5.1.4. Staff GA bertanggung jawab untuk

dibuat QHSE.
5.1.5. Staff GA bertanggung jawab dalam
penyerahan APD kepada Divisi HRD &
Crewing, baik untuk kebutuhan di Jakarta
maupun di kantor cabang / luar kota.
5.1.6. Divisi HC&BD wajib memiliki stock harian yang
disimpan di lokasi Divisi HC&BD, minimal 5
pasang untuk masing‐masing jenis PPE
dengan ukuran standard.
5.1.7. Staff GA wajib segera membuat Purchase
Request ke Divisi Purchasing, apabila stock
APD tidak mencukupi untuk masa 1 bulan
kedepan (stock minimal).
5.2. Purchasing, dan Logistik
5.2.1. Divisi Purchasing bertanggung jawab
melakukan pembelian PPE sesuai dengan
order yang diberikan oleh GA.
5.2.2. Divisi Purchasing wajib memonitor
pemenuhan ordernya dari vendor agar stock
dapat segera ditambah / diisi.
5.2.3. Staff Logistic menerima APD yang telah dibeli
oleh Divisi Purchasing. Staff Logistic wajib
membubuhkan tanda tangan, nama dan
tanggal sebagai bukti penerimaan serta
konfirmasi atas kebenaran jumlah, jenis dan
ukuran PPE yang diterima
5.3. QHSE
5.3.1. Divisi QHSE bertanggung jawab dalam
melakukan pencatatan untuk memonitor
penggantian APD untuk Karyawan & Crew.

5.1.4. GA staff is responsible for providing and
controlling the adequacy of PPE stock. Number
of stock at any time at least sufficient to meet
the needs of the next month, according to the
data updates are made every month by QHSE.
5.1.5. GA staff responsible for the distribution of the
PPE to the Division HRD & Crewing , both for
Jakarta and branch office / out of town.
5.1.6. HC&BD Division should have daily stock stored
in HC&BD Division location, at least 5 pairs for
each type of PPE with standard size .
5.1.7. GA staff shall immediately make a Purchase
Request to the Division of Purchasing, if PPE is
not sufficient stock for the next month
(minimum stock).
5.2. Purchasing and Logistics
5.2.1. Purchasing Division is responsible for the
purchase of PPE in accordance with the orders
given by the GA .
5.2.2. Purchasing Division shall monitor the order
fulfillment from the vendor so that the stock
can be immediately added / completed .
5.2.3. Logistic Staff received PPE that has been
purchased by the Purchasing Division. Logistic
Staff shall give the signature , name and date
as proof of receipt and confirmation of the
correctness of the amount , type and size of
PPE received.
5.3. QHSE
5.3.1. QHSE Division is responsible for monitoring the
recording for replacement PPE for Employees &

5.4. Operational
5.4.1. Divisi Operational bertanggung jawab
memastikan bahwa semua Crew di kapal telah
menggunakan APD sesuai dengan standard
yang ditetapkan.
5.4.2. Divisi Operational wajib mengingatkan Crew
agar selalu menggunakan APD dan
memeliharanya dengan baik.
5.5. Karyawan dan Crew
5.5.1. Karyawan atau Crew yang menerima APD
wajib membaca isi pada Formulir Penerimaan
PPE FM‐OS‐QHSE‐12.01 sebelum
menandatanganinya sebagai bukti bahwa ia
telah memahami isinya dan sanggup
memenuhinya.
5.5.2. Nakhoda bertanggung jawab untuk
memastikan personil kapal (crew)
menggunakan APD yang sesuai sebelum
memulai pekerjaannya.
5.5.3. Semua personil kapal (crew) harus
menggunakan APD sebelum bekerja dan
mengetahui tata cara pemakaian serta
bertanggung jawab atas perawatan dan
pemeliharaan PPE miliknya.
5.5.4. Karyawan atau Crew bertanggung jawab
sepenuhnya atas penggantian kehilangan atau
kerusakan APD (kecuali karena rusak secara
alami) sesuai dengan harga yang tertera pada
Formulir Penerimaan APD (FM‐OS‐QHSE‐

