SAFE WORK OBSERVATION

PURPOSE

Safe Work Observation (SWO) is a technique used to verify that work is performed efficiently and
in compliance with agreed HES standards. The purpose of this procedure is to identify guidelines
for carrying out such observations. This procedure will set down the approach to be used on this
project. It can be used in all occupations and their associated tasks.

SWO is a critical step in our drive toward Zero Incidents. It guides us through the steps of each job
and assists in the recognition of those actions that may cause an accident. Safe Work Observation
is a pro-active approach that permits us to take action prior to the event and thus avoid loss. It
increases employees’ HES awareness by involving suitably trained workplace personnel to
conduct the observations.

Complete job observations are to be carried out on all critical jobs in each area as the highest
priority. Changes in procedures or equipment are to be given the next priority, followed by general
procedures, instructions and Safe Work Procedures.

Pengamatan Kerja Selamat (SWO) adalah suatu teknik yang digunakan untuk memastikan
pekerjaan dilakukan secara efisien dan sesuai dengan standar HES. Tujuan prosedur ini adalah
untuk memperkenalkan petunjuk dalam melakukan pengamatan. Prosedur ini menuliskan
pendekatan untuk digunakan pada proyek ini. Ini dapat digunakan dalam semua pekerjaan dan
pekerjaan yang berhubungan.

SWO adalah suatu langkah kritis di dalam pengarahkan kita ke Zero incident. Petunjuk setiap
langkah-langkah dari masing-masing pekerjaan dan membantu dalam pengenalan tindakan yang
dapat menyebabkan kecelakaan. Safe Work Observation adalah suatu pendekatan pro aktif yang
memperbolehkan kita untuk mulai bertindak sebelum terjadi dan menghindari kerugian. Itu
meningkatkan kesadaran karyawan akan HES dengan menyertakan pelatihan personil di tempat
kerja untuk melakukan pengamatan.

Menyelesaikan pekerjaan pengamatan untuk melaksanakan semua pekerjaan yang kritis pada
setiap area sebagai prioritas yang paling utama. Merubah prosedur atau peralatan sebagai
prioritas berikutnya, yang diikuti oleh prosedur umum, instruksi dan prosedur kerja selamat.

SCOPE

The purpose of this procedure is to set out the requirements for Safe Work Observations.

It also lays out the time frame for work observations on the project, methods of recording the
observations, and follow up mechanisms.

Tujuan prosedur ini adalah untuk memperkenalkan ketentuan untuk Pengamatan Kerja Selamat.

Ini juga termasuk waktu untuk pengamatan kerja pada proyek, metoda pencatatan pengamatan,
dan mekanisme tindak lanjut.
APPLICATION

This procedure applies to both Tripatra-Fluor and Contractor personnel.

Prosedur ini berlaku bagi Tripatra-Fluor dan Kontraktor.

DEFINITIONS

1.0 GENERAL

Training for selected personnel will be undertaken on Safe Work Observations processes.

Two Safe Work Observations will be conducted by each trained HES person per week. Current
expectation is for one Safe Work Observation to be conducted per trained person (non TF HES)
per month.

Pelatihan untuk personil yang terpilih mengerjakan proses Pengamatan Kerja Selamat.

Dua (2) buah Pengamatan Kerja Selamat akan dilakukan oleh masing-masing pelatih HES setiap
minggunya. Harapan sekarang ini adalah untuk satu (1) Pengamatan Kerja Selamat untuk
dilakukan setiap orang yang ikut pelatihan (bukan TF HES) perbulannya.

2.0 RESPONSIBILITIES

2.1 Project/Construction Manager

The Project Manager is responsible for providing sufficient resources to comply with this procedure
and for assigning responsibility for Safe Work Observations to Line Management

mematuhi prosedur ini dan menugaskan orang yang bertanggung jawab untuk melakukan Safe
Work Observation ke taraf Manajemen.
2.2 Line Management

Line Management both Tripatra-Fluor and contractor are responsible to confirm:

 SWO checklists are produced for their area;
 The scope and frequency of the SWO’s for their area of responsibility meet project
requirements;
 Assign responsibility for SWO’s in their area of responsibility and verify they are carried out;

 Follow up actions are completed; and

 Review and feedback results of the SWO’s.

