 Definisi Cost-Utility Analysis

Cost-Utility Analysis adalah teknik ekonomi untuk

menilai efisiensi dari intervensi pelayanan
kesehatan

Outcome : Klinik yang khusus yaitu

Quality-Adjusted Life Year (QALY)  penggabu
ngan kualitas (morbiditas) maupun kuantitas
(mortalitas) hidup
Tujuan Cost-Utility Analysis

Tujuan dari CUA adalah untuk memperkira

kan perbandingan antara suatu biaya interv
ensi yang berhubungan dengan kesehatan
dan menghasilkan keuntungan dalam hal
kualitas hidup dalam setahun oleh para pen
erima manfaat kesehatan
Pentingnya CUA

Pada bebrapa kasus :

Seseorang yg terkena penyakit Kanker Lama hidup
dan kualitas hidupnya berbeda tergantung pada
pilihan terapinya.
Kadang suatu terapi dapat memperpanjang kualitas
hidup, tapi bersifat toksik .
Maka pengukuran CUA ini digunakan :
untuk mengukur dengan menggabungkan baik lama
kehidupan maupun kualitas hidupnya
Kelebihan dan Kekurangan CUA

Kelebihan :
Tipe outcome kesehatan yang berbeda
dan penyakit dengan beberapa outcome
dapat dibandingkan dengan menggunak
an satu unit pengukuran  QALY

Kekurangan :
Kesulitan untuk menentukan utility atau
QALY secara tepat
Pengunaan CUA
1. Jika Health related quality of life (HRQoL) adalah outcome penting, contoh :
membandingkan intervensi yang tidak berpengaruh pada mortilitas, tetapi
berpengaruh pada fungsi dan kenyaman pasien (terapi osteoarthritis)
2. Jika HRQoL adalah outcome penting, contoh : (terapi infark miokard akut) 
outcome yang diukur tidak hanya keselamatan jiwa, tetapi kualitas hidup
yang terselamatkan
3. Jika intervensi mempengaruhi baik mortilitas maupun morbiditas, misal :
evaluasi terapi estrogen pada wanita post menopouse yang dapat meningkat
kan kualitas hidup,dapat menurunkan mortalitas yang disebabkan penyakit
lain (misal peny. Jantung), tetapi dapat meningkatkan mortilitas dari penyak
it lain (misalnya kanker)
4. Jika intervensi yang dibandingkan mempunyai outcome yang luas dan diperl
ukan satu unit outcome untuk dibandingkan.
5. Jika tujuannya adalah membandingkan intervensi yang dilakukan evaluasi
terhadap biasa per QALY yang diperoleh

Deskripsi harus menggambarkan pengaruh kesehatan yang diharapkan

dari suatu penyakit atau keadaan kesehatan dengan singkat. Meliputi
nyeri, rasa tidak nyaman, keterbatasan dalam beraktivitas, waktu yang
digunakan untuk terapi, perubahan yang mungkin pada persepsi
kesehatan (cemas atau keprihatinan ) dan perubahan mental

Contoh (dialis ginjal di RS) : Anda selalu merasa lelah dan letih. Sebua

subjek yang akan ditentukan utility :

1.QWB (Quality of Well-Being Scale

2.HUI (the Health Utilities Index)
3.the EuroQol Group’s EQ-5D
QMB Meliputi gejala atau masalah ditambah
tiga dimensi status kesehatan, yaitu mobilitas,
aktivitas fisik, dan aktivitas sosial

HUI Terdiri dari 6 dimensi, yaitu penglihatan,

pendengaran, bicara, berjalan, kecakapan, kognisi,
nyeri dan ketidak nyamanan, serta emosi

The EQ-5D
Bagian 1:
Terdiri dari mobilitas, perawatan diri, aktivitas,
nyeri/ketidaknyamanan, ansietas/depresi
Bagian 2:
Visual analog scale 20 cm dengan endpoint status
kesehatan paling baik dan status kesehatan paling
buruk

Pada 3 metode sistem klasifikasi kesehatan tersebut , istilah subbyek

digunakan untuk menerangkan seseorang yang akan diberikan pertanyaan
untuk menentukan utility, atau score preference.

Kelebihan nya : Bahwa pasien lebih memahami pengaruh suatu penyakit lebih
baik dibandingkan masyarakt umum.
Namun pasien dapat juga memberikan hasil bias jika pasien tidak dapat
menggambarkan utility. Misal : (anak’” atau orang dgn dementia)
shg dapat digantikan oleh pelayanan kesehatan .

Judul : Analisis Biaya Dan Nilai Utilitas Pasien Hemodialisa Yang Diberikan Terapi Sevelamer Karbonat

Tujuan : Untuk mengetahui gambaran biaya yang diperlukan untuk terapi sevelamer karbonat dan
nilai utilitas pasien hemodialisa yang menggunakan terapi sevelamer karbonat di RS UGM Yogyakarta.

Alternatif / komparator : Sesuai

Deskripsi dari alternatif : Pengukuran health related quality of life (HRQOL) pasien hemo
dialisa salah satunya dengan mengukur utilitas pasien hemodialisa yang menggunakan terapi
sevelamer dan CaCO3 sebagai kontrol dengan kuesioner EQ-5D-5L pada awal (pre) dan akhir
(post) penelitian.

Kesimpulan : Rata-rata biaya yang diperlukan pasien hemodialisa dengan sevelamer karbo
nat untuk menangani kondisi gangguan mineral dan tulang adalah sebesar Rp 2.999.188,75±1
67.920,75 per pasien selama 8 minggu terapi. Nilai utilitas dan skor EQ-5D VAS baseline
pasien hemodialisa yang diberikan sevelamer karbonat adalah masing-masing 0,91±0,13 dan
75,93±9,34, dan di akhir penelitian (posttes) terdapat perubahan nilai utilitas dan skor EQ-5D
VAS sebesar 0,93±0,08 dan 77,44±11,12. Hasil uji statistik menunjukkan bahwa perubahan nil
ai utilitas dan skor EQ-5D VAS pasien hemodialisa yang diberikan sevelamer karbonat antara
awal(pre) dan akhir penelitian (posttes) adalah menunjukkan tidak berbeda bermakna (p>0,05)
