Mutu Pelayanan Kesehatan JKN-KIS Jangan Diabaikan

Jakarta (11/02/2022) – Implementasi Kelas Rawat Inap Standar (KRIS) penyelenggaraan program
Jaminan Kesehatan Nasional – Kartu Indonesia Sehat (JKN-KIS) terus menjadi pembahasan. Direktur
Utama BPJS Kesehatan, Ghufron Mukti menyebut terdapat beberapa tantangan yang dihadapi dalam
rencana pemberlakuan KRIS JKN.

“Dalam kondisi saat ini di mana aset netto BPJS Kesehatan sudah positif, fokus kami adalah
bagaimana meningkatkan mutu layanan. Ini sejalan dengan harapan peserta yg menginginkan
adanya standar mutu layanan agar prinsip ekuitas bisa dipenuhi. Jangan sampai penerapan KRIS ini
jika tidak disiapkan dengan baik akan mengurangi mutu layanan,” kata Ghufron dalam sesi webinar
Rapat Kerja Nasional Persi Tahun 2022 dengan tema Tarif Jaminan Kesehatan Nasional (JKN), Kelas
Standar dan Mutu, Jumat (11/02).

Dalam penerapan KRIS, Ghufron mengatakan kesiapan fasilitas kesehatan khususnya rumah sakit
juga harus terus diupayakan. Menurutnya, ada beberapa indikator yang harus didorong bagi rumah
sakit, khususnya dalam pemenuhan ketersediaan tempat tidur, pemenuhan dokter spesialis atau
subspesialis, hak atas obat hingga visitasi dokter harus berjalan dengan baik.

"Kami melihat bahwa tren kerja sama dengan rumah sakit terus mengalami peningkatan setiap
tahunnya. Begitu juga diimbangi dengan rasio kecukupan tempat tidur perawatan secara nasional
relatif cukup. Untuk itu, diharapkan dukungan kepada seluruh pihak dalam pemenuhan infrastruktur,
SDM dan alat kesehatan di fasilitas kesehatan sehingga mutu layanan tetap terjaga," tambah Ghufron.

Sementara itu, Anggota DJSN, Muttaqien menyebut ada perubahan tentang kelas standar PBI dan
non-PBI. Dirinya juga mengatakan rencana implementasi KRIS akan dilakukan secara bertahap.
Dirinya menyebut, ada 12 kriteria yang akan menjadi indikator dalam penerapan KRIS tersebut. Dari
kriteria tersebut, pihaknya terlebih dahulu akan melakukan ujicoba ke rumah sakit vertikal, rumah sakit
daerah dan rumah sakit swasta.

“Di tahun 2022 ini, DJSN bersama Kementerian Kesehatan dan BPJS Kesehatan akan melakukan
ujicoba penerapan KRIS. Harapannya asosiasi rumah sakit, khususnya PERSI dapat mendukung
upaya tersebut untuk melihat bagaimana penerapannya sehingga ditemukan apa yang harus
dilakukan dan diperbaiki dalam penerapan KRIS JKN ini,” jelas Muttaqien.

Dalam kesempatan yang sama, Direktur Jenderal Pelayanan Kesehatan Kementerian Kesehatan,
Prof. dr. Abdul Kadir, Ph.D. Sp.THT-KL(K) M.A.R.S mendukung pernyataan Direktur Utama BPJS
Kesehatan bahwa mutu dan kualitas pelayanan merupakan hal yang sangat penting apabila akan
mulai diterapkan KRIS JKN. Menurutnya, untuk meningkatkan mutu dan kualitas pelayanan, seluruh
pihak harus melakukan transformasi terhadap infrastuktur dan suprastruktur.

“Sebagus apapun infrastruktur rumah sakit tanpa didukung adanya suprastruktur, dari sistem hingga
sumber daya manusia yang baik pula maka tidak akan bisa meningkatkan mutu. Saat ini untuk tenaga
kesehatan, khususnya bagi dokter spesialis sangat kekurangan. Oleh karena itu, kami meminta
kepada lembaga pendidikan untuk menambah kuota dalam memproduksi lulusan terbaik kedokteran
sehingga penyebaran tenaga kesehatan ke daerah semakin baik,” kata Kadir.