Tripatra-Fluor and contractor line management who have been trained in the process are
responsible for organizing and conducting a minimum of one (1) Safe Work Observations per
month.

mengkonfirmasikan:
 Checklist SWO yang dibuat untuk area mereka;
 Lingkup dan Frekwensi SWO's untuk area tanggung jawab mereka sesuai dengan ketentuan
proyek;
 Menugaskan orang yang tanggung jawab untuk SWO's di area tanggung jawab mereka dan
memastikan melaksanakannya;
 Menyelesaikan Tindakan lanjut; dan
 Hasil tinjauan ulang dan umpan balik dari SWO'S.

Tripatra-Fluor dan kontraktor dari taraf manajemen yang telah dilatih proses ini bertanggung jawab
untuk mengorganisir dan melaksanakan sedikitnya satu (1) Safe Work Observation per bulannya.
2.3 Employees

Each individual is responsible for participating and cooperating with line managers and HES
personnel undertaking Safe Work Observations.

Trained personnel shall conduct the Safe Work Observations and consult with Line Management to
establish appropriate corrective actions.

Masing-masing individu bertanggung jawab untuk mengambil bagian dan bekerja sama dengan
taraf manajer dan personil HES dalam melakukan Pengamatan Kerja Selamat.
Personil yang dilatih akan melakukan Pengamatan Kerja Selamat dan membahas dengan
manajemen untuk menetapkan tindakan korektif yang sesuai.

3.0 SAFE WORK OBSERVATION

Safe Work Observation is the formal procedure of observing a job or task from start to finish and
comparing it with a known standard. This is carried out by first obtaining the standard for the job
and becoming familiar with the required steps. Information that can be used as standards includes,
but is not limited to:
 Safe Work Procedures;
 Job Safety Analyses;
 Safe Operating procedures;
 Equipment Specifications;
 Manufacturers Manuals; and

 Company Policies and Procedures.

Furnished with the required standard and the Safe Work Observation Process Report (Attachment
1 - Safe Work Observation Process Report) the observers can then proceed with the job
observation. This should be carried out in an open and non-threatening manner and this is best
achieved by prior discussion with the person/s doing the job.

Using the Safe Work Observation Process Report the observers must fill in the time and date of
observation and the job being observed. The observers must then clearly describe the job to be
observed. While observing the job, notes should be taken on any deviations that require correction.

The checklist shown on Page 1 of Attachment 1 should be used as a basis for the observation.
Projects may revise this checklist to suit specific project requirements.

A Bahasa Indonesia version of this report is available as Attachment 2.

Observations should be conducted as follows:

 Observers introduce themselves to the person doing the job (unless already a part of the
work crew) and explain the observation process;
 Observers watch work activities and record what is observed.
Note!!! The process is a NO NAME – NO BLAME process. The process is fact finding,
NOT fault finding so there is no need to put employee names on the checklist;
 The observers will speak to the individuals who were observed at the end of the observation
process to provide positive feedback outlining safe work practices observed and bring to an
awareness level those activities that were observed that had a potential for injury; and
 Employees on the crew will then be given the opportunity to make suggestions for
improvement and identify barriers to working safely on the job.

The data collected while performing observations will be used to identify barriers to working safely
so that they can be addressed – whether it be procedure changes or purchasing different personal
protective equipment etc. the data will also be tracked as a pro-active measure of safe behavior on
the site. Reports and Safe Work Observation information will be shared with employees at Toolbox
Meetings.

When carrying out any SWO it is important to make this a worthwhile experience for both the
observers and the person carrying out the job. This can be achieved by following some basic rules.
These are as follows:
 Set the scene by explaining what you would like to do in an open, honest and friendly
manner;
 If you are given a genuine reason why you should not be doing this observation at that time,
arrange a time suitable to both parties;
 Focus on the problem, not the person. Don’t be critical of the person;
 When making written comments don’t use names or apportion blame;
 Keep questions simple and to the point;

 Seek verbal feedback from the person doing the job and listen to what is said;
 Verify understanding by asking questions until it is clear; and
 End on a positive note and try to remain enthusiastic.

A copy of the completed form must be forwarded to the TF Project HES Professional within 24
hours of completion of the report. Any person required to take action as a result of the observation
must also be issued a copy of the form.

Pengamatan Kerja Selamat adalah prosedur yang formal dalam pengamatan pekerjaan atau
pekerjaan dari awal sampai akhir dan membandingkannya dengan standar. Ini berlaku sebagai
standar pertama untuk pekerja dan sehingga terbiasa dengan langkah-langkah yang wajib.
Informasi yang dapat digunakan termasuk sebagai standard, tetapi tidaklah terbatas pada:
 Prosedur Kerja Selamat;
 Analisa Kerja Selamat;
 Prosedur Pengoperasian Aman;
 Spesifikasi Peralatan;
 Manual Pabrik; dan
 Kebijakan Perusahaan dan Prosedur.

