DENGAN PENYAKIT ISPA

Pengertian ISPA
Infeksi Saluran Pernapasan Akut (ISPA) adalah penyakit infeksi yang
menyerang salah satu bagian atau lebih dari saluran napas, mulai dari
hidung (saluran atas) hingga alveoli (saluran bawah) termasuk jaringan
andeksanya, seperti sinus, rongga telinga tengah, dan pleura. ISPA
merupakan infeksi saluran pernapasan yang berlangsung selama 14 hari.
Infeksi Saluran Pernapasan Akut (ISPA) merupakan penyakit yang banyak
dijumpai pada balita dan anak-anak mulai dari ISPA ringan sampai berat.
 Etiologi
Proses terjadinya ISPA diawali dengan masuknya beberapa bakteri dari genus
streptokokus, stafilokokus, pneumokokus, hemofillus, bordetella, dan
korinebakterium dan virus dari golongan mikrovirus (termasuk didalamnya
virus para influenza dan virus campak), adenoveirus, koronavirus, pikornavirus,
herpesvirus ke dalam tubuh manusia melalui partikel udara (droplet infection).
Kuman ini akan melekat pada sel epitel hidung dengan mengikuti proses
pernapasan maka kuman tersebut bisa masuk ke bronkus dan masuk ke saluran
pernapasan yang mengakibatkan demam, batuk, pilek, sakit kepala dan
sebagainya. (Marni,2014).
Manifestasi Klinis
Gejala ISPA berdasarkan tingkat keparahan adalah sebagai berikut
Rosana (2016):

1. Gejala dari ISPA ringan Seseorang balita dinyatakan menderita

ISPA ringan jika ditemukan satu atau lebih gejala-gejala seperti
Batuk, Serak, yaitu anak bersuara parau pada waktu
mengeluarkan suara (pada waktu berbicara atau menangis), Pilek,
yaitu mengeluarkan lendir atau ingus dari hidung. • Panas atau
demam, suhu badan lebih dari 37°C atau jika dahi anak diraba
dengan punggung tangan terasa panas.
2. Gejala dari ISPA sedang Seseorang balita dinyatakan menderita ISPA sedang jika dijumpai gejala dari ISPA ringan
disertai satu atau lebih gejala-gejala sebagai berikut :
• Pernapasan cepat (fast breathing) sesuai umur yaitu: untuk kelompok umur kurang dari 2 bulan frekuensi nafas 60
kali per menit atau lebih untuk umur 2 -< 5 tahun.
• Suhu tubuh lebih dari 39°C.
• Tenggorokan berwarna merah.
• Timbul bercak-bercak merah pada kulit menyerupai bercak campak.
• Telinga sakit atau mengeluarkan nanah dari lubang telinga.
• Pernapasan berbunyi seperti mengorok (mendengkur).

3. Gejala dari ISPA berat Seseorang balita dinyatakan menderita ISPA berat jika dijumpai gejala-gejala ISPA ringan atau
ISPA sedang disertai satu atau lebih gejala-gejala sebagai berikut :
• Bibir atau kulit membiru
• Anak tidak sadar atau kesadaran menurun.
• Pernapasan berbunyi seperti mengorok dan anak tampak gelisah.
• Sela iga tertarik ke dalam pada waktu bernafas.
• Nadi cepat lebih dari 160 kali per menit atau tidak teraba.
• Tenggorokan berwarna merah
 Patofisiologi
Patofisiologi ISPA Perjalanan klinis penyakit ISPA dimulai dengan
berinteraksinya virus dengan tubuh. Masuknya virus sebagai
antigen ke saluran pernafasan menyebabkan silia yang terdapat pada
permukaan saluran nafas bergerak ke atas mendorong virus ke arah
faring atau dengan suatu tangkapan refleks spasmus oleh laring.Jika
refleks tersebut gagal maka virus merusak lapisan epitel dan lapisan
mukosa saluran pernafasan dan menyebabkan Iritasi virus pada
kedua lapisan tersebut menyebabkan timbulnya batuk kering
(Jeliffe, 1974). Kerusakan stuktur lapisan dinding saluran
pernafasan menyebabkan kenaikan aktifitas kelenjar mukus yang
banyak terdapat pada dinding saluran nafas, sehingga terjadi
pengeluaran cairan mukosa yang melebihi noramal. Rangsangan
cairan yang berlebihan tersebut menimbulkan gejala batuk
(Kending and Chernick, 1983).
 Perjalanan klinis penyakit ISPA ini dapat dibagi menjadi empat tahap, yaitu:

