B. Prosedur
Procedure
1. Pastikan bahwa mesin hemodialisis disiapkan sebelum kedatangan pasien di area
perawatan. Sedapat mungkin memastikan mesin hemodialisis diposisikan di sisi
yang sama dari akses CVC pasien.
Ensure that the haemodialysis machine is prepared before patient’s arrival in the
treatment area. Wherever possible ensure the haemodialysis machine is positioned
on the same side of the patient’s CVC access.

4. Lakukan penilaian dan pengamatan pada pasien dan dokumentasikan
Attend to patient assessment and observations, and documentation
5. Lihat rencana perawatan pasien / catatan medis untuk informasi yang
terdokumentasi mengenai akses CVC pasien.
Refer to patient care plan/medical notes for documented information regarding the
patient’s CVC access.
6. Lakukan cuci tangan

7. Kumpulkan peralatan, pastikan semua item diperiksa untuk masa kadaluarsa
Collect equipment, make sure all items are checked for expiration

10. Bersihkan permukaan troli secara menyeluruh menggunakan lap desinfektan sekali
pakai. Permukaan harus dibiarkan kering (30 detik) sebelum meletakkan benda apa
pun di atasnya.
Thoroughly clean surface trolley field using single use disinfectant wipe. The
surface must be allowed to dry (30 seconds) before placing any items on it.
11. Buka semua peralatan tanpa kontaminasi, pastikan bagian-bagian penting tidak
tersentuh dan tetap terlindungi (kasa steril dibiarkan terbuka dan syringe
dimasukkan kembali ke dalam bungkus asli) setelah pasien duduk di kursi / tempat
tidur dialisis mereka
Open all the equipment without contamination, ensuring key parts are not touched
and remain protected (sterile gauze left in open pack and syringes placed back into
original pack) once the patient is their dialysis chair/bed
12. Ambil NaCl 0.9% dari bloodline port Vena/ arteri dalam syringe 10 ml dan
letakkan kembali ke bagian steril
Take 0.9% NaCl from the venous bloodline / artery in the 10 ml syringe and place
it back into the sterile part

13. Siapkan pasien seperti dibawah ini

Prepare the patient as bellow:
 Posisikan pasien (berbaring posisi semi Fowler atau datar jika pasien tidak
dapat mentoleransi posisi)
Position the patient (lying semi Fowler position, or flat if patient can’t
tolerate position)
 Tidak menggunakan bantal (untuk mencegah emboli udara) jika
memungkinkan
No used pillows (to prevent air embolism) if possible
 Minta pasien untuk tidak berbicara selama koneksi / pemutusan (karena risiko
infeksi)
Ask the patient not to talk during connection/disconnection (due to risk of
infection)
 Berikan penutup rambut (jika perlu) dan masker kepada pasien (jika pasien
tidak dapat memalingkan muka) dan perintahkan pasien untuk
mengenakannya
Give a hair cover (if required) and mask to the patient (if patient is unable to
turn head away) and instruct patient to put them on
14. Mempersiapkan pasien untuk koneksi
Prepare patient are for connection
 Pastikan bahwa mesin atau peralatan tidak menghalangi akses ke pasien dan
peralatan dapat diakses.
Ensure that the machine or the equipment does not obstruct access to the
patient and that equipment is accessible.
15. Lakukan cuci tangan
Perform Hand Hygiene
16. Gunakan masker dan lakukan HH
Don mask and Perform HH
17. Gunakan sarung tangan no steril
Don non sterile gloves
18. Periksa dan observasi jalur CVC (luka) dan kulit sekitarnya tanpa ada balutan. Jika
menunjukkan tanda-tanda infeksi segera laporkan ke kepala perawat dan dokter
Check and observe the CVC pathway (wound) and surrounding skin without any
dressing. If you show signs of infection, immediately report it to the head of the
nurse and doctor
19. Tempatkan alas pelindung di bawah akses CVC. Instruksikan pasien untuk
menoleh ke sisi yang berlawanan dari tempat kateter berada
Place protective sheet under CVC access. Instruct the patient to turn their head to
the opposite side to where the catheter is located
20. Gunakan satu lumen kateter pada waktu berbeda dengan menggunakan teknik
aseptik, pegang lumen kateter arteri dengan satu tangan menggunakan sarung
tangan
Use one catheter lumen at different times using aseptic techniques, holding the
arterial catheter lumen with one hand using a gloves
21. Gunakan 0,5-2% Chlorhexidine / 70% tisu Alkohol atau kain kasa yang dibasahi
dengan desinfektan, bersihkan bagian luar lumen dan tutup CVC, bersihkan
setidaknya 30 detik semua area hub dan lumen. Pastikan lumen tidak bersentuhan

dengan apapun. Biarkan mengering selama 30 detik.

Use 0.5-2% Chlorhexidine / 70% tissue Alcohol or gauze moistened with
disinfectant, clean the outside of the lumen and lid of the CVC, clean at least 30
seconds in all areas of the hub and lumen. Make sure the lumen doesn't touch
anything. Let it dry for 30 seconds.
22. Lepaskan tutup CVC menggunakan Teknik aseptik
Remove the CVC cap using aseptic Technique
23. Pastikan hub CVC tidak menyentuh permukaan apa pun
Ensure the CVC hub does not come into contact with any surface.
24. Pastikan tidak ada larutan desinfektan memasuki lumen kateter, hanya bagian luar
hub yang bersih
Ensure no disinfectant solution enters the catheter lumen, only clean exterior of
hub

25. Pastikan ujung hub tidak menyentuh permukaan apa pun.

Ensure the hub tip does not come into contact with any surface.
26. Segera pasang klep steril dengan syringe 3 ml kosong (menggunakan Teknik
aseptik dan tarik 3 ml larutan dari CVC
Immediately attach an empty sterile 3 ml syringe (using aseptic Technique) release
clamp and withdraw 3 ml of solution from CVC
27. Klep kateter dan lepaskan jarum suntik 3ml lalu buang
Clamp catheter and remove 3ml syringe and discard
28. Segera berikan 0,9% NaCl pada jarum suntik 10ml (menggunakan Teknik aseptik),
lepaskan klem, flush lumen dan klem
Immediately give 0.9% Sodium Chloride to the 10ml syringe (using Non Touch
Technique), remove the clamp, flush the lumen and clamp
29. Setelah diklem, biarkan jarum suntik terhubung ke lumen CVC
After clamping leave the syringe connected to the CVC lumen
30. Ulangi prosedur untuk lumen kateter vena
Repeat the procedure for venous catheter lumen.
31. Gunakan teknik aseptik untuk membuka syringe10 ml jarum suntik dan
hubungkan bloodline arteri dan vena (satu demi satu) ke lumens kateter. Pastikan
semua koneksi (kateter lumens / bloodline) aman dan diposisikan dengan benar.
Buka semua klem dan mulai dialisis sesuai dengan.
Using aseptic technique to open 10 ml syringe and connect arterial and venous
bloodlines (one after the other) to the catheter lumens. Ensure all connections
(catheter lumens/bloodlines) are secure and correctly positioned. Open all clamps
and commence dialysis.
32. Lepaskan sarung tangan, buang dalam sampah medis
Remove gloves, dispose in medical waste

34. Dokumentasikan semua masalah CVC ke dalam rekam medis, parameter mesin
dan observasi pasien
Document any CVC problems into medical record, machine parameters and
patient observations
35. Bersihkan permukaan baki / troli
Disinfect clean the tray/trolley
36. Pastikan bahwa pasien aman dan nyaman, sebelum pindah.
Make sure that the patient is safe and comfortable, before moving away.
