Peran Ion Ca2+ dalam Mekanisme Penghantaran Impuls
Dalam mekanisme penghantaran impuls syaraf, impuls berjalan melalui satu neuron dengan neuron lainnya dalam bentuk aliran listrik. Namun, impuls tidak berjalan secara terus menerus diantara 2 neuron disebabkan karena adanya celah antara neuron yang disebut dengan celah sinaps. Untuk memungkinan agar impuls saraf tetap dapat berjalan, terdapat mekanisme yang disebut Neurotransmisi, yang terjadi dengan tujuan menghantarkan molekul neurotransmitter dari neuron pre-sinaptik yang kemudian akan menginisiasi impuls saraf di neuron post-sinaptik.1 Sejauh ini terdapat 3 mekanisme Neurotransmisi yaitu Synchronous, Asynchronous dan Spontaneous, yang semuanya menggunakan prinsip eksositosis.1 Dari ketiga mekanisme di atas model Synchronous sudah terkarakterisasi dengan baik dan memiliki kaitan erat dengan dampak kenaikan konsentrasi internal ion Ca2+ pada neuron. Model synchronous juga mendominasi kejadian neurotransmisi di tubuh manusia dengan persentase >90%.2 Berikut adalah gambaran singkat dari proses neurotransmisi dengan metode synchronous. Neurotransmitter yang akan terlibat dalam mekanisme neurotransmisi akan dikirimkan melalui vesikel transport. Proses Neurotransmisi umumnya diinduksi oleh influks ion Ca2+ akibat adanya peristiwa depolarisasi pada terminal akson. Sebelum dilepaskan, vesikel transport yang berisi neurotransmitter akan mengalami proses perakitan dan pengisian yang kemudian akan dilanjutkan dengan proses yang berlangsung di Active Zone dari terminal sinaps. Proses ini disebut dengan Docking & Priming. Dari perakitan hingga menuju ke proses Docking & Priming, vesikel neurotransmitter mengalami mobilisasi/translokasi.3 Mobilisasi dari vesikel pre-sinaptik yang berisi Neurotransmitter menempatkan vesikel pada lokasi yang siap tanggap terhadap perubahan kondisi fisiologi neuron selanjutnya. Depolarisasi neuron, akan memicu pembukaan kanal ion Ca2+ yang menyebabkan peningkatan drastis dari konsentrasi ion Ca2+ dalam sel. Hal ini kemudian ditanggapi oleh protein permukaan vesikel pre-sinaptik dengan melakukan perlekatan ke kompleks protein yang ada pada perumukaan internal dari membrane plasma neuoron.3 Perlekatan dari vesikel pre-sinaptik bertujuan untuk membentuk cincin fusi, yang akan menjadi jalan bagi pelepasan Neurotransmitter ke celah sinaps. Setelah pengosogan vesikel maka vesikel akan melepaskan diri melalui mekanisme endositosis. Vesikel kemudian akan didaur ulang dan siklus akan berlanjut kembali apabila rangsangan yang menghasilkan impuls terus terjadi.3 Berikut adalah penjelasan rinci dari mekanisme synchronous dalam kejadian nerotransmisi. a. Sintesis Vesikel dan Pemuatan Neurotransmitter Pada mulanya vesikel pre-sinaptik datang ke terminal akson dari badan sel dalam bentuk prekursor. Pada tahapan ini vesikel berada pada area reserve pool.3 Selanjutnya vesikel akan melalui tahapan pengisian neurotransmitter / neurotransmitter uptake. Mekanisme neurotransmitter uptake menggunakan prinsip transpor aktif / penggunaan ATP yang dibantu dengan gradien elektrokimiawi yang diinduksi oleh pompa proton.4 b. Mobilisasi Vesikel Vesikel yang telah berisi kemudian akan dipindahkan ke area Active Zone. Pada area ini terdapat komponen protein permukaan internal membran neuron berupa Syntaxin-1 dan Synaptosomal- associated Protein (SNAP) 25. Protein-protein tadi akan berfungsi sebagai penambat vesikel saat proses perlekatan nanti. Active Zone merupakan lokasi ditemukannya kanal ion Ca2+ yang menjadikan lokasi ini sangat aktif pada saat depolarisasi nanti.3 Proses mobilisasi / translokasi dari vesikel memiliki kaitan erat dengan mikrotubulus serta diinisisiasi oleh konsentrasi ion Ca2+ yang semula telah lebih dahulu berada dalam sel.5 c. Priming dan Docking Vesikel Sebelum terjadinya depolarisasi neuron, vesikel pre-sinaptik terlebih dahulu telah disiapkan di area Active Zone dengan kondisi telah tertambat ke protein-protein permukaan dari mebran internal neuron melalui priming & docking. Sebelum priming & docking terjadi, protein plasma Munc18 akan berikatan dengan Syntaxin-1. Kesatuan anatara Munc18 dan Syntaxin-1 akan mengikat SNAP- 25. Pada kompleks protein yang