Adneksa Kulit
1. Kelenjar Kulit => terdapat pada lapisan dermis
1o Kelenjar Keringat (glandula sudorifera)
Keringat mengandung air, elektrolit, asam laktat, dan glukosa. pH nya sekitar 4-6,8.
1 Kelenjar Ekrin => kecil-kecil, terletak dangkal di dermis dengan secret encer.
Kelenjar Ekrin terbentuk sempurna pada minggu ke 28 kehamilan dan
berfungsi 40 minggu setelah kelahiran. Salurannya berbentuk spiral dan
bermuara langsung pada kulit dan terbanyak pada telapak tangan, kaki,
dahi, dan aksila. Sekresi tergantung beberapa faktor dan saraf kolinergik,
faktor panas, stress emosional.
2 Kelenjar Apokrin => lebih besar, terletak lebih dalam, secretnya lebih kental.
Dipengaruhi oleh saraf adrenergik, terdapat di aksila, aerola mammae,
pubis, labia minora, saluran telinga. Fungsinya belum diketahui, waktu lahir
ukurannya kecil, saat dewasa menjadi lebih besar dan mengeluarkan secret
2o Kelenjar Palit (glandula sebasea)
Terletak di seluruh permukaan kuli manusia kecuali telapak tangan dan kaki. Disebut
juga dengan kelenjar holokrin karena tidak berlumen dan sekret kelenjar ini berasal
dari dekomposisi sel-sel kelenjar. Kelenjar palit biasanya terdapat di samping akar
rambut dan muaranya terdapat pada lumen akar rambut (folikel rambut). Sebum
mengandung trigliserida, asam lemak bebas, skualen, wax ester, dan kolesterol.
Sekresi dipengaruhi oleh hormon androgen. Pada anak-anak, jumlahnya sedikit.
Pada dewasa menjadi lebih banyak dan berfungsi secara aktif.
2. Kuku => bagian terminal lapisan tanduk (stratum korneum) yang menebal. Pertumbuhannya
1mm per minggu.
1o Nail root (akar kuku) => bagian kuku yang tertanam dalam kulit jari
2o Nail Plate (badan kuku) => bagian kuku yang terbuka/ bebas.
3o Nail Groove (alur kuku) => sisi kuku yang mencekung membentuk alur kuku
4o Eponikium => kulit tipis yang menutup kuku di bagian proksimal
5o Hiponikium => kulit yang ditutupi bagian kuku yang bebas
3. Rambut
1o Akar rambut => bagian yang terbenam dalam kulit
2o Batang rambut => bagian yang berada di luar kulit
Jenis rambut
3o Lanugo => rambut halus pada bayi, tidak mengandung pigmen.
4o Rambut terminal => rambut yang lebih kasar dengan banyak pigmen, mempunyai
medula, terdapat pada orang dewasa.
Pada dewasa, selain di kepala, terdapat juga bulu mata, rambut ketiak, rambut kemaluan,
kumis, janggut yang pertumbuhannya dipengaruhi oleh androgen (hormon seks). Rambut
halus di dahi dan badan lain disebut rambut velus.
Rambut tumbuh secara siklik, fase anagen (pertumbuhan) b erlangsung 2-6 tahun dengan
kecepatan tumbuh 0,35 mm perhari. Fase telogen (istirahat) berlangsung beberapa bulan. D
antara kedua fase tersebut terdapat fase katagen (involusi temporer). Pada suatu saat 85%
rambut mengalami fase anagen dan 15 % sisanya dalam fase telogen.
Rambut normal dan sehat berkilat, elastis, tidak mudah patah, dan elastis. Rambut mudah
dibentuk dengan memperngaruhi gugusan disulfida misalnya dengan panas atau bahan
kimia.
Hydrostatic pressure causes the area of the separation to fill with a fluid that is similar in
composition to plasma but has a lower protein level. The blister roof is composed of
intact stratum corneum and stratum granulosum with normal, necrotic and degenerated
prickle cells on both sides of the split. The basal cell layer usually shows little insult and

Systemic response
The release of cytokines and other inflammatory mediators at the site of injury has a
systemic effect once the burn reaches 30% of total body surface area.
Cardiovascular changes—Capillary permeability is increased, leading to loss of
intravascular proteins and fluids into the interstitial compartment. Peripheral and
splanchnic vasoconstriction occurs. Myocardial contractility is decreased, possibly due to
release of tumour necrosis factor. These changes, coupled with fluid loss from the burn
wound, result in systemic hypotension and end organ hypoperfusion.
Respiratory changes—Inflammatory mediators cause bronchoconstriction, and in severe
burns adult respiratory distress syndrome can occur.
Metabolic changes—The basal metabolic rate increases up to three times its orig inal
rate. This, coupled with splanchnic hypoperfusion, necessitates early and aggressive
enteral feeding to decrease catabolism and maintain gut integrity.
Immunological changes—Non-specific down regulation of the immune response occurs,
affecting both cell mediated and humoral pathways

