KEKEBALAN NONSPESIFIK (INNATE IMMUNITY)
Sistem proteksi permukaan merupakan langkah awal dimana sebagian besar Antigen
ditolak masuk kedalam tubuh
Sistem proteksi permukaan dilakukan melalui mekanisme fisik, kimiawi dan
lingkungan/biologi
alamiah menghambat pertumbuhan mikroorganisme pathogen
Sistem pertahanan lingkungan, pada daerah tertentu dimana pertahanan fisiknya
relatif lemah dilengkapi mekanisme imunologis
Kekebalan yang dibawa sejak lahir
Secara genetik sudah ditentukan
Innate immunity ~ kekebalan non spesifik
Kekebalan lapis pertama dan kedua
Tiga Garis Pertahanan Melawan Infeksi:

Mekanisme Proteksi Permukaan Nonspecific:

magnitude higher levels of antibody
Ability of antibody to bind antigen also increases dramatically in
the secondary response
22
Injecting a new antigen “B” with “A":
Elicits only a primary response
Shows that a memory or prior exposure is required for the
accelerated response

o Menjelang lahir kemampuan bakterisidal dan fagositosis menurun disebabkan
karena sintesa glucocorticoid meningkat
o Sesudah lahi rrespon kemotaktik makrofag menurun
(Infectious Bovine Rhinotracheitis) IBR, Bovine Viral Diarrhea (BVD),

43
Absorpsi Kolostrum
menghambat absorpsi
o Permeabilitas usus terhadap protein paling tinggi segera setelah lahir →
Setelah 24 jam → Absorbsi Ig turun ketingkat yg rendah
Akibat Kekebalan Pasif asal Induk:
o IgG kolostrum melindungi anak terhadap septisemik
o IgA melindungi anak terhadap infeksi usus
o Bila IgG atau IgA rendah:
Kelahiran premature → kolostrum belum siap
Jumlah anak terlalu banyak
Luka pada putting susu/ambing
o Akibatnya pada hewan baru lahir:
Bisa timbul koliseptisemia, pneumonia dll

terhadap virus korona setelah 3 –9 hari post vaksinal
Anak babi peoral vaksin Transmissible Gastroenteritis Virus (TGE)
→ 3 hari postnatal → Kebal 5 –14 hari kemudian
o Tanggap kebal sistemik:
Pada anak sapi tidak menyusu → hipogamaglobinemia →
membentuk Ig sendiri umur 1 minggu → Tapi bila Ig cukup → Ig
sendiri baru setelah 4 minggu
Anak Babi tanpa kolostrum, tanggap kebal terhadap pseudo-rabies 2
hari post natal, tapi bila menyusui, tanggap kebal baru sesudah umur
5-6 minggu

