Abstrak
Latar Belakang: Latihan olah raga bertujuan untuk mengurangi gangguan spesifik penyakit dan
meningkatkan kualitas hidup pasien dengan penyakit paru obstruktif kronik (PPOK). Eksaserbasi PPOK
secara khusus berdampak negatif pada perkembangan PPOK. Intervensi terapi fisik tampaknya
diindikasikan untuk mempengaruhi eksaserbasi dan konsekuensinya. Namun, informasi tentang
pengaruh terapi fisik pada kejadian eksaserbasi masih langka. Penelitian ini bertujuan untuk mengetahui
potensi program terapi fisik yang diarahkan pada protokol sebagai sarana untuk mencegah atau
menunda eksaserbasi, mempersingkat durasi atau menurunkan keparahan eksaserbasi pada pasien
PPOK yang baru saja mengalami eksaserbasi. Selain itu, penelitian ini berfokus pada pengaruh terapi
fisik yang diarahkan protokol pada status kesehatan dan kualitas hidup serta pada efektivitas biaya dan
utilitas biaya pada pasien PPOK yang baru-baru ini mengalami eksaserbasi.
Metode / Desain: Sebuah kelompok prospektif yang terdiri dari 300 pasien PPOK di semua tahap GOLD
akan dibangun. Pasien akan menerima perawatan PPOK multidisiplin termasuk terapi fisik yang
diarahkan pada pedoman. Pasien dalam kelompok ini yang memiliki EMAS stadium 2 sampai 4
(pasca-bronkodilator FEV1/ FVC <0,7 dan FEV1 <80% dari prediksi), yang menerima penggantian oleh
perusahaan asuransi kesehatan untuk terapi fisik (pasca-bronkodilator Tiffeneau-index <0,6 ) dan yang
mengalami eksaserbasi PPOK akan diminta dalam waktu 56 hari untuk berpartisipasi dalam uji coba
terkontrol acak prospektif bersarang kohort (RCT). Dalam RCT kali ini, kelompok intervensi akan
mendapatkan program terapi fisik yang ketat untuk pasien PPOK. Terapi fisik yang diarahkan protokol
(pdPT) ini akan dibandingkan dengan kelompok kontrol yang akan menerima pengobatan palsu, yang
berarti tidak ada atau latihan olahraga intensitas sangat rendah (ST). Evaluasi ekonomi akan dimasukkan
ke dalam RCT. Pengukuran antropometri, komorbiditas, merokok, kapasitas latihan fungsional, kekuatan
otot perifer, tingkat aktivitas fisik, kualitas hidup terkait kesehatan, manfaat yang dirasakan pasien,
kepatuhan terapi fisik, tingkat motivasi, tingkat pembersihan lendir yang efektif, gejala eksaserbasi dan
kontak perawatan kesehatan karena ke COPD akan direkam. Pengukuran tindak lanjut dijadwalkan pada
3 dan 6 minggu, 3, 6, 12 dan 24 bulan setelah inklusi. Diskusi: Cara meminimalkan potensi masalah