menunjukkan reaksi apa-apa.
Tahap dini penyakit, dimulai dari

penyakit.Timbul gejala demam
dan batuk.
Tahap inkubasi, virus merusak

lapisan epitel dan lapisan mukosa.
Tubuh menjadi lemah apalagi bila
keadaan gizi dan daya tahan
sebelumnya memang sudah
rendah.
04 Tahap lanjut penyakit, dibagi
menjadi empat, yaitu dapat
sembuh sempurna, sembuh
dengan ateletaksis, menjadi kronis
dan dapat meninggal akibat
pneumonia.
 Menurut Saputro (2013), Pemeriksaan
penunjang yang lazim dilakukan adalah:

Pemeriksaan 1. Pemeriksaan kultur/biakan kuman (swab) :

hasil yang didapatkan adalah biakan kuman
(+) sesuai jenis kuman

Penunjang 2. Pemeriksaan hidung darah (deferential

count) : laju endap darah meningkat disertai
dengan adanya leukositosis dan bisa juga
disertai dengan adanya thrombositopenia.
3. Pemeriksaan Foto thorak dapat
mengungkapkan adanya pembesaran
jantung, edema atau efusi pleura yang
menegaskan diagnose CHF.
 Pencegahan
Menurut Hastuti, D (2013) pencegahan ISPA dapat
dilakukan dengan :
• Menyediakan makanan bergizi sesuai preferensi
anak dan kemampuan untuk mengkonsumsi
makanan untuk mendukung kekebalan tubuh alami.
• Pemberian imunisasi lengkap kepada anak
• Keadaan fisik rumah yang baik, seperti: ventilasi
dirumah dan kelembaban yang memenuhi syarat.
• Menjaga kebersihan rumah, tubuh, makanan, dan
lingkungan agar bebas kuman penyakit.
• Menghindari asap rokok, asap dapur.
• Mencegah kontak dengan penderita ISPA dan isolasi
penderita ISPA untuk mencegah penyebaran penyaki
Penataaksanaan
Menurut Kepmenkes RI (2011), Terapi untuk ISPA
tidak selalu dengan antibiotik karena sebagian besar
kasus ISPA disebabkan oleh virus. Infeksi Saluran
Pernapasan Akut (ISPA) atas yang disebabkan oleh
virus tidak memerlukan antiviral, tetapi cukup dengan
terapi suportif.
• Terapi Suportif Berguna untuk mengurangi gejala dan
meningkatkan performa pasien berupa nutrisi yang
adekuat, pemberian multivitamin.
• Antibiotik Hanya digunakan untuk terapi penyakit
infeksi yang disebabkan oleh bakteri, idealnya
berdasarkan jenis kuman penyebab, utama ditujukan
pada pneumonia, influenza, dan aureus.
 Asuhan keperawatan
1.Data Demografi wilayah sekolah berada di tengah tengah peukiman.
2. Data Lingkungan Fisik wilayah sekolah yang cukup dekat dengan
jalan yang banyak kedaraaan lalu lalang yang lokasi sekolah tersebut
deekat dengan pabrik penggilingan padi, yang mnegakibatkan banyak
apolusi udara.
3. Kondisi Kesehatan Umum Dari pernyataan kader di puskesmas
skitar banyak warga juga tekena pneumnia, dan ISPA, krena
banyaknya yang merokokdi kawasan komunitas tersebut, dan kader
juga mengataka banyak anak anak jga sudah mulai merokok.
4. Pengkajian dengan Questioner ataupun Observasi Hasil pengkajian
dengan lembar observasi diperoleh data sebanyak 15% anak ada
menderita ISPA di SDN X.