AN OVERVIEW OF IMMUNE SYSTEM (AGAINST
INFECTIOUS SYSTEM)
[By: Imam Megantara, dr., SpMK., SpTHT-KL., M.Kes]
46
o Human immune system begins to develop in the embryonic periodes
o Starts with hematopoietic stem cells (HSC)
o Immune cells (and other blood cells) made in Bone marrow – All are
descendents of self-renewing stem cells
How our immune system work? → The immune system protects against diseases with
layered defenses → Innate immunity vs Adaptive immunity:
o In innate immunity: recognition and response rely on traits common to groups
of pathogens
o In adaptive immunity: receptors provide pathogen-specific recognition
The Organisation of Immune Systems:
47
Comparison of Innate to Adaptive Immunity:
48
e. Phospholipase A2 (saliva, tears, and breast milk) are also
antibacterial
Gastric Juice:
a. Mixture of HCl, enzymes, and mucus.
b. pH between 1,2-3 kills microbes and destroys most toxins.
c. Many enteric bacteria are protected by food particles.
d. Helicobacter pylori neutralizes stomach acid and can grow in the
stomach, causing gastritis and ulcers.
Semen contains defensin and zinc that kill pathogens
Several microorganisms are capable of penetrating mucous membranes:
a. Papilloma virus
b. Treponema pallidum
c. Enteroinvasive E. coli
d. Entamoeba histolytica
o Biological Barrier:
Commensal flora competing with pathogenic bacteria and, in some case,
changing the conditions in their environment, such as pH or available
iron
Second Line of Defense → System that comes immediately into play if infectious
agents or pathogen past the surface barrier
o Humoral → Antimicrobial substance, plasma protein system (complement
system, coagulation system, kinin system), interferon, dan cytokines, CRP
o Cellular → Neutrophils-Basophile-Eosinophile (PMN), Macrophages (MN),
Null cell (LGL): Natural Killer (NK) and K Cell, Mast Cell
To replicate and spread → some microbes hijack cellular machinery (Intracellular
Pathogens) → These include viruses, intracellular bacteria (listeria) and intracellular
parasites (malaria, toxoplasma) → “Cell-Autonomous Defenses” → Cell produces an
immune response that acts on itself (Apoptosis, Interferon, Protein kinase (PK)-R/anti
viral stage)
49
Apoptosis:
o Apoptosis is a pathway of cell death induced by tightly regulated intraceluller
programed in which destined to die, activates enzymes that degrade cell’s own
DNA and nuclear and cytoplasmis proteins
o Necrosis is a spectrum of morphological changes that follow cell death in living
tissue due to progressive degradative of enzyme on lethally injured cell.
o Morphology of Apoptosis:
Cell shrinkage
Chromatin condensation
Formation of apoptotic body and blebs
Fagocytosis of apoptotic body
o Biochemical features:
Protein cleavage
DNA breakdown
Phagocyte recognition
o Mechanisms of Apoptosis:
Initiation phase (apoptotic enzyme getting activated)
a. Extrinsic pathway → dead receptor pathway [TNF family: Fas,
TNFR1]
b. Intrinsic pathway → mitochondrial pathway [pro-apoptotic
protein : BAK, BAX]
Execution Phase (cell actually died)
a. Activated caspase 3, 6, 8, 9,10 etc
b. Breakdown cytoskeleton protein, nuclear matris protein
50
o Necrosis vs Apoptosis:
Interferon (IFN):
o Antiviral proteins that interfere with viral multiplication
o Small proteins (15,000 to 30,000 kDa)
o Heat stable and resistant to low ph
o Important in acute and short term infections
o Have no effect on infected cells
51
o Host specific, but not virus specific
o IFN α and β: produced by virus infected cells and diffuse to neighboring cells
→ cause uninfected cells to produce antiviral proteins (avps).
o IFN ϒ: produced by lymphocytes → causing neutrophils to kill bacteria
o Anti-viral State induced by Interferon (IFN) → IFN will induce an antiviral state
via two enzyme :
Protein Kinase R (PKR) and 2’5’-oligoadenylate syntetase
52
Inflammatory Response:
o Triggered by tissue damage due to infection, heat, wound, etc.
o Four (or five) major symptoms of inflammation: redness, pain, heat, swelling,
and may also observe loss of function
o Functions of Inflammation:
Destroy and remove pathogens
If destruction is not possible, to limit effects by confining the pathogen
and its products.
Repair and replace tissue damaged
o Inflammation is the sum of the host’s defences to infection or noxious stimuli
o Aims to bring defence cells (immune cells) to the area of concern
o Goals:
Limit and control the inflammatory process
Prevent and limit infection and further damage
Initiative adaptive immune response
Initiate healing
o Stages of Inflammation:
53
Complement System:
o Complex of serum (molecule) proteins (~20 different proteins) that work
together to destroy invaders and recruit immune cells
o Produce a rapid, highly amplified response mediated by a cascade
phenomenon
o Activation of complement system is tightly regulated because end results can
be dangerous
o Activated by three different ways: Classical pathway, Alternative pathway,
MB-Lectin pathway
o The three pathways of complement activation converge:
54
o Classical, Alternative & Lectin Pathway:
55
Basophiles → smallest part → mediator
o Stages of Phagocytosis:
56
Triggering of PRRs on macrophage or dendritic cells can induce a large
variety of events including:
Increased phagocytosis
Production of cytokines and inflammatory mediators:
o Interferons to induce anti-viral state
o Chemokines to attract migrating cells
Increased cell migration
Changes in expression of molecules involved in T cell APC
function
Pathogen recognition receptors (PRR):
57
Different mammalian Toll-like receptors (TLRs) are specific for different classes
of microbial products:
The genes encoding the antigen receptors of T and B cells assembled by DNA
rearrangement → As a result of V(D)J recombination, every B and T cells expresses a
unique version of the antigen receptor → As a result of V(D)J recombination every
58
mature B cell expresses a unique antibody → Encounter with an antigen leads to
clonal expansion of B cells with a particular specificity
The genes encoding the T cell antigen receptor (TCR) are assembled
by DNA rearrangement as T cells develop in the thymus
V segments D J C
TCR locus:
structure in germline
DNA rearrangement
(rag1, rag2)
TCR locus:
structure in T cells
transcription
RNA splicing
translation
T cell
59
Most are CD4+
Recognize antigen on the surface of APCs
It’s Central role in immune response: activated TH are involved
in a number of antigen specific reactions:
1. Activate macrophages [included inflammatory
response]
2. Induce formation of cytotoxic T cells
3. Stimulate B cells to produce antibodies
Cytotoxic T (Tc) Cells:
60
T-Cells and Cellular Immunity:
o [Lymphocytes] T-cells are the first cells to recognize antigen specifically
o Cellular immunity is not transferred to the fetus
o Without T-cells there is no effective antigen specific immunity
o T cells interact specifically with antigen through T-cell receptors (TCR)
o TCR cannot recognize foreign antigens UNLESS they are associated with
major histocompatibility complex (MHC) proteins on the surface of cells
Major Histocompatibility Complex:
o All MHC are imbedded in the lipid membrane of cells and project outward from
the cell surface
o The MHC complex proteins present on chromosome 6 in humans
o Class I-MHC proteins and Cytotoxic T-cells (Tc) → Class I pathway is useful
in destroying cells infected by viruses or transformed by tumors → How they
work?
61
Protein antigens manufactured in the cell by viruses or
tumors are degraded in the cytoplasm and transported to the
Endoplasmic reticulum
62
o Class II-MHC proteins and T-Helper cells (TH)
64
o Fc region: Stem of monomer only. Important because they can bind to
complement or cells