sudah terbentuk, kemudian akan menarik Munc13 dan protein Active Zone yang akan menyelesaikan pembentukan area penambatan.3 Proses docking adalah ketika vesikel pre-sinaptik mendekati Active Zone, sedangkan pada proses priming terjadi perlekatan antara membrane vesikel dengan mebran neuron akibat adanya penambatan protein permukaan. Synaptobrevin-2 yang ada pada permuakaan vesikel pre-sinaptik akan berikatan dengan kompleks Syntaxin-1 dan SNAP-25 membentuk kompleks Soluble N- ethylamalemide-sensitive factor activating receptor (SNARE) protein. Protein lain yang berada pada permukaan vesikel adalah Synaptotagmin-1 yang bertindak sebagai detectrot ion Ca2+ akan berikatan ke phosphatidylinositol (4,5) bisphosphate (PI(4,5)P2) / PIP2.3,6 d. Pembentukan Cincin Fusi Vesikel Saat terjadi depolarisasi neuron, maka kanal ion Ca2+ akan membuka dan menaikan konsentrasi ion Ca2+. Synaptotagmin-1 akan mengikat 5 ion Ca2+ (3 pada domain C2A dan 2 pada domain C2B). Pengikatan ion Ca2+ menjamin kecepatan dari fusi vesikel dan juga menginduksi perputaran kompleks protein SNARE dalam membentuk cincin fusi. Proses ini juga didukung oleh protein Complexin yang berlekatan ke kompleks SNARE.3 Pembentukan dari cincin fusi terjadi dalam proses bertahap dimulai dari pembentukan stalk, hemifusion hingga akhirnya menjadi cincin / pori fusi yang dapat dilewati oleh neurotransmitter. Fusi yang dimaksud disini adalah yang terjadi antara membran ganda vesikel pre-sinaptik dan membrane sel neuron.7 Konsentrasi ion Ca2+ yang dibutuhkan untuk memicu fusi sel dalam eksositosis memiliki kisaran yang bervariaasi pada beragam jenis sel. Untuk gambaran ada sel yang membutuhkan 510 M ion Ca2+ untuk memicu eksositosis, namun adapula sel yang membutuhkan hingga 100200 M ion Ca2+.8 e. Pelepasan Vesikel Vesikel tidak selamnya tertambat pada membrane sel. Setelah melakukan pelepasan neurotransmitter, vesikel bersama dengan synptobrevin akan mengalami endositosis. Endositosi akan mendaur ulang vesikel dan protein agar dapat digunakan dalam siklus selanjutnya. Kemudian kompleks protein SNARE akan dirombak oleh N-ethylmaleimide sensitive fusion (NSF) protein dengan menggunakan transport aktif.9 A. The role of Ca2+ Ion on Nerve Impulse Transmission In the mechanism of conducting nerve impulses, the impulse travels through one neuron with another in the form of electricity. However, impulses do not run continuously between 2 neurons caused by a gap between neurons called synaptic cleft. To enable the nerve impulse to proceed, there is a mechanism called Neurotransmission, which occurs with the goal of delivering neurotransmitter molecules from pre-synaptic neurons that will then initiate nerve impulses in post-synaptic neurons.1 So far there are three mechanisms of Neurotransmission: Synchronous, Asynchronous and Spontaneous, all of which use the principle of exocytosis.1 Of the three mechanisms above the Synchronous model is well characterized and closely related to the effect of increasing the internal concentration of Ca2+ ions in neurons. The synchronous model also dominates the incidence of neurotransmission in the human body by a percentage of> 90% .2 Here is a brief overview of the neurotransmission process by the synchronous method. The neurotransmitters involved in the neurotransmission mechanism will be transported through the transport vesicles. The process of neurotransmission is generally induced by Ca2+ ion influx due to depolarization events in the axon terminal. Prior to release, transport vesicles containing the neurotransmitter will undergo assembly and filling process which will then proceed with a process that takes place in the Active Zone of the synapse terminal. This process is called Docking & Priming. From assembly to the process of Docking & Priming, neurotransmitter vesicles are mobilized / translocated.3 Mobilization of the pre-synaptic vesicles containing the Neurotransmitter puts the vesicles at a location ready to respond to the further changes in the physiological state of the neuron. Depolarization of neurons, will trigger the opening of Ca2+ ion channels which causes a drastic increase of the concentration of Ca2+ ions in the cell. This