Dilengkapi dengan standard yang diperlukan dan Laporan Proses Pengamatan Kerja Selamat
(Attachment 1 - Safe Work Observation Process Report) peninjau kemudian bisa meneruskan
pengamatan kerja itu. Ini harus dilaksanakan dengan pembicaraan yang bersifat terbuka dan tidak
dengan sikap mengancam dan ini akan lebih baik bila terlebih dulu berdiskusi terhadap orang
yang melakukan pekerjaan.

Peninjau dalam menggunaan Proses Laporan Pengamatan Pekerjaan Aman harus mengisi waktu
dan tanggal pengamatan dan pekerjaan diamati. Peninjau harus dengan jelas menguraikan
pekerjaan yang diamati. Ketika mengamati pekerjaan itu, catatan harus dilakukan pada setiap
penyimpangan yang memerlukan koreksi.

Checklist yang terdapat pada halaman 1 dari Attachment 1 harus digunakan sebagai basis dalam
pengamatan. Proyek dapat meninjau kembali daftar nama untuk disesuaikan dengan kebutuhan
proyek yang lebih spesifik.

Laporan Versi Bahasa Indonesia ini tersedia Attachment 2.

Pengamatan harus mengikuti sebagai berikut:

 Peninjau memperkenalkan diri mereka kepada orang melakukan pekerjaan (kecuali jika telah
bagian dari regu kerja) dan menjelaskan proses pengamatan;
 Peninjau mengamati aktivitas pekerjaan dan menulis apa yang diamati.
Catatan!!! Proses adalah TIDAK ADA NAMA- TIDAK ADA DISALAHKAN. Proses adalah
pencarian fakta, TIDAK menyalahkan temuan sehingga tidak perlu menaruh nama pada
daftar nama;

 Peninjau akan berkata kepada individu tersebut siapa yang telah diamati pada saat proses
pengamatan untuk memberikan umpan balik positif dengan menguraikan praktek kerja
selamat yang diamati dan memberikan tingkat kesadaran untuk aktivitas yang diamati
memiliki potensi untuk injury; dan
 Karyawan pada regu kerja akan diberi kesempatan untuk membuat usul untuk peningkatan
dan mengidentifikasi kendala bagi dalam bekerja dengan selamat.

Data dikumpulkan selagi melakukan pengamatan digunakan untuk mengidentifikasi kendala bagi
pekerja aman sehingga mereka dapat menentukan apakah perlu perubahan prosedur atau
pembelian alat pelindung diri yang berbeda dan lain-lain. Data akan juga digunakan sebagai
pengukuran pro-aktif dari perilaku selamat pada lokasi itu. Laporan dan Informasi SWO akan
diberikan bersama pada Toolbox Meeting karyawan.

Ketika melakukan SWO penting halnya untuk membuat pengalaman yang bermanfaat untuk
keduanya yakni peninjau dan orang yang menyelesaikan pekerjaan tersebut. Ini dapat dicapai
dengan mengikuti beberapa ketentuan dasar. Ini sebagai berikut:
 Membuat permulaan dengan menjelaskan apa yang kamu akan lakukan dengan
menggunakan pertanyaan terbuka, ramah dan jujur;
 Jika kamu diberi alasan yang sebenarnya mengapa kamu harus tidak melakukan pengamatan
pada waktu itu, susun waktu yang sesuai bagi kedua belah pihak;
 Pusatkan pada masalah, bukan orang itu. Jangan terlalu kritis pada orang tersebut;
 Ketika membuat komentar tertulis tidak menggunakan nama atau menyalahkan;
 Pertanyaan yang sederhana dan langsung;
 Lakukan umpan balik lisan dari orang melakukan pekerjaan dan mendengarkan apa yang
dikatakan;
 Memastikan mereka mengerti dengan menanyakan pertanyaan sampai jelas; dan
 Diakhiri dengan perkataan yang positif dan berusaha agar tetap bersemangat.

Salinan format yang diselesaikan harus diserahkan kepada TF Proyek HES Profesional dalam 24
jam penyelesaian laporan tersebut. Setiap orang yang memerlukan tindakan lanjut dari hasil
pengamatan juga harus pula memiliki salinan format.