5.4. Operational
5.4.1. Operational Division is responsible for ensuring

to read the contents of Acceptance Form of
PPE FM‐OS‐QHSE‐12.01 before signing as proof
that he understood its contents and able to

5.5.2. The master is responsible for ensuring
personnel aboard (crew) using the appropriate
PPE before work.
5.5.3. All personnel aboard (crew) must use PPE
before work and know the usage procedures
and is responsible for the care and
maintenance of PPE.
5.5.4. Employee or Crew is responsible for any loss or
damage replacement PPE ( except for broken
naturally ) according to the price listed on the
Acceptance Form of PPE (FM‐OS‐QHSE‐12.01).
Implementation of the replacement will be
done by cutting the salaries of employees or
crew concerned by HR based on information
from the QHSE.

7.2 Jenis APD
7.2.1. Pakaian Kerja (Wearpack / Overall)

2. Coverall wajib digunakan terutama bagi
personil yang bekerja di lapangan.

mewajibkan crew untuk mengenakan
coverall.
4. Coverall harus dibuat pas di badan.

7.2.1. Work Clothes ( Wearpark / Coverall )

employees, which will be equated from the

personnel who work on site

a coverall.
4. Coverall should be made fits in the body.

made from cotton because it is more

Usage Glasses

1. Get used to install and remove the glasses

Pemakaian Respirator catridge
1. Letakkan pengikat kepala pada dudukan
di bagian belakang kepala (gb.1).
2. Tarik tali ke arah kiri dan kanan, lalu
posisikan respirator pada muka menutupi
mulut dan hidung (gb. 2).
3. Setelah posisi respirator menutupi mulut
dan hidung, tarik tali ke bagian leher
belakang lalu pasangkan tali jadi satu dan
tarik bagian belakang tali (kekiri dan
kekanan) untuk mengatur kekuatan dari
tali pengikat (gb. 3).
4. Lakukan pemeriksaan tekanan negatif
dengan meletakkan telapak tangan
dibagian lingkaran atas yang terbuka
diatas catridge. Lalu perlahan‐lahan tarik
nafas, jika respirator memendek sedikit
dan tidak terdapat udara antara muka
dengan respirator, maka pemasangan
respirator sudah benar (gb. 4).
5. jika pemakai tidak dapat memakai
respirator dengan benar, jangan masuk ke
daerah kontaminasi, dan beritahu atasan
atau QHSE Officer.
Perawatan Respirator catridge
1. Lepaskan setiap bagian respirator, bagian
Catridge, pengikat kepala, katup
pernafasan keluar, dan katup pernafasan
kedalam (Catridge Tidak Boleh dicuci).
Bersihkan dan desinfeksi bagian
respirator dengan cairan sabun lalu
gosok perlahan‐lahan. Jangan gunakan
pembersih yang mengandung minyak.
(Catatan: Suhu air tidak boleh melebihi
400 C).
2. Basuh dengan air bersih lalu letakkan di
daerah yang tidak terkontaminasi sampai
kering.
3. Jangan menaruh respirator catridge di
sembarang tempat, terutama di area‐

Respirator Cartridge Usage
1. Place the fastener head on the stand at the
back of the head (gb.1).
2. Pull the rope to the left and right, and then
position the respirator on the face covering
the mouth and nose (Fig. 2).
3. Once the position respirator over his mouth
and nose, pull the rope to the back of the
neck and then attach the rope together and
pull the back strap (left or right) to adjust
the strength of the strap (Fig. 3).
4. Perform a negative pressure by putting
your palms on the open loop section above
the cartridge. Then slowly inhale, if
respirator shortened a bit and there is no
air between the face of the respirator, the
respirator installation is correct (Fig. 4).
5. If the user is not able to wear a respirator
correctly, do not go into areas of
contamination, and tell your supervisor or
QHSE Officer.
Care Respirator Cartridge
1. Remove each piece respirator, part of
Cartridge, fastener heads, valves breathing
out and breathing valve into (Cartridge Not
Allow washed). Clean and disinfected
respirator with liquid soap and rub gently.
Do not use cleaners that contain oils. (Note:
Water temperature should not exceed 400
C).
2. Rinse with clean water and then place in
areas that are not contaminated until dry.
3. Do not put the respirator cartridge in any
place, especially in areas that contain many