is then responded by the surface protein of pre-synaptic vesicles by attaching to the protein complex present in the internal enclosure of the neuron plasma membrane.3 The attachment of the pre-synaptic vesicles aims to form a fusion ring, which will be the path for the release of the Neurotransmitter into the synaptic cleft. After vesicle being emptied then vesicles will be detached through the mechanism of endocytosis. The vesicles will then be recycled and the cycle will resume when impulses that produce impulses continue to occur.3 Here is a detailed explanation of the synchronous mechanism in the event of neurotransmission. a. Vesicle Synthesis and Neurotransmitter Uptake At first the pre-synaptic vesicles come to the axon terminal of the cell body in the form of precursors. At this stage the vesicle is in the reserve pool area.3 Next the vesicle goes through the neurotransmitter / neurotransmitter uptake stage. The uptake neurotransmitter mechanism uses the principle of active transport / ATP use that is aided by the electrochemical gradient induced by the proton pump.4 b. Vesicle Translocation The contained vesicles will then be moved to the Active Zone area. In this area there is an internal surface protein component of a neuron membrane in the form of Syntaxin-1 and Synaptosomal-associated Protein (SNAP) 25. These proteins will act as vesicle retarders during the later attachment process. Active Zone is the location of the discovery of Ca2+ ion canal that makes this location very active at the time of depolarization later.3 The process of mobilization / translocation of vesicles has a close connection with microtubules and is initiated by the concentration of Ca2+ ions which had previously been in the cell.5 c. Vesicle Docking and Priming Before the onset of neuronal depolarization, pre-synaptic vesicles have been prepared first in the Active Zone with conditions already tethered to surface proteins from the internal neuron through priming & docking. Before priming & docking occurs, the Munc18 plasma protein will bind to Syntaxin-1. Unity between Munc18 and Syntaxin-1 will bind SNAP-25. In the already established protein complex, it will attract Munc13 and the Active Zone protein which will complete the formation of the tethering area.3 The docking process is when the pre-synaptic vesicles approach the Active Zone, whereas in the priming process there is attachment between the vesicle membrane and the neuron mebrane due to the surface proteins. Synaptobrevin-2 present in the pre- synaptic vesicle will bind to the Syntaxin-1 and SNAP-25 complexes to form the Soluble N-ethylamalemide-Sensitive Factor Activating Receptor (SNARE). Another protein on the surface of the vesicle is Synaptotagmin-1 which acts as a Ca2+ ion detectrot to bind to phosphatidylinositol (4,5) bisphosphate (PI (4,5) P2) / PIP2.3,6 d. Formation of Vesicle Fusion Ring When depolarization of neurons occurs, the Ca2+ ion channel will open and raise the concentration of Ca2+ ions. Synaptotagmin-1 will bind 5 Ca2+ ions (3 on C2A and 2 domains on the C2B domain). The binding of Ca2+ ions ensures the velocity of the vesicle fusion and also induces the rotation of the SNARE protein complex in forming the fusion ring. This process is also supported by the Complexin protein attached to the SNARE complex.3 The formation of the fusion ring occurs in a gradual process starting from the formation of stalk, hemifusion until it eventually becomes a fusion ring / pore that can be passed by neurotransmitters. The fusion referred to herein is what occurs between the double membrane of the pre-synaptic vesicle and the neuron cell membrane.7 The concentration of Ca2+ ions required to trigger cell fusion in exocytosis has a range that varies across different cell types. For illustration there are cells that require 5-10 M Ca2+ ions to trigger exocytosis, but those that require up to 100-200 M Ca2+ ions.8 e. Vesicle Detachment The vesicles are not immersed in the cell membrane. After the release of neurotransmitters, vesicles together with synptobrevin-2 will be endocytosized. Endocytosis will recycle vesicles and proteins in order to be used in the next cycle.
