruang Pasien rawat inap
Ruangan untuk pasien yang memerlukan asuhan keperwatan dan
pengobatan secara berkesinambungan lebih dari 24 jam.
2. Ruang Administrasi
Ruang untuk menyelenggarakan kegiatan administrasi khususnya
pelayanan pasien di ruang rawat inap. Ruangan ini menjadi satu
dengan nurse station di ruang rawat inap bagian dean lantai 2
ruangan ini dilengkapi meja kursi, lemari berkas/ arsip. Telepon. dan
komputer Kegiatan di ruangan in meliputi: Tempt serah terima
pasien barum dan pulang, pendatanganan surat pernyataan keluarga
pasien ( apabila diperlukan persetujuan pengobatan, tindakan
perawatan ataupun tindakan bedah).
3. Ruang kepala rawat inap
Ruangan ini bergabung dengan ruangan Bidang keperawatan yang
mana ruangan terserbut untuk melakukan menejemen asuhan dan
pelayanan keperwatan diantaranya pembuatan program kerja dan
pembinaan.
4. Ruang Linen bersih
Ruang untuk menyimpan bahan - bahan linen bersih yang akan di
gunakan di ruang rawat, dan linen bersih tersebut di tempatkan di
dalam lemari yang ada di nurse station.
5. Ruang linen kotor
Ruang untuk menyimpan bahan - bahan linen kotor yang telah di
gunakan di ruang rawat inap sebelum di bawa ke rang cuci ( laundri) di
lengkapi wadah seperti ember untuk linen yang kotor.
6. Spoolhoek
Fasilitas untuk membuang kotoran bekas pelayanan pasien khususnya
yang berupa cairan. Pada ruangan spoolhoek harus di sediakan kran air
bersih untuk tempatmencuci tangan atau tempat cairan.
7. Kamar mandi / toilet
Fasilitas diatur kebutuhan dan harus dijaga kebersihannya karena
dengan kamar mandi/toilet yang bersih.
Bentuk Pelayanan Kesehatan
1) Pelayanan kesehatan tingkat pertama (primer), Diperlukan untuk
masyarakat yang sakit ringan dan masyarakat yang sehat untuk
meningkatkan kesehatan mereka atau promosi kesehatan.Contohnya:
Puskesmas,Puskesmas keliling, Klinik.
2) Pelayanan kesehatan tingkat kedua ( sekunder), Diperlukan untuk
kelompok masyarakat yang memerlukan perawatan inap, yang sudah
tidak dapat ditangani oleh pelayanan kesehatan primer.Contoh: Rumah
Sakit tipe C dan Rumah Sakit tipe D.
3) Pelayanan kesehatan tingkat ketiga ( tersier), Diperlukan untuk
kelompok masyarakat atau pasien yang sudah tidak dapat ditangani
oleh pelayanan kesehatan sekunder.Contohnya: Rumah Sakit tipe A
dan Rumah sakit tipe B.
Pelayanan kesehatan dibedakan menjadi 2 yaitu :

1. Pelayanan Kedokteran, Pelayanan dengan cara pengorganisasian yang

bersifat sendiri atau secara bersama-sama dalam suatu organisasi,
tujuan utamanya untuk menyembuhkan penyakit dan memulihkan
kesehatan, serta utamanya adalah perseorangan dan keluarga.
2. Pelayanan Kesehatan Masyarakat, Pelayanan Kesehatan Masyarakat
Ditandai dengan cera pengorganisasian yang umunnya secara
bersama-sama dalam suatu organisasi, tujuan utamanya yaitu untuk
memelihara dan meningkatkan kesehatan serta mencegah penyakit,
serta sasaran utamanya adalah kelompok dan masyarakat.
 Sistem Rujukan

Sistem Kesehatan Nasional membedakannya menjadi dua macam yaitu:

1. Rujukan Kesehatan, Upaya pelayanan kesehatan dalam pencegahan
penyakit dan peningkatan derajat kesehatan.Rujukan ini dibedakan
menjadi tiga yaitu:- Rujukan teknologi- Rujukan sarana- Rujukan
Operasional.
2. Rujukan Medik, Upaya pelayanan kedokteran dalam penyembuhan
penyakit serta pemulihan kesehatan. Rujukan medic terdiri dari
penderita, pengetahuan, dan bahan laboratorium.
 Puskesmas

Pusat Kesehatan Masyarakat, disingkat Puskesmas, adalah

fasilitas pelayanan kesehatan yang menyelenggarakan upaya
kesehatan masyarakat dan upaya kesehatan perseorangan
tingkat pertama, dengan lebih mengutamakan upaya promotif
dan preventif, untuk mencapai derajat kesehatan masyarakat
yang setinggi-tingginya.
Upaya kesehatan di Puskesmas terbagi menjadi Upaya Kesehatan Masyarakat dan
Upaya Kesehatan Perseorangan.
Upaya Kesehatan Masyarakat, meliputi:
1. Upaya kesehatan masyarakat esensial yang terdiri dari pelayanan promosi
kesehatan, kesehatan lingkungan, kesehatan ibu, anak dan keluarga berencana;
pelayanan gizi; pelayanan pencegahan dan pengendalian penyakit.
2. Upaya kesehatan masyarakat pengembangan disesuaikan dengan prioritas masalah