Pemakaian Masker Kimia
1. Letakkan posisi masker menghadap ke
atas di tangan anda, dengan bagian
hidung masker berada diujung jari dan
tali kepala dibiarkan tergantung di
bawah tangan. (gb.1).
2. Tarik tali atas dan letakkan ke bagian
kepala bagian atas, sedangkan tali
bawah tarik melewati kepala sampai
pada bagian bawah telinga anda (gb. 2).
3. Letakkan ujung jari tangan anda diatas
logam pada bagian hidung masker.
Bentuk bagian logam sesuai dengan
bentuk hidung anda dengan menekan ke
dalam sambil menggerakkan ujung jari
anda ke bawah di kedua sisi bagian
hidung masker (gb. 3).
4. Pemasangan masker pada wajah
haruslah dicek apakah sudah tepat dan
benar‐benar pas, pada setiap kali
pemakaian. Untuk mengecek apakah
pemakaian sudah tepat, tempatkan
kedua tangan menutupi masker dan
keluarkan nafas. Jika udara masih
mengalir disekitar hidung sesuaikan lagi
bagian hidung masker dengan bentuk
hidung anda. Jika udara masih tetap
bocor pada tepian masker, perbaiki
posisi tali masker di kepala (gb. 4)
Perawatan Masker Kimia
1. Biasakan memasang dan melepas
masker dengan benar.
2. Simpanlah masker ditempat yang bersih
(tempat penyimpanan alat pelindung
diri yang disediakan).

Use of Masks Chemistry
1. Place the mask face‐up position on your
hand, with the nose tip of the finger and
the mask was left dangling head strap
under the arm. (Gb.1).
2. Pull the rope up and put to the top of the
head, while the bottom rope to pull over
your head on the bottom of your ear (Fig.
2).
3. Place the tip of your fingers over the metal
on the nose mask. Shape metal parts in
accordance with the shape of your nose by
pressing into while moving your finger
down on either side of the nose mask (Fig.
3).
4. Installation on a face mask must be
checked whether it is appropriate and
completely fit, at any time of usage. To
check whether the user is right, place your
hands over the mask and exhale. If the air is
still flowing around the nose adjust again
its nose mask with the shape of your nose.
If the air is still leaking at the edges of the
mask, the mask strap in the correct position
of the head (Fig. 4)

Pemakaian Earplug
1. Agar memasukkan earplug lebih mudah
dan sempurna, tarik telinga anda ke
arah atas samping dan tempatkan
earplug di lubang telinga anda.
2. Masukkan satu persatu (bagian kanan
dan kiri) earplug ke lubang telinga anda,
dan tekan earplug sedalam mungkin ke
dalam telinga.
3. Pemasangan earplug dikatakan benar
apabila, seluruh earplug masuk ke dalam
telinga dan suara bising yang didengar
sudah berkurang atau tidak sekeras bila
tidak menggunakan earplug.
4. Pemasangan earplug dikatakan tidak
benar apabila, bagian earplug tidak

Usage Earplug
1. In order to enter more easily and perfect
earplug, pull your ear toward the top side
and place the earplug in your ear hole.
2. Insert one by one (right and left) earplug
into your ear hole, and press earplug deep
as possible into the ear.
3. Installation earplug said to be true if, the
entire earplug into the ear and audible
noise is reduced or not as hard as when not
using earplugs.
4. Installation is not correct if the earplug is
said, does not make it all part earplug into
the ear, and still be heard loud noises or no
