B. Prosedur
Procedure
1. Jelaskan prosedur kepada pasien
Explain procedure to patient
2. Kumpulkan peralatan dan barang habis pakai, pastikan bahwa semua barang
diperiksa tanggal kedaluwarsa
Collect equipment and consumables, ensuring that all items are checked
expiry date.
3. Pastikan mesin atau peralatan lain tidak menganggu akses ke pasien
Ensure the machine or other equipment does not interfere with access to the
patient
4. Gunakan apron dan masker, kemudian lakukan cuci tangan
Don Apron and face shield, then Perform Hand Hygiene Gunakan sarung
tangan non steril
Don non sterile gloves
5. Selesaikan hemodialisis pasien dan masukkan kembali darah dari
ekstrakorporeal kedalam tubuh pasien, pastikan dialyser benar-benar terkuras
dan port dialisat ditempatkan kemblai ke mesin.
Complete haemodialysis patients and reinsert it into the extracorporeal
blood from the patient's body, make sure dialyser completely drained and
dialysate ports were placed back into the machine.
6. Pastikan CVC pasien dan bloodline diklem
Ensure patient’s CVC and bloodlines are clamped.
7. Lepaskan sarung tangan dan lakukan cuci tangan
Remove gloves and Perform Hand Hygiene
8. Bersihkan permukaan area Aseptik secara menyeluruh menggunakan lap
desinfektan sekali pakai. Permukaan harus dibiarkan kering (30 detik)
sebelum menempatkan benda apa pun.
Thoroughly clean surface of Aseptic area using single use disinfectant wipe.
The surface must be allowed to dry (30 seconds) before placing any items.
9. Buka semua kebutuhan tanpa kontaminasi, pastikan bagian-bagian penting
tidak tersentuh dan tetap terlindungi (kasa steril dibiarkan terbuka dan
syringe ditempatkan kembali ke bungkus asli)
Open all needs without contamination, make sure the important parts
untouched and remain protected (sterile gauze is left open and the syringe
placed back into the original wrap)
10. Tarik NaCl dari mesin dialisis melalui set infus atau ampul ke dalam jarum
suntik 10ml dan letakkan jarum suntik 10ml kembali ke area steril. Ulangi
untuk jarum suntik 3ml jika menggunakan larutan pengunci (sesuai dengan
resep pasien). Tuangkan larutan desinfektan (0,5% -2% Chlorhexidine
dengan alkohol 70%) ke kain kasa steril
Withdraw N/Saline from dialysis machine via infusion set or ampoule into
10ml syringe and place 10ml syringes back into packet. Repeat for 3ml

syringe if using locking solution (according to patient prescription). Pour the

disinfectant solution (0,5%-2% Chlorhexidine with 70% alcohol) onto sterile
gauze.
11. Siapkan pasien seperti dibawah:
Prepare the patient as bellow:
 Posisikan pasien (berbaring posisi semi Fowler, datar jika pasien dapat
mentoleransi posisi)
Position the patient (lying semi Fowler position, flat if patient can
tolerate position)
 Jangan ada bantal (untuk mencegah emboli udara) jika memungkinkan
No pillows (to prevent air embolism) if possible
 Minta pasien untuk tidak berbicara selama koneksi / pemutusan (karena
risiko infeksi)
Ask the patient not to talk during connection/disconnection (due to risk
of infection)
 Berikan penutup rambut (jika perlu) dan masker kepada pasien (jika
pasien tidak dapat memalingkan muka) dan perintahkan dia untuk
mengenakannya
Give a hair cover (if required) and mask to the patient (if patient is
unable to turn head away) and instruct him/her to put them on
12. Mempersiapkan area pasien untuk pemutusan
Prepare patient area for disconnection
 Posisi di kursi / tempat tidur pasien
Positioning at patient chair/bed.
 Memastikan bahwa mesin atau peralatan tidak menghalangi akses ke
pasien dan peralatan itu dapat diakses
Ensuring that the machine or the equipment does not obstruct access to
the patient and that equipment is accessible.
13. Lakukan cuci tangan
Perform Hand Hygiene
14. Gunakan sarung tangan non steril
DON non sterile gloves
15. Tempatkan alas pelindung di bawah akses CVC. Instruksikan pasien untuk
menoleh ke sisi yang berlawanan dimana tempat kateter berada
Place protective sheet under CVC access. Instruct the patient to turn their
head to the opposite side to where the catheter is located
16. Gunakan satu lumen kateter pada waktu berbeda dengan menggunakan teknik
aseptik, pegang lumen kateter arteri dengan satu tangan menggunakan sarung
tangan
Use one catheter lumen at different times using aseptic techniques, holding
the arterial catheter lumen with one hand using a gloves.
17. Putuskan sambungan bloodline dari CVC menggunakan Teknik aseptik
Disconnect the bloodline from the CVC using aseptic technique
18. Pastikan hub CVC tidak menyentuh permukaan apa pun.
Ensure the CVC hub does not come into contact with any surface.
19. Gunakan 0,5-2% Chlorhexidine / 70% tisu Alkohol atau kain kasa yang
dibasahi dengan desinfektan, bersihkan bagian luar lumen dan tutup CVC,

bersihkan setidaknya 30 detik semua area hub dan lumen. Pastikan lumen
tidak bersentuhan dengan apapun. Biarkan mengering selama 30 detik.
Use 0.5-2% Chlorhexidine / 70% tissue Alcohol or gauze moistened with
disinfectant, clean the outside of the lumen and lid of the CVC, clean at least
30 seconds in all areas of the hub and lumen. Make sure the lumen doesn't
touch anything. Let it dry for 30 seconds.
20. Pastikan tidak ada larutan desinfektan memasuki lumen kateter, hanya bagian
luar hub yang bersih.
Ensure no disinfectant solution enters the catheter lumen, only clean exterior
of hub.
21. Pastikan ujung hub tidak menyentuh permukaan apa pun
Ensure the hub tip does not come into contact with any surface.
22. Segera memberikan NaCl 0,9% yang diisi dalam 10 ml syringe
(menggunakan teknik aseptik), lepaskan klem, siram lumen, dan klem
Immediately attach 0,9% NaCl filled in 10ml syringe (using NTT), release
clamp, flush lumen and clamp
23. Lepaskan jarum suntik 10ml kosong dan pasang jarum suntik 3ml diisi
dengan cairan pengunci - sesuai dengan resep pasien. Lepaskan penjepit
Remove empty 10ml syringe and attach 3ml syringe filled with locking
solution – according to patient prescription. Release clamp
24. Lepaskan jarum suntik 3ml dan pasang tutup baru kateter
Remove 3ml syringe and attach new catheter caps
25. Ulangi prosedur untuk lumen kateter vena.
Repeat the procedure for venous catheter lumen.
26. Lepaskan dan buang bloodline segera ke tempat sampah klinis atau gunakan
kembali ember yang siap untuk diproses ulang dialyzer
Remove and dispose of bloodlines immediately into clinical waste bin or
reuse bucket ready for dialyzer reprocessing
27. Lepaskan sarung tangan dan masker, lakukan Kebersihan Tangan
Remove gloves and face shield perform Hand Hygiene
28. Observasi pasien dan masalah CVC yang harus diselesaikan dan
didokumentasikan
Patient observations and any CVC problems to be completed and
documented
29. Bersihkan permukaan baki / troli / area kerja
Disinfect clean the tray / trolley / work surface
30. Lepaskan dan buang gaun / apron
Remove and dispose of gown /apron
31. Lakukan Kebersihan Tangan saat selesai
Perform Hand Hygiene on completion
