PATHOLOGY AND
PATHOGENESIS
■ Human leukocyte antigen (HLA)-B27 adalah faktor resiko
genetik utama utk ankylosing spondylitis (AS), arthritis
reaktif, psoriatik arthritis, spondiloarthropati yang
berhubungan dengan IBD, dan uveitis anterior akut.
■ HLA-B27 positif pada >90% pasien dgn AS, dan positif
pada 50-70% pasien dengan bentuk lain dari
spondiloarthritis
■ Sebaliknya, hanya 5-15% populasi umum yang memiliki
HLA-B27 positif. Fungsi imunologi dari HLA-B27adalah
utk mengikat peptida dari protein yang terdegradasi di
sitosol, dan memunculkannya pada permukaan sel, agar
dapat dikenali oleh sel T CD8+.
■ Bukti eksperimental bahwa HLA-B27 memainkan peran
langsung pada patofisiologi penyakit adalah dari
penelitian dengan mencit transgenik, dimana
overekspresi dari HLA- B27 menyebabkan inflamasi
spontan pada GI dan sendi2.
■ Sebanyak 20% keluarga dari pasien dgn AS yang HLA-
B27 positif akan mengalami Spondiloarthropati.
■ Penyakit ini berhubungan dengan frekuensi inflamasi
yang melibatkan entesis (entesis: tempat dimana
tendon dan ligamen berhubungan dengan tulang) dan
tulang-tulang aksial, dan sering ditemukan inflamasi
saluran cerna mikroskopik atau makroskopik, walaupun
pada pasien tanpa keluhan gejala saluran cerna
PERAN MIKROBA DAN SALURAN
CERNA
■ Beberapa patogen saluran cerna atau genitourinaria
telah diketahui sebagai pemicu arthritis reaktif yang
berhubungan dgn HLA-B27 yaitu Campylobacter,
Chlamydia, Salmonella dan Shigella.
■ Adanya produk bakterial di dalam sendi, menjelaskan
hubungan potensial antara infeksi saluran cerna dan
terjadinya inflamasi sendi pada arthritis reaktif
EKSPRESI SITOKIN PADA
ANKYLOSING SPONDYLITIS
■ Inflamasi yang terjadi pada AS dan spondiloartropati
telah diamati dengan melihat produksi sitokin
■ Produksi sitokin oleh antigen-presenting cells, seperti
makrofag dan sel dendritik, memegang peranan penting
dalam mengarahkan respon imun adaptif
■ Satu sistem pengenalan yg penting adalah bagian dari
Toll- like receptors (TLR), yang menginduksi produksi
sitokin seperti TNF-alpha dan IL-6
■ Sehingga, TLR bertindak sebagai persimpangan antara
respon imun innate dan respon imun adaptif, dimana
invasi mikroba diterjemahkan dari respon imun non
spesifik menjadi respon imun spesifik antigen. Hal ini
penting dalam patogenesis Spondiloartropati.
TREATMENT AND
ASSESSMENT
Pendahuluan
• Ankilosing spondylitis (AS) suatu bentuk penyakit
kronik pada tulang belakang yang dikenal sebagai
Spondiloathropati (SpA).
• Pasien datang dengan nyeri tulang belakang yang
bersifat inflamasi, dapat berkembang lebih lanjut lanjut,
menyebabkan menyatunya tulang belakang.
• AS dapat juga melibatkan sendi perifer, entheses, dan
struktur non artikuler (seperti pencernaan dan bilik
anterior mata)
• Semua manifestasi ini perlu diperhitungkan ketika
menilai dan memberikan terapi pada pasien
Pendahuluan
Treatment goal:
reducing symptoms
improving and maintaining spinal flexibility and normal
posture
reducing functional limitations
maintaining the ability to work
decreasing the complications associated with the disease
■ NSAIDs, including selective inhibitors of cyclo- oxygenase 2,
are the first-line drug treatment for pain and stiffness.
Continuous NSAID treatment is recommended for persistently
active, symptom- atic disease, with doses adjusted in
accordance with the severity of symptoms.
■ On-demand treat- ment with NSAIDs is acceptable when
continu- ous treatment causes unacceptable side effects and
is recommended for persons with stable spondyloarthritis. No
particular NSAID is pre- ferred in terms of efficacy. The risks
of cardiovas- cular, gastrointestinal, and renal effects should
be taken into account.
■ For patients whose symptoms are not con- trolled by NSAID
therapy or for whom NSAIDs have unacceptable side effects,
the use of TNF inhibitors is strongly recommended.
■ In 13 ran- domized, controlled trials and many open-label
studies, five TNF inhibitors — infliximab, etanercept,
adalimumab, golimumab, and certolizumab — have produced
rapid, profound, and sustained improvement in both objective
and subjective indicators of disease activity and patient func-
tioning
■ The predictors of a good response to TNF inhibitors include a
young age, a short disease duration, a high baseline level of
inflammatory markers, and a low baseline level of functional
disability, but patients at any dis- ease stage may benefit.
■ The presence of active infection or a high risk of infection,
advanced heart failure, lupus, multiple sclerosis, and cancer
are contraindications to treatment with TNF inhibitors.
■ Patients should be tested for the presence of latent or active
tuber- culosis. If either form of tuberculosis is present,
treatment must be started before the initiation of a TNF
inhibitor.
■ Carriers of the hepatitis B virus (HBV) surface antigen should
be treated prophylactically, and patients with antibodies to
the HBV core antigen should be monitored for reactivation.
■ Recent data support the cautious use of TNF inhibitors during
pregnancy
■ The long-term use of systemic glucocorticoids is relatively
contraindicated, partly because of the increased risk of
vertebral osteoporosis, but may be unavoidable in some
patients with severe uveitis or inflammatory bowel disease.
■ There is little evidence to indi- cate that methotrexate is
beneficial for patients with ankylosing spondylitis, even in
conjunction with TNF inhibitors.