Rencana implementasi KRIS masih perlu dimatangkan. Hal ini sesuai dengan kesimpulan Rapat
Dengar Pendapat (RDP) Komisi IX DPR RI yang mendesak DJSN untuk bersama-sama dengan
Kementerian Kesehatan RI, BPJS Kesehatan, dan asosiasi rumah sakit secara kontinu untuk
memperjelas kesepakatan definisi dan kriteria KRIS dengan mempertimbangkan infrastruktur,
Sumber Daya Manusia Kesehatan (SDMK), dan alat kesehatan yang dimiliki oleh fasilitas kesehatan
serta menyusun pentahapan implementasi KRIS dan mitigasi risikonya melalui peta jalan secara rinci.

The Quality of JKN-KIS Health Services Cannot Be Overlooked

Jakarta (11/02/2022) – Implementation of the Standard Inpatient Class (KRIS) for the implementation
of the National Health Insurance – Healthy Indonesia Card (JKN-KIS) program continues to be
discussed. The President Director of BPJS Kesehatan, Ghufron Mukti said that there were several
challenges in the plan to implement KRIS.

"In the current condition where BPJS Kesehatan's net assets are positive, our focus is on how to
improve the service quality. This is in line with the expectations of participants who want standards in
service quality. Therefore, the equity principle can be fulfilled. We shouldn’t let the implementation of
KRIS go unprepared and reduce the quality of services," said Ghufron in the webinar session of the
2022 Persi National Working Meeting with the theme National Health Insurance Tariff (JKN), Standard
and Quality Class, Friday (11/02).

Ghufron said that health facilities especially the hospitals need to make preparations so that they are
ready to implement KRIS. According to him, there are several indicators that must be improved for
hospitals, especially in the fulfilment of the availability of beds, specialist or subspecialist doctors, the
right for medicine and doctor visits.

"We see that the trend of cooperation with hospitals continues to increase every year. As well as
balanced with the ratio of care beds nationally which is relatively sufficient. For this reason, we hope
for the support from all parties in the fulfilment of infrastructure, human resources and medical
equipment in health facilities. Hence, the service quality can be maintained," added Ghufron.

Meanwhile, DJSN Member, Muttaqien said that there was a change in the standard classes of PBI
and non-PBI. He also said that the KRIS implementation plan would be carried out in stages. He said
there were 12 criteria that would be indicators in the implementation of the KRIS. From these criteria,
his party will first conduct trials to vertical hospitals, regional hospitals and private hospitals.

"In 2022, DJSN with the Ministry of Health and BPJS Kesehatan will pilot the application of KRIS. It is
hoped that hospital associations, especially PERSI, can support this effort to supervise the
implementation. Therefore, we can find what must be done and improved in the implementation of the
JKN KRIS," explained Muttaqien.

On the same occasion, the Director General of Health Services at the Ministry of Health, Prof. dr.
Abdul Kadir, Ph.D. Sp.THT-KL(K) M.A.R.S supports the statement of BPJS Kesehatan President
Director. The service quality is very important in the implementation of JKN KRIS. According to him,
to improve the quality of service, all parties must transform the infrastructure and superstructure.

“No matter how good the hospital infrastructure is, but if it’s without the support of a superstructure,
from the system to good human resources, it will not be able to improve the quality. Currently, there is
a shortage of health workers, especially specialists. Therefore, we ask educational institutions to
increase their quota in producing the best medical graduates so that the distribution of health workers
to the regions is getting better," said Kadir.

KRIS implementation plan still needs to be finalized. This is in accordance with the conclusion of the
Commission IX DPR RI Hearing Meeting (RDP) which urged DJSN to work together with the
Indonesian Ministry of Health, BPJS Kesehatan, and hospital associations to continuously clarify the
agreement on the definition and criteria of KRIS by taking into account the infrastructure, human
resources (HR) and medical devices owned by health facilities as well as arranging the stages of KRIS
implementation and risk mitigation through detailed roadmaps.