Pathophysiology
The entity rhabdomyolysis associated acute renal failure (ARF) was well known from
World War I. However, the underlying pathophysiology was first explained by an
experimental model of Bywaters and Stead. Three basic mechanisms underlie
myoglobin induced nephrotoxicity:
1. Renal vasoconstriction.
2. Direct heme protein induced cytotoxicity.
3. Intraluminal cast formation and tubular obstruction
Renal Vasoconstriction
First, severe muscle necrosis in crush injuries cause fluid accumulation in third space
and resultant intravascular fluid (IVF) depletion and aggressive volume repletion during
early postinjury period dramatically decreases kidney injury. This IV hypovolemia results
in activation of renin-angiotensin system, vasopressin and sympathetic nervous system
and further aggravates vasoconstriction.
Second, severe muscle injury generates endotoxins like endothelin-1, thromboxane A2,
TNF-α and they activate endotoxin cytokine cascade resulting in renal vasoconstriction.
Third, nitric oxide (NO), a potent endogenous vasodilator is scavenged by the heme
proteins myoglobin.[10,11] In the setting of myoglobinemia, renal hypoperfusion is
exacerbated because myoglobin maintains mean arterial pressure during volume
depletion states by NO scavenging and increasing peripheral vascular resistance. Thus,
hypovolemia could not be detected clinically and ischemic tissue injury is aggravated.
Moreover, renal vasoconstriction can facilitate heme toxicity by decreasing GFR and
prolonging their circulating halflife, promoting proximal tubular uptake and increasing
the
propensity for cast formation. IV volume depletion stimulates fluid reabsorption in
tubules and increases intraluminal myoglobin concentration, favors cast deposition, and
tubular obstruction.

Larutan kristaloid adalah larutan air dengan elektrolit dan atau dextrosa, tidak
mengandung molekul besar (Adewale, 2009). Kristaloid dalam waktu singkat sebagian
besar akan keluar dari intravaskular, sehingga volume yang diberikan harus lebih banyak
(2,5-4 kali) dari volume darah yang hilang (Rudra et all, 2006). Kristaloid mempunyai
waktu paruh intravaskuler 20-30 menit. Ekspansi cairan dari ruang intravaskuler ke
interstital berlangsung selama 30-60 menit sesudah infus dan akan keluar dalam 24-48
jam sebagai urine (Mulyono, 2006).Secara umum kristaloid digunakan untuk
meningkatkan volume ekstrasel dengan atau tanpa peningkatan volume intrasel
(Tonessen AS., 1990). Macam-macam cairan kristaloid antara lain (Tonessen AS., 1990):
Cairan kristaloid cukup baik untuk terapi syok hipovolemik. Keuntungan cairan kristaloid
antara lain mudah tersedia, murah, mudah dipakai, tidak menyebabkan reaksi alergi dan
sedikit efek samping. Kelebihan cairan kristaloid pada pemberian dapat berlanjut
dengan edema seluruh tubuh sehingga pemakaian berlebih perlu dicegah (Martin,
2005).
Larutan RL adalah larutan isotonis yang paling mirip dengan cairan ekstraseluler. RL
dapat diberikan dengan aman dalam jumlah besar kepada pasien dengan kondisi seperti
hipovolemia dengan asidosis metabolik, kombustio dan sindroma syok. Ringer asetat
memiliki profil serupa dengan Ringer Laktat. Tempat metabolisme laktat terutama
adalah hati dan sebagian kecil pada ginjal, sedangkan asetat dimetabolisme pada hampir
seluruh jaringan tubuh dengan otot sebagai tempat terpenting.
Electric burns, which are most common in men between 20 and 40 years of age, constitute
20% of burn-related mortalities. It occurs by electric current or lightning strike. Low-voltage
electrical burns are considered to be less than 1000 volts and high-voltage electrical burns
are considered to be more than 1000 volts; electrical burns between 250 and 1000 volts
should be followed up just like high-voltage electrical burns since these patients may
develop unconsciousness, compartment syndrome, and myoglobinuria/hemoglobinuria.
In low-voltage accidents, burns are limited on the skin, however, go down into deeper
tissues. In high-voltage accidents, there are traces just like stapler pierce, ulceration, and
scarring. In the lightning strike, necrotic areas start from where the current entered and
progress along the line. As a result of direct contact with the electricity, systemic
complications such as cardiac arrhythmia, necrotic areas in the soft tissues and bones may
develop as well as thermal damage as the current passes through the whole body.
Radiation burns It is caused by the uptake of radioactive material. The local radiation burns
caused by high radiation doses (8–10 Gy) are similar to thermal burns except for several days
to weeks of delayed latency. The degree of radiation damage depends on the dose.
Erythema on the skin is the earliest finding. After weeks of exposure to highdose radiation,
necrosis and ulceration of the skin may develop.
- Sunburns
It develops due to uncontrolled and prolonged exposure to sun or light sources containing
UVB. Sunburn is the contact dermatitis due to ultraviolet B rays (295–315 nm), which is the
most erythematous wavelength. While 20 minutes is enough to get a minimal erythematous
dose (MED) in a clear summer day, 1 full day sunbathing is needed to reach 20 times the
MED dose. People reaching this dose have sunburns with individual differences. The skin
reaction starts at 4–6 hours and ends at 72 hours.
