2.1 Definisi
Penyakit Perthes adalah gangguan pada kaput femur yang terdiri dari
proses nekrosis, kolaps, perbaikan dan remodeling yang muncul pada dekade
pertama kehidupan, khususnya pada anak laki-laki. Penyebab dari penyakit ini
dihipotesiskan sebagai satu atau beberapa kejadian vaskular yang diikuti oleh
revaskularisasi. Selanjutnya hal ini dapat mengakibatkan kesulitan dan nyeri pada
bagian pinggul. Penyakit Perthes pertama kali dipublikasikan secara independen
oleh Jacques Calv, Arthur T. Legg and Georg Perthes pada tahun 1910.
2.2 Epidemiologi
2.3 Etiologi
Meskipun etiologi dari penyakit ini belum jelas, keadaan patologis yang
ada disebabkan oleh nekrosis avaskular dari epifisis kaput femoralis. Proses infark
dan perbaikan selanjutnya bertanggung jawab terhadap sindrom yang dikenal
sebagai Legg-Calve-Perthes (buku ajar bedah).
Kaput femur bergantung pada pembuluh epifisis lateral untuk suplai darah
saat usia 4 sampai 7 tahun. Penyebab dari infark kaput femur masih kontroversial
dan bisa dikarenakan oleh arteri itu sendiri oleh karena trombosis venosus. Suplai
darah arteri pada sisi yang terkena dapat melemah, dengan obstruksi pada arteri
kapsular superior atau arteri medial sirkumflex. Dalam pemberian kontras vena
pada kaput femur merupakan kaliber medium, sama dengan vena kutaneus atau
vena serebral. Hipertensi vena telah ditemukan pada pasien dengan penyakit
Perthes. Meskipun demikian, masih belum dapat dipastikan apakah trombosis
merupakan hal utama atau hanya berkontribusi dalam penyakit ini sebagai
kombinasi dari etiologi lainnya. Anatomi vaskular pada anak laki-laki usia antara 4
samapi 8 tahun membuat mereka lebih rentan terhadap keadaan hipoerkoagulabel
dan faktor-faktor lainnya. Kejadian protrombotik pada keadaan hiperkoagulabel
dapat mengakibatkan trombosis dan infark dari kaput femur.
Penyakit Perthes lebih sering terjadi pada pasien dengan displasia skeletal.
Terdapat hubungan antara penyakit Perthes dan ADHD. Perokok pasif atau riwayat
ibu yang merokok selama masa kehamilan dapat menjadi faktor yang
berkontribusi pada penyakit ini. Penyakit Perthes adalah kondisi non traumatik,
meskipun demikian riwayat trauma minor dapat ditemukan pula.

Vascular thrombosis is uncommon at a young age but may
occur due to a genetic defect, [22] such as resistance to activated
protein C. Protein C is a vitamin K-dependent prothrombotic
protein that leads to curtailment of procoagulant enzymes, factors
Xa and thrombin, via factors V and VIII. [30] Factor V Leiden is
implicated in the prothrombotic process by virtue of a substitution
that blocks the binding of activated protein C to factor V. [31] [32]
It is not clear whether the deficiency is due to conversion or
resistance to the activated form. However, protein C deficiency
causes thrombosis in medium-calibre veins resulting in bone and

2.5 Patofisiologi
Children with Perthes' Disease have been found to exhibit growth delay
affecting both their skeletal maturity as well as their anthropometric status.
Research in Liverpool has also shown that the feet are small in the sufferer when
compared to their peer group. Other growth paramenters ehibit a tendency for
smallness when compared to normal unaffectd children

Penyakit Perthes melalui 4 tahap dalam kurun waktu 2 sampai 4 tahun sejak awal
kemunculannya, yaitu:
2.4 Pemeriksaan
The age range can vary from 2 to 12 years and can also

of incongruity at the end of the natural history and worse prognosis.

2.7 Prognosis
The strongest predictor of outcome was femoral head involvement of more or less
than 50% (odds ratio (OR) = 7.76, 95% confidence interval (CI) 2.82 to 21.37),
followed by age at diagnosis (OR = 0.98, 95% CI 0.92 to 0.99) and the lateral
pillar classification (OR = 0.62, 95% CI 0.40 to 0.98). In children over six years at
diagnosis with more than 50% of femoral head necrosis, proximal femoral varus
osteotomy gave a significantly better outcome than orthosis (p = 0.001) or
physiotherapy (p = 0.001). There was no significant difference between the
physiotherapy and orthosis groups (p = 0.36), and we found no difference in
outcome after any of the treatments in children under six years (p = 0.73).

KESIMPULAN
Perthes merupakan suatu self limiting disease pada kaput femur yang
terdiri dari nekrosis, kolaps, perbaikan dan remodeling yang terjadi pada decade
pertama kehidupan, khususnya lebih sering terjadi pada anak laki-laki. Pasien
biasanya memiliki fenotip karakteristik perawakan pendek, usia tulang tertunda,
dan hiperaktivitas. Perthes biasanya bersifat unilateral, kecuali pada 10-20 persen
kasus yag ditemui. Gejala klinis meliputi gaya berjalan pincang dan nyeri pinggul
sering menjalar ke paha, lutut, pangkal paha, pantat atau perut. Nyeri akan
semakin memburuk dengan aktivitas. Penatalaksanaan yang dilakukan bergantung
pada usia pasien. Pilihan yang ada meliputi termasuk mobilisasi dan pemantauan,
penahanan tanpa pembedahan atau operasi, serta prosedur penyelamatan.