66
Antigens:
o Molecules from a pathogen or foreign organism that provoke a specific immune
response
o Most are proteins or large polysaccharides from a foreign organism
Microbes: capsules, cell walls, toxins, viral capsids, flagella, etc.
Non microbes: pollen, white egg, RBC surface molecules, serum
proteins, and surface molecules from transplanted tissue
o Lipids and nucleic acids are only antigenic when combined with proteins or
polysaccharides
o Molecule Weight = 10,000 kDa or higher
o Hapten: Small foreign molecule that is not antigenic. It must be coupled to a
carrier molecule to be antigenic
o Epitope:
Small part of an antigen that interacts with an antibody
Any given antigen may have several epitopes
Each epitope is recognized by a different antibody
o Antigen-Antibody Complex: Formed when an antibody binds to an antigen it
recognizes. What is the consequences of Antigen-Antibody Binding?
67
Naturally Adapted Immunity → Obtained in the course of daily life
o Active Immunity:
Antigens or pathogens enter body naturally
Body generates an immune response to antigens
Immunity may be lifelong (chickenpox or mumps) or temporary
(influenza or intestinal infections)
o Passive Immunity:
Antibodies pass from mother to fetus via placenta or breast feeding
(colostrum)
No immune response to antigens
Immunity is usually short-lived (weeks to months)
Protection until child’s immune system develops
Artificially Adapted Immunity → Obtained by receiving a vaccine or immune serum
Active Immunity:
Antigens are introduced in vaccines
(vaccinations/immunization)
Body generates an immune response to antigens
Immunity can be lifelong (oral polio vaccine) or temporary
(tetanus toxoid)
Passive Immunity:
Preformed antibodies (antiserum) are introduced into body by
injection
Snake antivenom injection from horses or rabbits
Host immune system does not respond to antigens
Immunity is short lived (half life three weeks)
Duality of Adaptive Immune:
o Humoral Immunity (Ab Mediated)
Involve B cells and production of antibody
Stimulated B cells which secrets antibody called plasma cells
Ab are found in extracellular fluids and the surface of B cells
Against circulating bacteria, bacterial toxins, and viruses
68
o Cell Mediated Immunity
Involve T cells that recognize antigens on the surface of cells,
organisms, or tissues: Helper-T and Cytotoxic T-cells
T cells regulate proliferation and activity of other immune cells
Defense against: bacteria and viruses that are inside host cells and are
inaccessible to antibodies, fungi, protozoa, and helminths, cancer cells
and transplanted tissue
Relationship Between Cell-Mediated and Humoral Immunity:
o Antibody Production:
T-Dependent Antigens:
Antibody production requires assistance from T helper cells
A macrophage cells ingest antigen and presents it to TH cell
TH cell stimulates B cells specific for antigen to become plasma
cells
Antigens are mainly proteins on viruses, bacteria, foreign red
blood cells, and hapten-carrier molecules
T-Independent Antigens:
Antibody production does not require assistance from T cells
Antigens are mainly polysaccharides or lipopolysaccharides
with repeating subunits (bacterial capsules)
Weaker immune response than for T-dependent antigens
o Antibody Dependent Cell Mediated Cytotoxicity:
ADCC is the killing of an antibody-coated target cell by a cytotoxic
effector cell through a nonphagocytic process, characterised by the
release of the content of cytotoxic granules or by the expression of cell
death-inducing molecules
Target cell or organisms is covered with antibodies, leaving Fc portion
sticking outwards, and then target cells/organism is lysed by substances
secreted by attacking cells
Natural killer (NK) cell and other nonspecific cells that have receptors
for Fc region are stimulated to kill targeted cells
FcγRIIa and FcγRIIIa polymorphism impact ADCC efficacy by IgG1
antibodies
69
Destruction of Large Parasites by ADCC
70
