adakah gambaran khas ileus mekanik.
Manajemen
Awasi KU/VS/ akut abdomen dan trias stabilitas ( cairan, elektrolit, asam-basa).
Karena terdapat dehidrasi berat rehidrasi s/d dehidrasi teratasi (Ebb phase)
bila sudah teratasi berlanjut ke “flow phase” dengan pemberian cairan nutrisi
maintenance.
Dekompresi bawah dengan rectal tube.
Dekompresi atas dengan NGT dialirkan s/d puasa bila intoleransi diit.
Injeksi antibiotik broad spectrum.
Pemeriksaan penunjang lainnya untuk menegakkan diagnosis.
Bila diagnosis sudah tegak Definitive treatment / operation.
7. Patofisiologi
a. Hipoalbumin :
-Penurunan produksi albumin (kerusakan sel hati), kekurangan intake, peningkatan
pengeluaran albumin).
-Kebocoran ekstravaskuler disebabkan peningkatan permeabilitas.
-Hemodilusi.
-Inflamasi akut dan kronis ; permeabilitas ↑
degradasi albumin ↑ disebabkan ↑ cytokine TBF & ILG
sintetis albumin ↓
b. Hiponatremia : peningkatan kehilangan natrium (muntah,diare).
c. Hipokalemia : defisit kalium (Renal loss : obat, muntah, hipermagnesia).
Tanpa defisit kalium : alkalosis, insulin, hiperventilasi.
d. Trombositopenia : disebabkan terjadinya konsumptif koagulopati disebabkan oleh
pelepasan endotoksin karena translokasi kuman.
DIC : distensi usus (kembung) menekan vasa usus translokasi kuman ke sirkulasi
endotoksin dilepaskan konsumptif koagulopati faktor koagulopati ↓↓ DIC.
8. Indikasi pemasangan ventilator pada bayi.
Gagal nafas : PO2 < 60
PCO2 >50
Indikasi lepas ventilator bila sudah ada nafas spontan.
- Pasien sadar/ nafas spontan.
- AGD baik/optimal sesuai penyakit.
- Mode ventilator sudah low setting.
- Hemodinamik stabil.
- Dosis inotrophic minimal.
- Perfusi vaskular perifer baik.
- Pasien tidak gelisah.
- Tidak ada bleeding di area airway.
- Refleks batuk baik.
- Puasa cukup.

1. Persiapan pasien megacolon congenital yang akan dilakukan pemeriksaan colon in loop
sehingga hasilnya representative, yaitu :
- Dilakukan irigasi / washing out dengan rectal tube dan dengan larutan NaCl 0.9%
dengan residu 10 cc/kgBB, dilakukan setiap pagi dan sore sampai dengan bersih dan
pemberian antibiotik.
- Anak/pasien bebas dari irigasi 1x24 jam atau 2x24jam dan bebas manipulasi anus.
- Untuk bayi s/d usia 2 tahun, tidak ada persiapan khusus yang diperlukan.
- Untuk anak > 2 tahun :
a. Pada malam hari sebelum pemeriksaan hanya makan-makanan diit rendah serat.
b. Pada malam hari sebelum pemeriksaan minum obat pencahar seperti laxative atau
bila diperlukan diberikan urus-urus seperti fleet enema.
- Pada pemeriksaan diusahakan pasien tidak banyak bicara, dan tidak menangis untuk
mencegah aerofagia.
- Edukasi orang tua tentang tata cara, guna,dan risiko dari pemeriksaan colon in loop.
